An Influence of Home Visiting Hospice on the Basic of Social Support on the Old People's Attitude Toward Death원문보기

Dueto the increase of cancer patients and chronic diseases of old people, who have to face the death after a long painful process, as an increase of the aged in population, the needs of hospice care have come up to the surface. The problems of these people are not only restricted to an individual matter but to a family and even a social problem in the end with various factors interacting as a whole at a medical, psychological, social and financial level. They are continuosly fighting with themselves in the admist of psychological conflicts not to speak of physical suffering and it is not unusual to see them dying in dispair, loneliness and isolation with losing sights of true meaning of life. It is the reality in Korea that most of our hospice care pays attention mainly to the patients'symptom controls or nursing care, not to their social and financial issues which are of importance, too. The approach to the patients who are under hospice care should be made by the specialists who are professionally trained. Therefore the purpose of this study is to provide social support the old patients who are under hospice care at home and to find out its influence on their attitude toward death. 16 subjects out of 49 hospice patients from Y Hospital in G City starting from 1 September for 30 days were selected. 15 questions as a study instrument were used to see general characteristics and a structured questionaire on attitude on death with 16 questions which is an instument translated by Hyegyong Suh from FODS(Fearof Death Scale) made by Collect & Lester in 1969. Social support has been provided to the subjects for 76 days between 1 September and 15 November. SPSS Program for collected data were used. General characteristics were scored with real numbers and percentage. Analysis of questions before and after providing social support were scaled with its average and ranks. Individual comparison before and after providing social support was calculated in average. A paired t-test was used to score the total average, and a paired t-test was also used for a sectional analysis on attitude toward death The findings of the study are as follows : First, the highest ranked attitude (average 4.50) among vaious questions on an attitude toward death before providing social support was: "I am afraid to see myself losing memories and getting deteriorated" the lowest ranked (average 2.25) was : "I feel much comfortable not knowing about death". Second, the highest ranked attitude (average 3.44) among vaious questions on an attitude toward death after providing social support was: "I feel much comfortable not knowing about death"; the lowest ranked (average 1.94) was :If I were diagnosed to have an uncurable disease, I would want my children to inform me of the truth" Third, an analysis of total average of 16 subjects before and after providing social support showed a significant difference with an average of 3.4375 before and an average of 2.7227 after, which means that their attitude toward death has changed in a positive way (p<0.05). Fourth, the highest ranked attitude (average 3.70) was a fear of dying process but this was mostly influenced (average 2.73) by the amount of social support given. The next rank was a fear of dying of others indicating from 3.55 to 2.81, and it was followed by a fear of dying process of others indicating from 3.50 to 2.73. However, the fear of one's own death has not changed much, notwithstanding of social support, indicating little difference from 3.00 to 2.72. Generally speaking, the subjects with social support experineced a positive attitude and thinking until the end of their remaining days, awaring of their illness, facing problems in life, and opening to a religion. In summery, as the social support positively influence the dying patients'attitude toward death, it is thought the organizations or individuals who are involved in the care for the terminally ill patients should provide social support so as to help them to accept their death positively.
