3. Results
1) Current status of pediatric cancer survivorship program in foreign countries
◦ There are various types of follow-up programs centering on survivors.
◦ Key survices provided by LTFU programs should include: monitoring for and managing physical late effects, providing health education to survivors regarding their diagnoses, treatment exposures, and potential late effects, providing referrals to specialists and resources as indicated, encouraging wellness and health promotion activities, addressing psychosocial nees of survivors and affected family members, assessing and providing intervention for educational and/or vocational nees, assisting with financial and insurance issues, guiding transition from pediatric to adult-focused healthcare, empowering survivors to advocate for their own healthcare needs and facilitating survivorship research
2) Current status of support services for pediatric cancer survivors in Korea
◦ A survey on the awareness and needs of medical staff (doctors, nurses) who caring for pediatric cancer survivors
- Pediatric cancer survivors appear to continue to be treated by pediatric specialists even in adulthood, suggesting needs of the program for adult pediatric cancer survivors.
- Most pediatric cancer survivors continue to follow-up in institutions that received cancer treatment even after termination of treatment.
- Pediatric cancer survivors do not achieve developmental tasks during treatment. It suggests that the support program that takes into account the life cycle such as growth, development, adolescence, and schooling.
- An essential factor in the integrated support program for pediatric cancer survivors was the follow-up of late complications, education of cancer treatment summary and late complications, secondary cancer follow-up, psychological emotional support, return to school and learning support.
- The national health insurance support, the strengthening of professional education, and the support of personnel expenses are needed in order to expand the pediatric cancer survivor support programs.
◦ A Survey on the perception and needs of pediatric cancer survivors and families
- There are many willingness to participate in the program: to help the transition to adult-centered problems, to manage the late complications due to previous cancer treatment, to provide health education for my cancer, treatment, possible complications and to help return to school and learning.
- Pediatric cancer survivors want to continue to receive care from pediatric cancer survivor program until adulthood. However, in late cancer-related complication a transition to specialized programs for adult cancer survivors may be desired to manage late cancer-related complications in adulthood.
- Risk-based follow-up, community-based integrated support programs for pediatric cancer survivors are needed.
- Pediatric cancer survivors are experiencing physical and psychological complications due to their cancer experience, but they are growing through it. Most do not have any problems, but it is necessary to screen for survivors with problems.
3) Recommendation for pediatric cancer survivors integrated support service models, pilot project and operation plan, step-by-step development plan
- It should be made up of multi-disciplinary specialists and systematic linkage of service provision in hospitals and communities
- Based on the risk, and taking into account the life cycle of children and adolescents. It should provide integrated services that take into consideration the disease itself, treatment history, physical, psycho-social and economic problems, growth, development, school, occupation, lifestyle, genetic susceptibility, and family factors.
- it is necessary to start with small institutions, and to spread it with educational development of specialist for pediatric cancer survivors. It is necessary to start pediatric cancer survivors who have completed active cancer treatments, but to be able to intervene at the time of diagnosis, and to expand the age group and the subjects (parents, siblings, etc.)
- Development of the national health insurance support according to future models and Korean LTFU guidelines are required.
