Anemia is the most commonly occurring nutritional problem throughout the world. The worldwide prevalence of anemia in pregnant women is estimated at 10-13%. In Korean study, the estimated prevalence of anemia during pregnancy is 17-33%, and that of anemia during lactation is 19%. Anemia during pregnancy is associated with increased risks of low birth weight and preform delivery. The purpose of this study were to investigated the nutritional status changes of iron, folate, copper, manganese, and chromium indices during pregnancy and lactation in a cross sectional study and longitudinal study, and to determin the relationship of hematological status between maternal and cord blood, and to evaluate the effect of iron status markers and folate status on pregnancy outcome. In a cross sectional study, Maternal RBC number, Hct, and concentrations of Hb, serum iron, and ferritin were significantly decreased during pregnancy. But, Maternal total iron binding capacity(TIBC) and soluble transferrin receptor(sTfR) were increased during pregnancy. In the serum concentrations of folate and copper were significantly increased during pregnancy, but whole blood folate, erythrocyte folate, serum manganese, and chromium concentrations were not significantly different during pregnancy. In a longitudinal study, Maternal RBC number, Hct, and concentrations of Hb, serum iron, and ferritin were decreased during pregnancy as in a cross sectional study, and increased until 3 month, and decreased over 6 month postpartum. But, Soluble transferrin receptor(sTfR) and the ratio of sTfR : ferritin were significantly increased during pregnancy and decreased over 6 month Postpartum. Maternal serum folate and copper concentrations were significantly increased during pregnancy and decreased over 6 month postpartum. But, whole blood folate, erythrocyte folate, serum manganese, and chromium concentrations were not significantly different during pregnancy and postpartum. Dietary intakes were low compared with Korean current recommendations during pregnancy and postpartum, especially intakes of iron folate were very low. In the third trimester, maternal Hb and ferritin concentrations were significantly associated with iron supplement intakes in a cross sectional study. The levels of serum iron, TS, and ferritin in cord blood were found to be elevated relative to the corresponding maternal blood. There were significant correlations between the maternal Hb level in the third trimester and Hb level in cord blood and infant's birth weight. And, erythrocyte folate level in the cord blood was associated with infant's birth weight and height. In conclusion, Maternal iron nutritional status is depleted during pregnancy and plasma volumn expansion dilutes the hematological variables causing them to reduce in concentration. Intakes of folate supplement during pregnancy ensure maintenance of adequate folate nutrition status. There were no correlation between the levels of iron and folate in maternal and cord blood. These results suggest that there is an active process of transfer for iron from mother to the fetus, and this tranport is independent of fetal erythropoiesis. Maternal iron and folate nutritional status during postpartum were to be elevated relative to them during pregnancy, but is not to be continued.
