In particular, LIUS has been shown to enhance chondroycte proliferation during fracture repairs, actually, therapeutic LIUS in the management of osteoarthritis (OA) has been reported in several studies. However, the biochemical and histological effects of LIUS on human OA have never been systematically investigated. Therefore, the objective of this study was to investigate the effects of LIUS on chondrocyte proliferation and matrix synthesis in articular chondrocytes in three different culture conditions, the expalnt, the alginate culture and the monolayer, and determine the most efficient intensity of LIUS as a treatment modality. Materials and Methods: Articular cartilage was harvested from patients who had undergone arthroplasty of OA and rabbit knee. Cartilage explants and isolated chondrocytes from articular cartilage were sonicated in various culture conditions as explant culture, three dimensional culture, monolayer culture. After treatment, the intensity-dependent effects of LIUS was evaluated by measuring cell viability, proliferation and by analysing cell morphology and expression of cartilage specific ECM using Western blot, RT-PCR and Northern blot analysis. Results: 1. In the explant culture condition, there was no effect of LIUS on chondrocytes proliferation but the expression of proteoglycan and collagen was increased at 200 mW/cm2 intensity. 2. In three dimensional culture using alginate bead, proliferation of chondrocyte was not influenced but its viability was increased 1.6 times at 200 mW/cm2 intensity and synthetic rate of proteoglycan and collagen were also increased. At this time, the numbers of RER and mitochondria were increased in cellular level examined by TEM. 3. In monolayer culture, proliferation of chondrocyte was increased 1.5 times at 100 mW/cm2 intensity and production of type II collagen was increased. However, proteoglycan secretion was not changed. 4. The effect of LIUS on rabbit chondrocyte showed similar result with human chondrocyte's. Conclusion: LIUS has the stimulatory effects on proliferation of chondroyctes in monolayer culture, and has stimulatory effects on metabolism of the extracellular matrix of articular chondrocytes in explant and alginate culture. From these results, it was concluded that ultrasound has potentially important effects on arthritic cartilage in vitro, indicating a potentially important role in cartilage repair and in the treatment of osteoarthritic cartilage.
