Patients with inflammatory bowel disease(IBD), Crohn's disease(CD) and ulcerative colitis(UC) have chronic inflammation in the gastrointestinal tract. The cause of IBD is unclear, however dietary factors have been considered as a possible risk factor for IBD. The purpose of this study was to investigate nutritional status and dietary patterns of IBD patients in Korea. CD(n=22) and UC(n=28) patients and gender-matched healthy controls were studied in case-control design. Anthropometry and blood analysis were used to evaluate the patient's nutritional status. List of food preference and frequency questionnaire for 43 classified common foods were used to investigate subject's dietary pattern before and after diagnosis. Food preference was estimated by five likert scale and food frequency was estimated by semi-quantative food frequency questionnaire. Current nutrient intake status was determined by 3-day recall method. CD patients were in the mild-malnutrition status showing that their body weight, %IBW, body fat, LBM, TSF and MAC were lowest among the 3 groups. UC patients and controls were normal. Hb, Hct and WBC were also low in CD patients, particularly TLC and Alb were significant low in CD and UC patients. IBD patients over-ate rarely and ate less fat on meat than healthy control did. Before diagnosis, CD patients did not preferred vegetables(p<.05) and ate fishes, vegetables, and fruits less frequently. UC patients did not preferred vegetables and have high frequency for fruits and fast foods. After diagnosis, CD patients have low preference for vegetables, dairy products, and fats, and have low frequency for cereals, meats, vegetables, and fats. UC patients have low preference for fats and have low frequency for meats and vegetables. The current intake of UC and CD patients for calorie, Ca, Fe and Vit. B were lower than RDA for each nutrient. The P:M:S ratios were 2.1:1.9:1 for CD, 1.9:1.7:1 for UC, and 1.5:1.1:1 for control group. The ratio of N-6/N-3 were 15.8:1 for CD, 13.1:1 for UC, and 11.2:1 for control group. In conclusion, most IBD patients, particularly CD patients were malnourished because of their poor intake. Dietary intake patterns of IBD patients were low in dietary fiber and high in refined carbohydrate.
