파킨슨 질환은 신경퇴행성 질환 중 두 번째로 많이 발생하는 질환으로서 특히 세계 노인성 뇌신경질환 치료제 시장에서 1위에 해당할 정도로 최근 유병률이 급격히 증가하는 질환이다. 아직 명확한 발병기전이 밝혀지지 않았으며, 현재 치료방법으로는 일차적으로 소실되는 도파민성 신경전달물질을 외부에서 보충하고자 하는 시도로써 도파민 전구체인 레보도파 혹은 다양한 도파민성 수용체에 결합능을 가진 도파민 agonist가 사용되고 있으나, 이는 증상을 완화 시키는 효과를 지니며 파킨슨병의 진행을 근본적으로 변화시키지는 못하여 환치를 위한 치료제는
파킨슨 질환은 신경퇴행성 질환 중 두 번째로 많이 발생하는 질환으로서 특히 세계 노인성 뇌신경질환 치료제 시장에서 1위에 해당할 정도로 최근 유병률이 급격히 증가하는 질환이다. 아직 명확한 발병기전이 밝혀지지 않았으며, 현재 치료방법으로는 일차적으로 소실되는 도파민성 신경전달물질을 외부에서 보충하고자 하는 시도로써 도파민 전구체인 레보도파 혹은 다양한 도파민성 수용체에 결합능을 가진 도파민 agonist가 사용되고 있으나, 이는 증상을 완화 시키는 효과를 지니며 파킨슨병의 진행을 근본적으로 변화시키지는 못하여 환치를 위한 치료제는 없는 실정이다. 파킨슨 질환은 뇌신경 질환의 특성상 인간 세포를 이용한 in vitro 실험이 불가능하기 때문에 임상실험에 들어가서야 사람에 대한 실험이 이루어지고 있고, 초기 증상이 없어 임상에 적용되는 환자군은 이미 파킨슨 질환이 상당히 진행되어 있는 상태이기 때문에 실제 사람에게 적용 후 약효 혹은 독성 반응이 신약개발 단계에서 도출되는 결과와는 많은 차이가 있을 수밖에 없다. 따라서 이를 대체할 방법이 필요한데 이에 줄기세포가 대안으로 떠오르고 있다. 특히 배아줄기세포(hESC), 역분화줄기세포(iPSC), 핵치환 줄기세포 (NT-hESC)로 크게 분류되는 전분화능줄기세포(pluripotent stem cell)는 무한한 증식능력과 신체의 모든 장기 조직으로 분화할 수 있는 능력을 가지고 있어 줄기세포 치료제로서의 활용뿐만 아니라 인체조직과 매우 흡사하여 신약 개발단계에서 독성평가를 통한 동물실험을 대체하기 위한 노력이 많이 이루어지고 있다. 또한 줄기세포를 이용하여 임상을 대체할 수 있는 도구로서의 가능성을 보여주고 있는데 이는 인간의 모든 조직으로 분화할 수 있고, 최근 조직공학기술의 발달, organoid 기술의 발달 등으로 인하여 인간의 조직을 만들려는 시도들이 논문으로 발표되면서 새로운 화두로 떠오르고 있다.
따라서 본 연구에서는 환자에서 유래된 전분화능줄기세포를 확보하고 이를 유지 배양하며 특정 신경 세포로의 분화법 확립 및 임상평가 기반 모델을 마련하고자 한다. 역분화 줄기세포를 확보하기 위해 국가줄기세포은행으로부터 Sendai virus를 이용한 역분화줄기세포를 분양받고, ATCC로부터 동일한 바이러스를 이용한 환자유래 역분화줄기세포를 구매하였다. Xeno-free, feeder-free 조건하에서의 유지배양을 확립하였으며 Otc4, Nanog, Alkaline Phosphatase assay 등 미분화 유지 표지자들을 측정하여 미분화의 특성을 확인하였다. 이후 도파민 신경세포로의 분화법을 확립하기 위하여 monolayer culture, EB formation culture, 그리고, 분화 키트를 이용한 분화법을 이용하여 분화를 진행하였다. 각각의 단계에서 현미경을 이용한 육안(morphology) 확인을 거쳤으며, 최종 단계 분화 마커인 TH, NURR1 등을 PCR 방법을 통하여 확인하였다. 이를 통해 전분화능 줄기세포 유지배양 및 도파민 분비세포로의 분화를 확인하였으며 2 차년도 연구 사업을 통해 분화된 도파민 분비세포를 이용하여 파킨슨질환 치료제의 효과를 확인할 수 있는 도파민 분비능력을 확인하기 위하여 LC-MS 및 Dopamine sensitive ELISA를 이용하였고, 신경세포로서의 역할을 하는지를 확인할 수 있는 patch clamp를 이용한 전기신호 확인을 하였다. 이를 이용하여 파킨슨병 치료제의 약력학적 평가방법을 구축하였고, 현재 시판중인 2 개의 의약품(Levodopa, Apomorphine)에 대하여 약력학적 평가방법을 적용, 효능을 평가하였다.
위의 연구사업 내용을 통해 줄기세포를 이용한 의약품 약력학적 평가방법의 장단점을 확인할 수 있었고, 앞으로 임상 연구에 도움을 줄 수 있는 기반연구가 될 것이라 생각된다.

The pluripotent stem cells, which are largely classified into embryonic stem cells (hESCs), induced pluripotent stem cells (iPSCs), and nuclear transfer human embryonic stem cells (NT-hESCs), have infinite proliferating capacity and differentiate into all organs of the body and it is very similar to human tissue as well as its use as a stem cell treatment agent, and efforts are being made to replace animal experiments through toxicity evaluation at the stage of drug development. In addition, it shows the possibility of using stem cells as a tool to replace clinical trial, which can differentiate into all human tissues, respectively, development of tissue engineering technique, development of organoid technology, As attempts have been published in the paper, they are emerging as new issues.
In this study, we aimed to establish a model for establishing the differentiation and clinical evaluation of specific neurons by the pluripotent stem cells derived from the patient. In order to obtain samples, iPSCs using Sendai virus were distributed from the National Stem Cell Bank, and the patient derived iPSCs from the same virus were purchased from the ATCC and xeno-free, and feeder-free culture conditions were established. The undifferentiated pluripotent stem cell markers such as Otc4, Nanog, and Alkaline Phosphatase assays were assayed to confirm the characteristics of iPSCs. To differentiate into dopaminergic neurons, differentiation was performed using monolayer culture, EB formation culture, and differentiation kit.
At each stage, the morphology was verified using a microscope, and the final stage differentiation markers TH and NURR1 were confirmed by PCR. This study confirmed the maintenance culture of pluripotent stem cells and their differentiation into dopamine-secreting cells.
To confirm the efficacy of Parkinson's disease drug, LC-MS and Dopamine sensitive ELISA were used to confirm the ability of dopamine secretion from dopaminergic neurons derived from human iPSCs, Also we confirmed cell signaling using a patch clamp to confirm its role as a neuron. We have constructed a pharmacodynamic evaluation method for the treatment of Parkinson 's disease and evaluated the efficacy of levodopa and apomorphine by using this evaluation method. The results of the above research project confirmed the advantages and disadvantages of the pharmacodynamic evaluation method of the drug using stem cells, and it will be the possible base data to be examined when the evaluation using the stem cell is made in the future when the drug approval data is submitted.
