연구도구는 Davis(1995)가 개발한 CECSS(Consumer Emergency Care Satisfaction Scale)를 간호상황에 알맞게 용어를 수정보완하여 사용하였으며 CECSS의 신뢰도는 Cronbach's α=.84 로 나타났다. ...

In today's highly competitive health care environment, nurse manager's understanding of the concepts of quality assurance will assist the hospital to be marketable. Especially, quality of nursing care in the emergency department is very important for the first image of the hospital and patient satisfaction with care. The purpose of this study is to provide basic datas for the improvement of the quality in the emergency nursing services by investigating the level of patient's satisfaction with nursing care and identifying the factors influencing patient satisfaction with the emergency nursing care. The subjects were 215 patients age of 15 and over, who had visited the emergency department of the hospital with more than 500 beds, locating in Chonju, Chonbuk area. The data was collected from August 25, 1997 to September 24, 1997, using the adjusted tool of the DAVIS CECSS(Consumer Emergency Care Satisfaction Scale, 1995) which consisted 19 items with 5 likert scale. The data of 201 subjects was analyzed by descriptive statistics for frequency, percentage, mean and standard deviation, and by Cronbach's α, factor analysis, t-test, one-way ANOVA, and Scheffe’ test using with SPSS/PC^+ program. The results of the study are summerized as follows : 1. The characteristics of the subjects showed ; female (52.2%), male (47.8%) in gender, and age ranged from 15 to over 60, among these, same 27.3% were from 40 to 59 and over 60 years old in both sex. The majority(68.2%) of the subjects were living in Chonju·Wanju region and, 71.6% were married. The high school graduates(32.8%) showed the highest proportion in education level. 24.4% of the subjects were housewife in occupation, and 44.3% were the presbyterian for their religion. The majority(63%) of the subjects economic status showed middle class level. 2. The pattern of the using emergency department services for the majority of the subjects revealed; 84.6% of the subjects had medical insurance coverage for their medical services. the majority(61.2%) of the subjects were involved with a service of the Internal Medicine, direct visit by the subjects(74.1%), automobile of an individual (55.2%) for the transportation, visiting from 3PM to 10:59PM (38.3%) most, staying up to 6 hours (32.8%), with no history of visiting emergency department before (56.2%), with an acute illness(54.7%), for the short distance R the convenience of the traffic for the location of the emergency department (26.4%), admission for the follow up care (68.2%), with improvement of health condition after emergency care (70.2%), willing to come back for an emergency care in the future and recommend to others (66.7%). 3. Factor analysis of the 19 items on the satisfaction level revealed for 5 factors ; socio-psychological factor, educational factor, technical- professional factor, intra-interpersonal factor, and nursing staffing factor. 4. The mean score for the patient's satisfaction with the overall emergency nursing services was 3.184(±0.58) out of the highest score 5. The highest factor of the patient's satisfaction with the emergency nursing services was technical-professional factor(3.540±0.78), and the lowest factor was educational factor (2.345±1.08). "The nurse was skillful in performing his/her duties" was the highest score item 4.035(±0.84), and "The nurse gave me instructions about caring for myself at home" was the lowest score item 2.189(±1.00) among all the 19 items. 5. The factors showed the significant difference to the overall subjects' satisfaction with the emergency nursing care were ; in economic status(F=5.07, p=0.01), for the improvement of health condition after the emergency care(F=3.32, p=0.04), and the factors showed the significant difference of the subjects' satisfaction level with the emergency nursing care were, technical-professional factor(t=-2.38, p=0.02) in gender, intra-interpersonal factor(F=2.90, p=0.04) in age, intra- interpersonal factor(F=2.76, p=0.04) in religion, socio-psychological factor(F=6.52, p=0.00) and educational factor(F=3.54, p=0.03) in economic status, intra-interpersonal factor(F=2.61, p=0.05) in reason of visiting emergency department, socio-psychological factor(F=3.83, p=0.02) and educational factor(F=4.96, p=0.01) for the improvement of health condition after the emergency care. In conclusion, this study revealed that educational factor and nursing staffing factor were lower level and the multiple factors related to the patient's satisfaction with emergency nursing services. Therefore, nursing management strategics must established for remove patient's dissatisfaction factors and improvement of the emergency nursing care quality through continuous measurement and assessment of the patient's satisfaction with emergency nursing services.
