본 연구는 우리 국민의 건강 위해가능 영양성분 저감 정책의 과학적 근거 마련을 위해 총 당류 및 트랜스지방 DB를 최신화하여 당류, 나트륨 및 트랜스지방 섭취량을 심층 분석하고, 당류 및 나트륨에 대해서는 섭취량 심층 분석이 가능하도록 시스템을 구축하였다. 기 구축된 총 당류 DB와 트랜스지방 DB는 국내·외 식품성분DB를 조사·비교분석하여 데이터를 검증하였고, 가공식품의 DB는 영양표시 정보를 수집하여 구축하였다. 질병관리본부와 조화 추진을 위해 최신화된 총 당류 DB를 수정·보완하고 일원화하였다. 2017년 국민건강영양조사 원시
본 연구는 우리 국민의 건강 위해가능 영양성분 저감 정책의 과학적 근거 마련을 위해 총 당류 및 트랜스지방 DB를 최신화하여 당류, 나트륨 및 트랜스지방 섭취량을 심층 분석하고, 당류 및 나트륨에 대해서는 섭취량 심층 분석이 가능하도록 시스템을 구축하였다. 기 구축된 총 당류 DB와 트랜스지방 DB는 국내·외 식품성분DB를 조사·비교분석하여 데이터를 검증하였고, 가공식품의 DB는 영양표시 정보를 수집하여 구축하였다. 질병관리본부와 조화 추진을 위해 최신화된 총 당류 DB를 수정·보완하고 일원화하였다. 2017년 국민건강영양조사 원시자료와 총 당류 및 트랜스지방 DB를 연계하여 심층 분석한 결과, 우리 국민의 총 당류 섭취량은 점차 감소하는 추세[76.9 g/day(`15) → 64.7 g/day(`17)]이며, 연령별 중 19-29세의 당류 섭취량이 가장 크게 감소하였다. 우유를 제외한 가공식품으로부터의 당류 섭취량도 감소하였으나[47.1 g/day(`15) → 39.6 g/day(`17)], 전체 당류 섭취 중 가공식품이 차지하는 비율은 최근 3년간 약 61%로 당류 섭취에 가공식품이 크게 기여하는 것으로 나타났다. 특히 가공식품 중 음료류(12.5g/day, 31.6%), 과자·빵류 및 떡류(5.9g/day, 14.9%), 당류(5.3g/day, 13.4%) 등이 당류 섭취 주요 식품으로 나타났다. 연령별로는 1-2세는 발효유류, 3-5세는 과일·채소음료, 6세-49세는 탄산음료류, 50-64세는 커피류, 65세 이상은 벌꿀류 등의 설탕류가 당류 섭취 주요 식품으로 나타나 연령별로 차이를 보였다. 우리 국민의 나트륨 섭취량 역시 점차 감소하는 추세[3,890 mg/day(`15) → 3,477 mg/day(`17)]로 WHO 섭취 권고량(2,000 mg/day) 이상 섭취자 비율이 감소하였으나[76.6%(`15) → 73.1%(`17)], 여전히 높은 비율을 보였다. 특히, 30-49세의 나트륨 섭취량이 가장 높았으며, 다른 연령대와는 다르게 2,000 mg/day 이상 섭취자 비율의 변화가 거의 없었다. 전체 나트륨 섭취의 90% 이상을 조미식품, 장류, 절임류 또는 조림류 등과 같은 가공식품으로부터 섭취하는 것으로 나타났다. 가정식 및 음식업소 음식으로부터의 나트륨 섭취량이 각각 1,413 mg/day, 1,312 mg/day로, 남자는 음식업소 음식으로부터 여자는 가정식 음식으로의 나트륨 섭취량이 높은 것으로 나타났다. 가정식 중에서는 김치류, 음식업소 음식 중에서는 면 및 만두류로부터의 나트륨 섭취량이 가장 높았다. 우리 국민의 1일 평균 트랜스지방 섭취량은 0.46 g/day로 나타났으며, 19-29세의 섭취량이 가장 높았다(0.67 g/day). 가공식품으로부터의 트랜스지방 섭취량은 0.23g/day로 나타났으며, 12-29세의 섭취량이 가장 높았다. 트랜스지방 에너지섭취비율은 0.1% 수준으로,권고기준(전체에너지 섭취량의 1% 미만) 대비 양호한 것으로 나타났다. 한국인유전체역학조사사업 도시기반 코호트 자료의 참여대상자를 질환별로 나누어 당류 및 나트륨 섭취량을 분석하였다. 당뇨병 환자의 당류 섭취량은 비질환자에 비해 다소 낮았으나, 고혈압 환자와 비질환자의 나트륨 섭취량은 차이가 없었다. 고혈압 환자의 나트륨 섭취를 줄이기 위한 교육 필요성이 있을 것으로 보인다. 당류 및 나트륨 섭취량 분석 시 소요시간 및 인력을 줄이기 위하여, 영양위해평가 플랫폼에 심층분석 시스템을 탑재하였다.
국민건강영양조사 최신 원시자료를 반영하여 계층별, 섭취종류별, 식품군별 및 음식군별 당류 및 나트륨 섭취량을 보다 쉽고 편리하게 파악할 수 있게 하였다. 본 연구는 건강 위해가능 영양성분인 당류 및 나트륨의 섭취량을 심층적으로 분석함으로써 저감화 정책의 과학적 근거를 마련하였으며, 영양위해평가 플랫폼의 심층분석 시스템을 개발하여 향후 정책 활용을 위한 기초자료를 생산할 수 있는 도구를 마련하였다.

In this study, intake of sugar, sodium, and trans fat was analyzed in depth by updating database of total sugar and trans fat, and in-depth analysis system was developed for sugar and sodium intake of Korean. Previous total sugar DB and trans fat DB were reviewed in comparison with other domestic and foreign DB, and nutrition labelling information of processed food was collected to update DB. Total sugar DB was modified and unified in cooperation with Korea Centers of Disease Control & Prevention(KCDC). As a result of in-depth analysis using Korea National Health and Nutrition Examination Survey(KNHANES) 2017 and the total sugar DB, daily totalsugar intake of Korean had decreased[76.9 g/day(`15) → 64.7 g/day(`17)], and young adults aged 19-29 showed thelargest decrease. Sugar intake from processed food except white milk had also decreased [47.1 g/day(`15) → 39.6 g/day(`17)], although the proportion of sugar intake from processed food among daily total sugar intake was maintained as about 61% during recent three years. Among the processed food, beverages(12.5 g/day, 31.6%), Confectionaries, breads or rice cakes (5.9 g/day, 14.9%), and saccharides (5.3 g/day 13.4%) mainly contributed to sugar intake. The main contributing food groups were different by age groups: fermented milks for the age of 1-2, fruit/vegatable beverages for the age of 3-5, carbonated beverages for the age of 6-49, coffee for the age of 50-64, and sugars as saccharides for the age over 65. Sodium intake of Korean had decreased[3,890 mg/day(`15)→ 3,477 mg/day(`17)]. Although the proportion of Korean over WHO recommendation(2,000 mg/day) had also decreased[76.6%(`15) → 73.1%(`17)], it was still high. Adults aged 30-49 showed the highest sodium intake, andtheir proportion over WHO recommendation was maintained, unlike other age groups. Processed foods such as seasoning food, soy sauce and pastes, and pickled foods or boiled foods accounted for over 90% of daily sodium intake. Sodium intake from homemade meals and restaurant food were 1,413 mg/day, and 1,312 mg/day, respectively. Men showed higher sodium intake from restaurant food, while women showed higher sodium intake from homemade meals. Regarding dish groups, kimchi for homemade meals, and noodles and dumplings forrestaurant food had the highest sodium intake for each meal type. Daily trans fat intake of Korean was 0.46g/day, and young adults aged 19-29 showed the highest trans fat intake. Trans fat intake from processed food of Korean was 0.23 g/day, and the age of 12-29 showed the highest trans fat intake from processed food. Trans fat intake of Korean was 0.1% of their total energy intake, which was under the recommendation of WHO(under 1%E). This study also analyzed sugar and sodium intake of patients with diabetes or hypertension using data from Korean Genome and Epidemiology Study(KoGES) - health examinee study. Sugar intake of diabetic patients was higher than that of non-diabetic subjects, while sodium intake of hypertensive patients and non-hypertensive subjects had no significant difference. Nutrition education to reduce sodium intake especially for patient with hypertension is necessary. To save the time and personnel for the analysis of sugar and sodium intake, in-depth analysis system was established on the Nutri-Risk Platform. Sugar and sodium intake according to classes, intake types, food groups, and dish groups can be easily and conveniently obtained with the latest data set of KNHANES. This study provided scientific evidences for sugar and sodium reduction policies by analyzing intake of potentially hazardous nutrients in depth. By developing in-depth analysis system on the Nutri-Risk Platform, this study also provide a tool which can produce baseline data for application to future policy.
