□ 연구개요
전 세계적으로 당뇨환자가 증가하고 있으며 당뇨병성 족부궤양 (Diabetic foot ulcer, DFU)은 당뇨의 심각한 합병증 중의 하나로 환자의 삶의 질을 저하시키고 큰 경제적 부담을 주게 된다. DFU의 치료에 다양한 치료방법들이 행해지고 있으나 아직 DFU는 치유하기 힘든 합병증으로 효과적인 치료방법이 필요한 상태이다.

□ 연구개요
전 세계적으로 당뇨환자가 증가하고 있으며 당뇨병성 족부궤양 (Diabetic foot ulcer, DFU)은 당뇨의 심각한 합병증 중의 하나로 환자의 삶의 질을 저하시키고 큰 경제적 부담을 주게 된다. DFU의 치료에 다양한 치료방법들이 행해지고 있으나 아직 DFU는 치유하기 힘든 합병증으로 효과적인 치료방법이 필요한 상태이다.
이에 본 연구에서는 DFU의 치료 시 시행되는 지방유래 줄기세포(Adipose-derived stem cells, ADSCs)의 치료에서 저출력 레이져 치료(Low-level laser therapy, LLLT)를 병행하였을 때 상처치유를 증진 시킬 수 있는지에 대해 당뇨 동물 모델을 이용하여 알아보자 하였다.
□ 연구 목표대비 연구결과
Streptozocin을 이용하여 Rat에게 당뇨를 유발하였다. Rat의 발등에 상처를 유발하고 관리하는 과정은 순조롭게 이루어졌다. 그러나 Rat의 inguinal fat tissue를 이용한 ADSCs의 추출과 배양을 하는 과정에서 배양이 잘 이루어지지 않아 실험에 필요한 ADSCs를 확보하지 못하였다. 이후 기존에 제품으로 나와 있는 Human-ADSCs Exosome을 줄기세포 대신 사용하여 사전실험을 시행하였다. 사전실험 결과 그룹 간의 창상치유의 차이를 볼 수 없었다. 논의 결과 치료에 대한 변수가 많은 경우 치료방법의 효과를 정확히 증명할 수 없다고 판단하였다. LLLT가 DFU의 치료에 효과가 있는지 실험동물의 발을 이용한 실험은 아직 없는 상태로 본 실험에서는 우선 LLLT의 효과를 알아보고자 하였으며 LLLT의 레이져 조사방법을 다르게 시행하여 효과적인 LLLT의 방법을 알아보고자 하였다. 따라서 사전실험으로 당뇨를 유발한 Rat의 양측 뒷발에 창상을 유발한 후 같은 에너지의 레이져를 조사하되 한 군은 하루에 single dose로 6 J/cm2의 레이져를 조사하고, 한 군은 2 J/cm2씩 창상 1, 3, 5일째 나누어 레이져를 조사하였다. 사전실험 결과 대조군에 비하여 레이져를 조사한 군에서 창상치유 효과가 빠르게 나타남을 확인할 수 있었다. 이에 본 실험에서는 사전실험의 레이져 세팅에서 레이져 출력의 세기를 달리하는 조사방법을 추가하여 진행하였다. 총 50마리의 Rat을 대조군을 포함한 4군으로 나누어 실험을 진행하고 있다. 창상 후 2주간 창상의 크기를 측정하고 창상 6, 10, 14일에 창상 조직을 채취하여 조직학적 분석을 통하여 창상치유의 정도를 비교 분석할 예정이다.
□ 연구개발성과의 활용 계획 및 기대효과(연구개발결과의 중요성)
LLLT를 이용한 DFU의 치료는 이미 임상적으로 시행되고 있다. 그러나 LLLT의 치료효과를 증명하는 것은 쉽지 않으며 기존의 동물실험들에서는 동물의 발에서 치료효과를 확인한 실험은 없었다. 따라서 본 연구를 통해 동물의 발에서 LLLT의 치료효과를 증명할 수 있다면 LLLT의 DFU에 대한 치료효과를 더욱 명확히 증명할 수 있으며, 다양한 레이져 조사 방법을 서로 비교함으로써 임상적으로 LLLT를 적용할 때 가장 효과적인 레이져 조사 방법을 제시할 수 있을 것이다. 본 실험의 결과와 경험을 바탕으로 동물의 발을 이용하여 DFU에 대한 ADSCs의 치료효과 및 LLLT와 ADSCs의 병행치료 시 효과를 명확하게 증명할 수 있을 것으로 기대한다.

Although various treatments are being used for DFU, DFU is still a difficult complication to cure, and an effective treatment method is required. We investigated whether low-level laser therapy (LLLT) can enhance wound healing in the treatment of adipose-derived stem cells (ADSCs), which is performed during the treatment of DFU. We tried to find out about it using a diabetic animal model.

Diabetes was induced in rats using Streptozocin. The process of inducing and making wounds on rat's feet went smoothly. However, in the process of extracting and culturing ADSCs using rat inguinal fat tissue, culturing was not performed well, so ADSCs necessary for the experiment could not be obtained. Afterwards, a pilot study was conducted using Human-ADSCs Exosome, which was previously released as a product, instead of stem cells. As a result, there was no difference in wound healing between the groups. We judged that the effectiveness of the treatment method could not be accurately demonstrated if there were many variables for treatment. There have been no experiments using the feet of experimental animals to determine whether LLLT is effective in the treatment of DFU. First, we tried to investigate the effect of LLLT and find an effective method of LLLT using various laser parameters. In the pilot study, after inducing wounds on feet of the diabetes-inducing rat, the same energy laser was irradiated, but one group was irradiated with a laser of 6 J/cm2 on days 1, and the other group was irradiated with a laser of 2 J/cm2 on days 1, 3, and 5 seperately. As a result, it was confirmed that the wound healing effect appeared faster in the laser-irradiated group than in the control group. Therefore, in this experiment, an irradiation method of varying the intensity of laser output in the laser setting of the pilot study was added. A total of 50 rats were divided into 4 groups including the control group to conduct the experiment. The wound size will be measured for 2 weeks after the wound, and the tissue will be harvested on the 6th, 10th, and 14th days of the wound to compare and analyze the degree of wound healing through histological analysis.

Treatment of DFU using LLLT is already being performed clinically. However, it is difficult to prove the therapeutic effect of LLLT, and there has been no experiment to confirm the therapeutic effect using the feet of experimental animals. Therefore, if we can prove the therapeutic effect of LLLT in the feet of experimental animals through this study, the therapeutic effect of LLLT on DFU can be more clearly demonstrated. Based on the results and experiences of this experiment, it is expected that the therapeutic effect of LLLT and ADSCs on DFU can be clearly demonstrated using experimental animal feet.
