본 연구는 원헬스 연구사업의 일환으로 사람과 밀접하게 생활하는 반려동물, 보호자, 그리고 주변환경(가정환경과 동물병원환경)에서 그들이 보유한 세균들의 항생제 내성 분포율과 전달 기전을 파악하기 위함이다. 본 연구는 3개의 세부팀으로 연구를 추진하였다. 제1세부는 건강하거나 질병이 있는 반려동물의 샘플링과 세균분리 및 내성기전; 2세부는 그람음성균 내성 및 사람과 반려동물 간 내성균 전파 기전; 3세부는 그람양성균 내성 및 사람과 반려동물 간 내성균 전달 기전에 대한 연구를 각각 수행하였다. 각 세부의 주요 결과는 다음과 같다. 제1세부: 질병이 있는 반려동물에서 대장균과 폐렴막대균의 광범위 세팔로스포린 내성율이 30% 이상이었고 대부분 다약제내성균이었다. 또한 지금까지 반려동물에서 보고되지 않았던 카바페넴 내성(NDM-5) 대장균 ST410 클론들이 수도권 동물병원에서 산발적으로 출현하였다. 제2세부: 건강한 반려동물에서 광범위 세팔로스포린 내성 대장균들은 CMY-2와 CTX-M-55 타입의 내성형이 빈번하였고 반면 건강한 사람분리주에서는 CTX-M-14와 CTX-M-15가 우세하였다. 또한, CMY-2 대장균 ST405 클론들은 반려동물, 보호자, 사람환자들의 장내에서 분포하였고, 반려동물에서 흔하였다. 제3세부: 3종의 메치실린 내성 포도알균 중에 MRSA ST72-SCCmec IV 클론들이 지역사회에서 흔하게 분포하였고, 사람뿐만 아니라 반려동물의 비강에도 집락화 되어 있었다. 그 외에도 MRSE나 MRSP 등도 드물게 사람-반려동물 간에 전달되었다.
본 연구를 통해, 집과 같은 제한된 공간에서 사람과 반려동물에서 존재하는 세균 또는 내성균들은 서로 간에 전달된다는 것을 확인하였다.

The purpose of this study is to investigate the distribution of antibiotic resistance and the mechanism of transmission of bacteria in pets, guardians, and the surrounding environment (household environment and veterinary hospital environment) living closely with humans as part of the One Health research project. The mechanism of transmission of methicillin-resistant staphylococci and extended-spectrum cephalosporin- resistant enterobacteriaceae between pet-owner investigated through various molecular genetic data. This study consisted of three subteams. Subteam 1 includes sampling (pet, owner, and surrounding environment), bacterial isolation and resistance mechanisms for healthy or diseased pets; Subteam II studied Gram-negative bacterial resistance and the mechanism of delivery of resistant bacteria between humans and pets; Subteam III also studied the Gram-positive bacterial resistance and the mechanism of transfer of resistant bacteria between human and companion animals. The main results of each team are as follows. Subteam Ⅰ: E. coli and K. pneumoniae showed extended-spectrum cephalosporin (ESC) resistance of more than 30% in diseased companion animals. Most of them were multi-drug resistant bacteria. In addition, carbapenem-resistant (NDM-5) E. coli ST410 clones, which have not been reported in domestic pets, have appeared sporadically in metropolitan veterinary hospitals. Subteam Ⅱ: The ESC-resistant E. coli from healthy pets was frequently resistant to CMY-2 and CTX-M-55 resistance types, while CTX-M-14 and CTX-M-15 predominated in healthy human isolates. In addition, CMY-2 E. coli ST405 clones were distributed in the gut of pets, guardians and human patients, and were common in pets.
Subteam III: MRSA ST72-SCCmec IV clones among the three methicillin-resistant staphylococci, were common in the community and colonized not only in humans but also in the nasal cavity of companion animals. In addition, some MRSE and MRSP clones were rarely transmitted between humans and pets. In this study, it was demonstrated that medically important antimicrobial resistant- or susceptible bacteria which are present in humans and companion animals in a limited space like household are transferred to each other.
