연구의 목적 및 내용
◆ 목표: 동종 조혈모세포이식을 받은 급성 골수성백혈병 환자에서 NGS 분석법을 미세잔류백혈병 측정을 위해 최적화하여 재발예측 유효성을 검증하고 재발 고위험군에 대한 치료옵션으로 맞춤면역치료의 근거를 탐색
□ 1세부 연구: 차세대 염기서열 분석법의 미세잔류백혈병 측정 유효성 검증 및 재발 고위험군 맞춤면역 치료 근거 발굴
○ 발굴 및 검증을 위한 독립된 전향적 코호트의 치료시기별(진단, 항암화학요법 후, 이식전, 이식 후 3개월, 6개월, 12개월, 24개월, 36개월) 골수 시료 수집 및 임상
연구의 목적 및 내용
◆ 목표: 동종 조혈모세포이식을 받은 급성 골수성백혈병 환자에서 NGS 분석법을 미세잔류백혈병 측정을 위해 최적화하여 재발예측 유효성을 검증하고 재발 고위험군에 대한 치료옵션으로 맞춤면역치료의 근거를 탐색
□ 1세부 연구: 차세대 염기서열 분석법의 미세잔류백혈병 측정 유효성 검증 및 재발 고위험군 맞춤면역 치료 근거 발굴
○ 발굴 및 검증을 위한 독립된 전향적 코호트의 치료시기별(진단, 항암화학요법 후, 이식전, 이식 후 3개월, 6개월, 12개월, 24개월, 36개월) 골수 시료 수집 및 임상정보 구축: 2013년 이후 현재까지 독립된 2개의 코호트를 이미 구축하여 양질의 시료 및 임상정보를 확보 가능
○ 최적화된 NGS를 통한 미세잔류백혈병의 이식 재발 예측 유효성 검증: 돌연변이 유전체 종류, 환자특성, 이식의 종류 및 방법, 백혈병 subtype 별 차이 분석 및 기존의 검사법(형태학적 검사, 유전자별 중합 효소연쇄반응 분석법)과 비교
○ 재발 고위험군에 대한 맞춤형 면역세포치료 및 면역항암제치료 근거 확립을 이한 이식 후 면역재구성 및 면역관문 수용체/리간드 분석
① 이식 후 면역세포들(αβ T cells, γδ T cells, regulatory T cells, B cells, NK cells 등)의 회복 정도를 유세포분석법을 이용 시기별로 평가
② 면역세포들에서 면역관문 수용체 및 리간드 분석(PD-1, PD-L1/L2, CTLA4, CD80, TIM3, Gal-9, 등)
□ 2세부 연구: 미세잔류백혈병 진단을 위한 차세대 염기서열 분석법 최적화 및 추적 알고리즘 개발
