나트륨 섭취량을 줄이기 위해서는 임상현장에서 나트륨 섭취를 감소시키기 위한 여러 방법들의 이행을 저해하는 요인을 우선적으로 파악하고 이들 저해 요인을 제거, 극복, 해결할 수 있는 이행 실천 전략 마련이 필요하다. 동시에 적극적인 저나트륨 건강증진 식단 개발, 홍보 및 교육 강화, 이행 실천 제고를 위한 중재 전략이 우선적으로 마련되어야 한다.
본 연구팀은 IoT 기반 기술(저염식이 APP)을 기반으로 고혈압 환자에서 나트륨 감소 및 저염식이 실천을 위한 이행연구를 시행하였다. 본 조사연구는 1) 저염식단 보급을 위하여

본 연구팀은 IoT 기반 기술(저염식이 APP)을 기반으로 고혈압 환자에서 나트륨 감소 및 저염식이 실천을 위한 이행연구를 시행하였다. 본 조사연구는 1) 저염식단 보급을 위하여 개발된 앱을 사용하여 임상 현장에서 고혈압 환자들을 대상으로 16주간의 중재를 시행하였다 2) 중재 전후 환자들의 나트륨 섭취량(일회뇨에서 가와사키법으로 추정)과 혈압의 변화를 살펴보았으며 3) 저염식이와 고혈압에 대한 행동, 이해, 지식을 평가하였으며 4) 중재기간 동안 앱을 통한 저염식단의 제공, 문자 메시지를 통한 식단 교정 및 카드 뉴스 등의 고혈압 및 저염식 관련 정보를 제공하여 나트륨 저감의 가능성과 촉진 저해 요인을 살펴보았다. 2개 권역의 4개 대학병원에서 220명의 환자를 등록하였으며 최종적으로 180명의 자료를 분석할 수 있었다. 16주간의 중재를 통하여 나트륨 섭취량의 감소와 수축기 및 이완기 혈압의 감소를 확인할 수 있었고, 고혈압과 저염식이에 대한 이해와 행동의 변화를 관찰할 수 있었다. 이행의 촉진 저해 요소를 확인하기 위한 동반 질환의 유무, 성별, 연령대에 대한 하위 분석에서는 유의미한 요인은 확인되지 않았다. 최근 나트륨 섭취 감소가 미미한 가운데 본연구자들은 IoT 기반의 중재의 효과를 확인하였으며, 이를 임상현장에 실천하기 위하여 향후 대조군 연구를 통한 검증이 필요할 것으로 보인다. 앱의 고도화, 저염식이 실천에 구체화된 행동 제시, 저염 식단의 효율적인 확산 및 디지털화된 혈압 측정 자료들과 함께 프로토콜이 구축된다면 향후 국민보건 향상에 필수적인 저염식이 실천이 가능할 것으로 본다.

To reduce sodium intake, it is necessary to first identify factors that inhibit the implementation of various methods to reduce sodium intake in clinical sites and prepare an implementation strategy to eliminate, overcome, and solve these inhibitors. At the same time, intervention strategies should be prepared first to develop an active low-sodium health promotion diet, strengthen public relations and education, and enhance implementation practices.
Based on IoT-based technology (low-salt diet APP), this research team conducted an implementation study for sodium reduction and low-salt diet practice in hypertensive patients. This study conducted 16 weeks of intervention for hypertensive patients at clinical sites using 1) an app developed for the distribution of low-salt diets 2) Changes in sodium intake and blood pressure in patients before and after intervention (estimated by Kawasaki method) and 3) behavior, understanding, and knowledge of low-salt and hypertension were evaluated. 4) High-salt and low-salt related information were provided through apps, text messages, and card news during the intervention. 220 patients were registered at four university hospitals in two regions, and finally 180 data could be analyzed. Through 16-week intervention, a decrease in sodium intake and systolic and diastolic blood pressure could be confirmed, and changes in understanding and behavior of hypertension and low-salt diet could be observed. No significant factors were identified in the sub-analysis of the presence or absence of comorbid diseases, gender, and age group to identify factors that hinder the promotion of implementation. While the recent decrease in sodium intake has been insignificant, we have confirmed the effect of IoT-based intervention, and it is expected that verification through future control studies will be necessary to practice this in the clinical field. If the protocol is established along with the advancement of the app, the presentation of actions embodied in the practice of low-salt diet, the efficient spread of low-salt diet, and digitized blood pressure measurement data, it will be possible to practice low-salt diet, which is essential for improving public health.
