We investigated the 2.5 years course of illness and the predictors of social outcome of 56 patients with bipolar disorder who had been hospitalized. As a preliminary step for prospective outcome study, a retrospective assessment was used. At the point of admission and after hospitalization several clinical variables of the patients were investigated from their psychiatric medical records and the current social functions of the patients were assessed through interviews with the patients or their families using GAF scores and major items of PSYCH-UP. Mean score analysis was conducted by the groups of several clinical variables during the period of observation, and a multivariate regression analysis was carried out to identify significant social outcome predictors. We found that the patients with bipolar disorder experienced 3.55 times of episodes on average during the 2.5 years including depressive, hypomanic, manic and mixed episodes. The number of depressive, hypomanic and manic episodes appeared to be similar, but the number of depressive episode were slightly larger. In addition, the numbers of episode recurrence and mean duration of admission was identified as predictors for social functions, but the age of onset, socioeconomic status, premorbid function, duration of illness, psychotic symptoms and compliance appeared not to be predictor of the social outcome. This study has a significant meaning of being the first study in korea that followed up various courses and identified predictors for social outcome of patients with bipolar disorder.
