연구의 목적 및 내용
최종 목표 :
혈액암에서 면역원성을 가진 신항원(neoantigen)을 동정하여, 신항원 기반 면역세포치료 개발 가능성을 탐색한다.

- 최근 유전체 정보를 기반으로 하여, 종양-특이적이면서 조직적합항원(MHC)에 결합하여 T 세포를 활성화시키는 ‘면역원성(immunogenecity)’을 가진 돌연변이를 ‘신항원(neoantigen)’이라 정의하며, 이러한 신항원이 항암 백신 또는 면역세포치료의 새로운 표적으로 대두됨.
- 현재까지 미국을 중심으로 신항원 연구가 진행되어 오면서 BCR/ABL, IDH1 등 몇 종의 신항원이 규명되어 있으나, 아직 치료제 개발로 이어지지는 못하였음. 방대한 유전체 정보를 활용하여 새로운 신항원을 찾을 가능성은 아직 많이 남아 있으며, 특히 혈액암에 대한 연구 결과는 매우 부족한 실정임.
- 국내 신항원에 대한 연구 수준은 유전자 정보의 컴퓨터 분석 수준에 머물러 있으며, 특히 혈액암에 대한 연구는 거의 없음.
- 따라서, 본 과제에서는 혈액암 환자에서 수집된 종양검체로 유전체 분석을 수행하고 이를 토대로 신항원을 선별해내고자 하며, 이를 바탕으로 신항원에 대한 면역세포치료제 개발 목적의 후속연구를 가능하게 하는 것이 목표였음.
연구 내용 :
- 혈액암, 특히 급성골수성백혈병 (AML)을 대상으로 종양 검체를 확보하고, targeted gene sequencing을 위한 panel을 마련, 이 정보를 바탕으로 HLA와 mutant peptide의 binding을 in silico prediction, 이를 통하여 neoantigen 가능성이 높은 유전자를 선별해냄.
- 본 연구를 통해 AML 환자에서 빈도가 높게 발견된 mutation은 CBL, NPM1, NRAS, FLT3, DNMT3A, IDH1/2 순서였으나, HLA binding prediction 결과 NPM1, DNMT3A, NRAS, IDH1/2 순으로 확인됨. --> 특히, 매우 중요한 예후인자로서 작용하는 NPM1 mutation이 빈도도 높고 HLA binding probability도 높아, neoantigen으로서 적절한 candidate라 판단되었음.
- Neoantigen 후보 mutant peptide의 면역원성을 확인하기 위해 ELISPOT을 새로이 셋팅하였으며, healthy donor의 mononuclear cell과 NPM1, IDH1/2간 반응성을 확인함. --> NPM1 peptide와의 반응성이 관찰되기는 하나 매우 약하였음. HLA의 다양성, binding affinity가 원인일 것으로 판단.
- 환자 검체를 획득하여 neoantigen 후보 peptide의 반응성을 확인하고자 했으나, 1차 년도에 선별하였던 대상자의 경과 악화로 추가 대상자 선정이 필요해짐. --> 현재 추가 선별, 대상자 동의획득 과정 중에 있음.
- AML 외 기타 혈액암 대상 연구의 일환으로, 희귀질환인 plasmablastic lymphoma 7례를 수집하여 이들의 분자적 특성을 파악하고자 함. --> 명단 확보, 동의획득 중에 있음. 11월 중 NGS 분석 계획임.

연구개발성과의 활용계획(기대효과)
- neoantigen은 개별 환자의 특정 HLA와 결합할 수 있는 mutation이 발생해야만 활용할 수 있을 것이므로, 세포치료에 활용하고자 한다면 그 대상자가 매우 한정적일 수 밖에 없을 것인데, 본 연구결과를 활용하여 AML환자에서 치료 또는 재발 방지 목적의 neoantigen-specific T cell therapy 개발을 계획해볼 수 있겠음.
- 본 연구에서는 주로 NPM1과 IDH1/2에 초점을 맞추었으나, 본 연구 결과를 바탕으로 다른 neoantigen (특히 기존 문헌에서 고형암 neoantigen으로 검증된 NRAS를 AML에서도 활용할 가능성)에 대한 연구도 가능할 것임.
- 또한, PBL과 같은 희귀질환, 특히 해외 data와 다른 특성을 가진 환자군 (HIV-negative, 2nd malignancy)에서의 NGS 분석 자료는 매우 귀중한 정보이며, 이 질환에 대한 이해를 높이고 새로운 진단/치료 전략 모색에 기여할 것임.

- Recently, tumor-specific antigen which can be expressed by antigen-presenting cells and induce T cell activation, so called ‘neoantigen’, is also emerging as a novel therapeutic target.
- Previous studies identified that several genetic alteration such as BCR/ABL fusion, IDH1 mutation has immunogenicity compatible with neoantigen. However, immune cell therapy targeting these neoantigens has not introduced to clinical setting.
- Studies on neoantigens in hematologic malignancies are limited and identifying neoantigens based on genomic data in this field is still unmet need, especially in Korea.
- The purpose of this study is to perform targeted gene sequencing in samples obtained from patients with hematologic malignancies, to screen neoantigens which can be therapeutic target of immune cell therapy among numerous mutation profiles. To facilitate the future study on developing neoantigen-targeted cell therapy, this screening and validation study for neoantigens will be effectively performed for 2 years.
Results
- Thirty-two tumor samples were obtained from patients with hematologic malignancies including acute myeloid leukemia (AML).
- Targeted gene sequencing panel was set up to detect 102 genes associated with AML.
- With the results of targeted gene sequencing and HLA haplotyping, binding affinity was evaluated in silico between each mutation and any HLA molecule ; Frequently observed mutations were CBL, NPM1, NRAS, FLT3, DNMT3A, and IDH1/2, but mutations which are expected high expected binding affinity to HLA were NPM1, DNMT3A, NRAS, and IDH1/2. --> NPM1 mutation is frequent mutation expecially in normal chromosome AML, and is associated with prognosis. I can be considered as adequate cnadidate as a neoantigen.
- ELISPOT was set up to check immunogenicity of candidate neoantigens.
- Mononuclear cell (MNC) from healthy donor showed weak reaction to mutant NPM1 or IDH1/2 peptide. --> It may be due to HLA diversity and lack of donor’s HLA information.
- Plan to validation with the specific patient’s MNC and mutant peptide --> among expected 5 patients, 3 expired during the study period. To obtain more MNCs from patients with specific HLA type (A1101 or A0206) and mutant NPM1 were searched. (Obtaining consent is ongoing.)
- For the analysis of other hematologic disease exept AML, plasmablastic lymphoma (very rare, aggressive lymphoma, currently no published genomic analysis) were selected. --> Seven cases were identified. Plan to perform NGS in Nov 2019.
Expected Contribution
- The results of this sutdy can be used to define potential indication for neoantigen-specific T cell therapy, as a consolidation and salvage treatment, in patients with AML.
- NPM1 and IDH1/2 mutation were focused in this study, however, the results of targeted sequencing and HLA binding prediction would be useful in planning further study on other mutations as neoantigens. (Because NRAS has been reported that NRAS is a targetable neoantigen by T cell therapy in a patient with a metastatic solid tumor, there is a possibility of NRAS-specific immuno/cellular therapy for AML.
- Genomic analysis of PBL, especially HIV-negative and secondary malignancy condition, will add valuable knowledge of this rare and aggressive lymphoma. This would lead comprehensive understanding of PBL and contribute on investigating the effective diagnostic/therapeutic strategy.
