Analysis of validity of the links between nursing diagnoses & nursing interventions in the Home Healthcare Classification(HHCC)원문보기

The purpose of this study was to test the validity of links between nursing diagnoses and nursing interventions using the Home Health Care Classification (HHCC).
This study was a descriptive study which analyzed the home health care records of 130 clients who received care in the Home Care Department of Y University Medical Center, Seoul, between September 1998 and October 1999.
The study was done in two steps:
In the first step, the nursing diagnoses, nursing interventions, and nursing activities were classified using the HHCC and the nursing interventions were linked to the nursing diagnoses. In the second step, content validity of the 143 nursing interventions as linked to 17 nursing diagnoses was assessed by a team of experts which consisted of four professors in nursing, one specialist in QI, and from home care departments, an administrator, four head nurses, five charge nurses and 15 staff nurses. To further test validity, 25 home care nurses with more than two years of home health care experience were surveyed. The expert survey was used to establish the content validity of the linked nursing diagnoses and nursing interventions. The experts were asked to indicate on four point scale the validity of the link and to identify any interventions that needed to be deleted or added.
The results of the study are summarized as follows:
1. The results of classifying the nursing diagnoses, nursing interventions and nursing activities using the HHCC showed that for 1,375 home visits, there were 653 nursing diagnoses in 54 classification areas, and 9,941 nursing interventions in 106 classification areas.
2. The 653 nursing diagnoses in order of frequency were: Infection risk (13.17%), Skin integrity impairment (9.65%), Body nutrition deficit (8.73%), Skin integrity impairment risk (7.04%), Airway clearance impairment (5.36%), Colonic constipation (4.29%), Anxiety (4.29%), Acute pain (3.68%), Disabled family coping (3.22%) and Swallowing impairment (2.91%).
3. The 9,941 nursing interventions in order of frequency were: Intake/output(8.49%), Tracheostomy care (6.67%), Gastrostomy/Nasogastric tube insertion (4.06%), Positioning therapy (3.68%) and Inhalation therapy (3.65%). For the nursing activities, the frequencies were Direct care (43.43%), Teaching (32.75%), Assessing (29.24%), and Managing (3.75%).
4. Data were analyzd using the Content Validity Incidence (CVI) where if 80% or more of the expert group agree the item is retained. The CVI for the entire set of interventions was high at 0.97. Only one intervention achieved less than 80%, but 46 interventions were identified by the experts as needing to be deleted or added.
5. The final results for the links between nursing diagnoses and nursing interventions included 122 nursing interventions for 17 nursing diagnoses. The nursing diagnoses according to the number of nursing interventions which ranged from 4 to 12 were: 12 interventions for Body nutrition deficit, 11 each for Infections risk and Skin impairment, 8 for Colonic constipation, and 7 each for Physical mobility impairment, musculoskeletal alteration, Disabled family coping, Hyperthermia, Airway clearance impairment, Breathing pattern impairment, Swallowing impairment, and Anxiety.
The use of standardized classification in the areas of nursing diagnoses and interventions facilitates communication and clinical decision making. A common nursing language will unite nurses and make nursing care in identifiable essence in home care departments. This study also adds to the growing body of work that used nursing interventions to enhance quality of nursing care and build a unique body of knowledge in home health care.
