본 연구에서는 영양강화식품의 관리기반을 마련하기 위해 결핍 및 과잉 우려가 있는 비타민, 무기질 등 미량 영양성분 DB 구축 및 노출평가를 하였다. 또한, 건강 위해가능 영양성분의 저감화 정책에 대한 기초자료를 제공하기 위해 당, 나트륨 등 섭취량 심층 분석을 진행하였다.
이러한 결과를 바탕으로 영양성분 위해평가 플랫폼에 영양강화 시뮬레이션 모델을 적용하여 위해평가 시스템의 기능을 개선하고자 하였다.

미량 영양성분 노출평가는 과잉 섭취시 위험도를 고려해 지용성 비타민 1종(비타민 A), 수용성 비타민 1종(비타민 C), 무기질 2종(칼슘, 철)을 선정하여 평가하였다. 영양성분 database(DB)의 구축 방법에 따른 섭취량 차이를 파악하기 위해 국민건강영양조사(이하 국건영) 음식 데이터에 NLS(National laboratory system) 영양소 함량 데이터를 연결해 실측치 기반의 뉴_국건영 DB를 구축하였다. 국건영 DB로 평가 시, 상기의 네 가지 미량 영양성분의 평균 섭취량 모두 권장섭취량을 충족하지 못하는 것으로 나타났다. 뉴_국건영 DB로 노출량을 평가한 결과를 기존 국건영 DB와 비교 시, 칼슘 섭취량은 20% 더 높고, 비타민 C 섭취량은 43% 더 낮게 평가되었다. 품목제조신고서, 수입식품신고서를 이용하여 국민건강영양조사 식이 보충제 데이터를 최신화하여 뉴_건기 DB를 마련하였다. 뉴_건기 DB를 통해 건강기능식품 노출량을 분석한 결과 최근 3년간 건강기능식품 섭취자 비율은 증가하였고, 건강기능식품 또는 식이보충제를 섭취하는 집단일수록 상한섭취량 이상 섭취하는 사람의 비율이 높았다.
건강 위해가능 영양성분 섭취량은 2018년 국민건강영양조사와 기 구축된 총 당류 DB와 트랜스지방 DB를 연계하여 분석하였다. 우리 국민의 당류 섭취량은 2016년 대비 20% 감소하였으나[73.6 g/day(`16) → 58.9 g/day(`18)], 가공식품을 통한 당류 에너지 섭취비율 10% 초과 섭취자의 비율이 3-5세, 12-18세에서 높았고(44.9%, 44.7%), 가공식품으로부터의 당류 섭취량이 청소년층(12-18세)에서 가장 높았다(56.1 g/day). 가공식품 중 음료류를 통한 당류 섭취 비율이 6세 이상의 연령층에서 모두 높았으며, 음료류 중 6-49세는 탄산음료, 50-65세 이상은 커피로부터 당류 섭취량이 높은 것으로 나타났다. 당류 섭취량 95분위 이상에 해당하는 극단섭취자의 경우 당류를 통한 에너지 섭취비율이 평균 24.9%였으며, 가공식품 중 음료류를 통한 당류 섭취량이 가장 높았다(23.8 g/day). 가구형태별로는 1인 가구와 다인가구의 당류 섭취량이 유사하였으나, 1인가구의 가공식품 비중이 58%로 다인가구(51%)에 비하여 높은 비율을 나타냈다. 1인 가구의 구성특성을 보면 노인층(65세 이상)이 33%로 높은 비율을 차지하였고, 커피가 당류 섭취의 주된 급원으로 분석되었다. 청소년의 수면시간과 당류 섭취량과의 경향성을 확인해 본 결과, 남자 청소년에서 수면시간이 짧을수록 당류 섭취량이 증가하는 경향을 보였다.
우리 국민의 나트륨 섭취량은 감소 추세이나[3,669 mg/day(`16) → 3,274 mg/day(`18)] 나트륨 2,000 mg/day 초과섭취자의 비율이 30-49세에서 높았고(80.8%), 30-49세의 경우 음식업소로부터의 나트륨 섭취량이 가장 높게 나타났다(1,519.8 mg/day). 매식여부별 나트륨 섭취는 가정식(41.8%), 음식업소(34.1%), 단체급식(9.4%) 순으로 나타났고, 음식업소의 기여율은 2016년 대비 2018년 16.8% 감소했으며, 음식업소 음식 중 장아찌, 절임류에서 크게 감소하였다(102 mg/day → 41 mg/day, 59.8% 감소). 나트륨 섭취량 95분위 이상에 해당하는 극단섭취자의 경우, 하루 평균 나트륨 섭취량은 6,886.8 mg/day로 평균(3274.1 mg/day)의 2배, 권고량의 3배를 초과하는 것으로 나타났으며, 가공식품군 중 조미식품이 나트륨 섭취에 주로 기여하였다. 가구형태별로 분석한 결과, 1인 가구(3,286.0 mg/day)에서 다인가구(3,272.9mg/day)에 비해 나트륨 섭취량이 높게 나타났다. 연령별로는 30-49세의 1인가구에서 나트륨 섭취량이 가장 높았고(3,890.6 mg/day), 장류에 속하는 급원식품이 26.8%를 차지했다.
영양표시를 이용하는 대상자(3,256.5 mg/day)는 이용하지 않는 대상자(3,402.3 mg/day)보다 상대적으로 나트륨을 적게 섭취하였는데, 특히 30-49세에서 12.8% 정도 섭취량이 낮은 수준을 보였다.
우리 국민의 1일 평균 트랜스지방 섭취량은 0.45g/day으로 총 에너지의 0.1% 수준으로 WHO 권고기준(전체 에너지 섭취량의 1% 미만) 대비 양호한 수준이었다.
미량영양성분 및 건강위해가능 영양성분 분석의 효율성 제고를 위해 영양성분 위해평가 플랫폼에 최신 DB를 탑재하고, 식품에 미량영양성분 첨가시 영양성분 노출량 및 기준 미만/초과자 예측 프로그램을 구축하고, 노출평가를 위한 영양성분의 종류를 확대(트랜스지방,미량영양성분 4종 추가)하여 위해평가 기능을 대폭 개선하였다.
아울러, 빅데이터에 기반한 미량 영양성분 노출평가 및 영양강화 시뮬레이션 시스템을 지속적으로 구현하기 위해 인사혁신처 주관 공공데이터 뉴딜사업의 청년인턴(5명)을 활용하여 국내에 품목제조보고 및 수입신고된 약 7만 여건의 건강기능식품에 표시된 영양성분 함량 등을 정리하여 구축하였다. 이러한 결과를 바탕으로, 당류, 나트륨 등의 건강위해가능 영양성분의 저감화 정책 수립, 식품 등의 기능성 표시제 도입에 따른 미량영양성분 노출량 평가, 일반식품의 비타민, 무기질 등 첨가에 따른 영양강화기준 마련 등 정책 추진을 지원하고 국민들의 건강한 식생활 실천을 위한 교육·홍보 자료 등으로 적극 활용할 수 있도록 노력할 계획이다.

In order to provide the basis of national food fortification framework, this study established a micronutrient database and performed exposure assessment for vitamins and minerals of concern for excess and deficiency. In order to provide the basic data on potentially hazardous nutrients reduction policies, this study performed an in-depth analysis of sugar and sodium. Based on the results, the Nutritional Fortification Simulation were applied to the Nutrient Risk Assessment Platform to improve the system.
For the exposure assessment, this study selected vitamin A, vitamin C, calcium, and iron based on the risk of excessive intake. To examine the potential bias associated with the DB, this study constructed NEW-food DB by linking the food data from Korea National Health and Nutrition Survey (KNHANES) and actual measurement-based NLS food data. The average intake of all four micronutrients did not meet the recommended intake when evaluated with the KNHANES DB. When micronutrient intakes are analyzed by NEW_food DB, calcium intake was 20% higher while vitamin C intake was 43% lower than those of the KNHANES DB. Dietary supplement data from KNHANES was also updated based on the item manufacturing reports and import declarations (NEW_HFF DB). Over the past three years, the proportion of health functional food consumers has increased, and the people who consume health functional food or dietary supplements tended to intake micronutrients more than upper intake levels.
This study analyzed intake of the potentially hazardous nutrition using the 2018 KNHANES DB in conjunction with the previously established total sugar DB and trans fat DB. The sugar intake in Koreans decreased by 20% compared to the 2016 [73.6g/day(`16) → 58.9 g/day(`18)]. But, the proportion of people who consumed more than 10% of total energy from sugars in processed foods was high in 3-5 years old and 12-18 years old(44.9%, 44.7%), and the intake of sugar from processed foods was highest among the youth (12-18 years old, 56.1 g/day). Among processed foods, sugar intake from beverages was high among all age groups of 6 years old and older, while it was found that 6-49 years have high intake of sugars from carbonated beverages and 50-65 years have high intake of sugar from coffee. In the case of those consumed above the 95th percentile sugar intake, the energy intake from sugar was 24.9% on average and sugar intake from beverages was highest(23.8 g/day). In form of household, the sugar intake of single-person households and multi-person households was similar, but the proportion of processed foods intake in single-person households was 58%, which was higher than that of multi-family households (51%) and coffee was the main source of sugar intake. As result of association between sleep duration and sugar intake, male adolescents tend to increase sugar intake as sleep duration is shorter.
The sodium intake in Koreans was on decline[3,669 mg/day(`16) → 3,274 mg/day(`18)] , but the proportion of koreans who consume more than 2,000 mg/day of sodium was high at 30-49 years old(80.8%), and those who aged 30-49 had the highest sodium intake from the restaurant(1,519.8 mg/day). The sodium intake by meal types was turned out to be in the order of home meals(41.8%),restaurants(34.1%), and cafeteria-served meals(9.4%), while the contribution of restaurants decreased by 16.8% in 2018 compared to 2016, and the consumption of pickles in restaurants highly decreased(102 mg/day → 41 mg/day, 59.8% decreased).
Average intake of people who consumed above the 95th percentile sodium intake was 6,886.8 mg/day, more than twice the average sodium intake of total population and three times the recommended amount. Among processed foods, condiment foods were main contributors to sodium intake.
According to the analysis by the type of household, single-person households(3,286.0mg/day) consumed more sodium than multi-family households(3,272.9 mg/day). By age, single-person households aged 30-49 had the highest sodium intake (3,890.6mg/day) and main source of sodium intake of single-person households was the Korean traditional paste products, accounting for 26.8%. Those who used nutrition labels consumed(3,256.5 mg/day) relatively less sodium than those who did not(3,402.3 mg/day), especially those aged 30-49, which was 12.8 percent lower. The average daily intake of trans fats was 0.45 g/day, which is about 0.1% of total energy, conforming to the WHO recommendation (less than 1% of total energy intake).
In order to enhance the efficiency of analysis of micronutrients and potentially hazardous nutrients, the latest DB was installed in the nutrition risk assessment platform. The risk assessment function was greatly improved by establishing the prediction program for the nutrition exposure amount and the proportion of substandard/excessor depending on the food fortification, and by expanding the types of nutrients for exposure assessment (trans fat and four types of micronutrients are added).
In addition, the NEW_HFF DB was continuously established to implement a system for the micronutrients exposure assessment based on big data and the simulation system for nutrition fortification. The DB was established with the micronutrient contents information about 70,000 health functional foods products reported item manufacturing reports and import declarations, and using interns(5 persons) in Public Data New Deal Project administrated by Ministry of Personnel Management. Based on these results, we will support the implementation of policies and make efforts to actively utilize them as educational and promotional materials for the practice of healthy diet. The supports are as follows; establishing policies to reduce potentially hazardous nutrients such as sugar and sodium; evaluating for the amount of exposure caused by the introduction of functional labeling systems such as food; and providing standards for nutrition fortification according to the addition of vitamins and minerals in general foods.
