To investigate the risk factors associated with the perforation of appendicitis patients, we reviewed the medical records for 277 patients with postoperative diagnosis of appendicitis at a hospital in Taegu City, from January 1 to December 31, 1989. Of 277 appendectomy patients, sixty nine (24.9 %) were perforated cases. Age-specific incidence rates of perforation were as follows: 31.8 % for 0 - 9 years, 14.6 % for 10 - 19 years, 16.3 % for 20 - 29 years, 20.9 % for 30 - 39 years, 25.0 % for 40 - 49 years, 36.0 % for 50 -59 years, and 52.8 % for 60 years over. The incidence rates of perforation in childhood and elderly (above 60) were higher than those for adults (p <0.05). The perforation rate was significantly increased with the symption duration (the period from symptom onset to the time of surgery); 9.7 %- within 24 hours, 21.2 % for 25 - 48 hours period, 38.5 % for 49 - 72 hours period, and 57.6 % for above 73 hours period (p < 0.05). The perforation rates by season were as follows 15.9% in Summer, 22.2 % in Spring, 23.7 % in Autumn, and 37.7 % in Winter, therefore, the incidence of perforation had increased with the air temperature decreased (p < 0.05). The perforation rate (39.5 %) of patients who had fever (above 37.8 C) was significantly higher than that (13.9%) of patients who were not (below 37.8 C) (p < 0.05). The perforation rate was 21.6 % for those who didn't have complaints of rigidity on the abdomen, 24.7 % for those who had complaints of rigidity on the right lower quadrant area, and 53.8% for those who had complaints of rigidity on the other areas. The rigidity on the abdomen was also significantly associated with the perforation of appendix (p < 0.05). The perforation rates of those whose location of appendix (19.7%, 36.8 %) was free or retrocecal were significantly higher than those (52.9 %, 75.0 %) whose location of appendix were retrocecal or retroperitoneal (p < 0.05). The overall complication rate of 277 patients was 7.6 % and that of wound infection was 5.8 %. The incidence rate of complication in perforated cases (26.1 %) was significantly higher than that (1.4 %) in nonperforated cases (p < 0.05). The mean duration (days) of hospitalization in perforated cases (15.6 days) was significantly longer than that (9.0 days) in nonperforated cases (p < 0.05). We selected some factors such as symptom duration, body temperature, age, rigidity on the abdomen, the location of appendix, and admission season as significant variables by discriminant analysis to predict the perforation status preoperatively for the patients with appendicitis. We can predict the perforation status of appendix correctly with the level of 79.8 % accuracy using discriminant function model including above six variables.
