This study purports to investigate the degree of cognition, accomplishment and requirement of the clinical nutrition business of dietitians in the hospital and of satisfaction over nutritional consultation. The subjects were 89 dietitians working in hospitals with over 100 beds in Seoul&Kyoung-gi, Taegu&Kyoung-buk, and Taejon& Chung-cheong provinces and 82 patients. This survey was conducted from October 1st to the 30th, 1999. The results may be summarized as follows: on the number of beds per dietitian, 27% answered under 100 beds per dietitian, while 30.8% answered over 200 beds per dietitian. In the nutrition department, 53.9% was under the clinical health care division and 46.1% was under the general administrative division. About 67.4% of dietitians worked for clinical nutrition and food service, 32.6% of them worked only for clinical nutrition. In the case of the menu cycle, the 15 days cycle and 7 days cycle were 36.0% and 35.9%, respectively. Only 33.7% of the hospitals have rooms for nutritional consultation. The case number of nutritional consultation showed that 50.2% was Under 10 cases/week, and the sessions usually took 20-40 minutes for a patient. The diseases, which were frequently requested for nutritional consultation, were diabetic mellitus (29.8%) and renal disease (17.7%). The reason dietitians had low satisfaction over nutritional consultation was the lack of understanding ability on the part of the patients (34.4%). They also recommend to the purchase of some booklets and leaflets as educational materials for nutritional consultation. Most patients who availed of nutritional consultation were DM patients (64.6%), and the primary motivation for nutritional consultation came from doctors recommendations (81.1%). About 91.5% of the patients were satisfied with the nutritional consultation service they received, and 95.0% of them said that the consultation and education were helpful for their health care. The degree scores of cognition, accomplishment, requirement about the clinical nutrition 69.4±16.2, 69.6±16.2, 67.7±15.5, respectively. The scores for cognition, requirement and accomplishment about the nutrition business of dietitians who work only for clinical nutrition were higher than those who work for both the nutrition and food service businesses. The scores for the above items of dietitians who handled less bed were higher than those responsible for more beds.
