The purpose of this study was to analyze the elements, which are employed in the process of designing facilities for people with dementia, and then, to compare them with theories concerning environmental design for such patients. Major methods used in this study were: references to relevant literature as well as observations and analysis of actual environmental designs. For effective comparison between the theories and actual cases, an analytical framework was developed by gathering information from relevant texts, and afterwards that framework was utilized in the analysis of actual cases of environmental designs for people with dementia. The subjects of the analysis included eleven actual environmental models that were designed for people with dementia. Those designs had been published in periodicals, from advanced countries, which specialize in facilities for senior citizens. The analytical data used included floor plans and texts, as well as a variety of visual materials including photographs that well demonstrated characteristic elements of such facilities from the viewpoint of environmental design. The framework for analysis includes four categories: two that encompass more abstract, or theoretical aspects: a variety of theories on environmental designs for the elderly or people with dementia, and behavioral concepts which can be witnessed in the environments of people with dementia. There were also two categories, of a more concrete nature, that included broader and more composite environmental elements such as layout, and more specifically, and detailed environmental elements such as walls or doors, Under the framework for analysis, a total of 298 criteria were used to analyze the actual cases of environmental design. The following briefly explain the results of the case analysis.
First, a number of theories concerning environmental designs for the elderly or people with dementia were found to be well reflected in designs of actual facilities. For example, the Continuity Theory by Atchely was mostly reflected in the arrangements and in the size and scale of the spaces. The Environmental Press Theory by Lawton & Nahemow was seen in Design dealing with view, layout, and circulation. The Complementary & Similarity Theory by Carp & Carp echoes the designs of circulation, view and layout, and the Environment-Behavior Theory by Zeisel & etc. the designs of circulation, view and layout. The Self-Presentation Theory by Goffman was seen in the designing of walls and furniture, the Ecological Theory by Altman and Chemers in the designing of circulation, and the Neuro-Science Theory by the Zeisel and Raia in the scale/size of spaces.
In particular, the Environmental Press Theory, the Complementary & Similarity Theory, and the Environment-Behavior Theory were the most frequently reflected in environmental design. People with dementia demonstrate a variety of problems due to their weakening physical and cognitive capabilities as well as weakening ability to adapt to the environment, Thus, facilities were designed in ways to assist the elderly with weakened physical capabilities, to enhance their abilities to adapt to the environment, and to create an environment which can meet their needs and requirements, so that they can lead healthier and more independent lives, as well as enjoy an enhanced quality of life.
Second, it was found out that many of the physical characteristics of the facilities for people with dementia were based on theories about environment. For example, the layout most often reflected the Environment-Behavior Theory, the Continuity Theory, the Complementary & Similarity Theory, and the Environmental Press Theory. In particular, the Complementary & Similarity Theory was the most frequently reflected in the designs. More specifically, spaces were arranged in ways to compensate for the problems that people with dementia face when fulfilling their basic needs such as going to the bathroom, and to provide similarity in environment to meet their higher-level desires such as socializing. The Neuroscience Theory and the Continuity Theory were the major factors in determining the size/scale of the spaces. In other words, size or scale of their surrounding spaces were relatively small, just like their own homes, in order to provide the elderly with a feeling of familiarity with their past experience, to maintain consistency in the elderly's internal and external structures, as well as offer the same types of places as those they remembered. In the case of designing view, the Environment-Behavior Theory, the Environmental Press Theory, and the Complementary & Similarity Theory were reflected. In particular, the Environmental Press Theory and the Complementary & Similarity Theory were the most frequently witnessed. Since people with dementia experience psychological stress when attempting to process information they get from their environment due to their worsening cognitive capability, appropriately designed views can provide proper stimulus and information to them, and can help them better adapt to their environment. For designing circulation, the Ecological Theory, the Environmental Press Theory, the Complementary & Similarity Theory, and the Environment-Behavior Theory were reflected. In particular, the Environment-Behavior Theory was the most frequently applied. Characteristic behaviors of people with dementia include aimless wandering. Therefore, circulation were designed in ways to make such aimless wandering safer and more meaningful behavior.
By making comparisons between theories on environmental design for people with dementia with their applications in actual facilities, this study expects to offer a knowledge base for better environmental design, which takes into consideration the typical problems of people with dementia, and satisfies their needs. Analysis of relevant theories and actual facilities for people with dementia in advanced foreign countries will also help establish guidelines for designing such facilities which can meet the cultural characteristics and traditional conditions of Korea. A theoretical basis of environmental design for facilities for people with dementia will be able to serve as a bridge for further development of theories on broader issues, such as the elderly or environmental design in general.
