SPSS 12.0을 이용하여 Chi-square 와 ANOVA 및 Repeated measure ANOVA를 이용하여 분석 하였다. 결 과 : 연령과 직업은 그룹 간에 통계적으로 유의한 차이가 있었으며, 다른 두 그룹보다 대조군의 연령이 더 낮았으며(p=0.004), 직업을 가지고 있는 사람도 더 많았다(p=0.058). 고혈압교육프로그램 실시 전 고혈압에 대한 자각도는 교육강화군이 가장 심각하게 인지하고 있었으며 통계적으로 유의하였다.(p=0.007) 반복측정분산분석시 행동과 염도 복부지방률은 그룹 간에 유의한 변화가 있었으며, 지식과 태도, 이완기 혈압, 운동 및 BMI는 그룹 간에 유의한 차이는 없었으나, 교육전과 교육 후 및 6개월의 시간경과에 따라 유의한 변화가 있었다. 행동은 교육강화군(p=0.02)과 교육군(p=0.02)에서 실천도가 교육전보다 교육직후에 향상되었으며, 염도는 교육군(p=0.02)에서만 교육전보다 교육직후 유의하게 낮아졌다. 복부지방률은 교육전과 교육직후, 교육직후에서 6개월경과 시에는 차이가 없었으나, 교육전과 6개월경과 후 비교 시 유의한 차이를 보였다. 지식(p<0.001)과 태도(p=0.05) 및 운동(p=0.003)은 교육 전에서 교육직후의 기간동안만 유의하게 향상되어졌으며, 이완기혈압(p=0.02)과 BMI(p=0.014)도 교육전에서 교육직후의 기간동안에만 유의하게 저하되었다. 수축기 혈압은 시간의 변화에 따른 유의한 변화는 없었으나, 교육전에서 교육직후의 기간동안은 유의한 변화가 있었다.(p=0.021) 결 론: 고혈압교육프로그램 직후의 시기에만 고혈압교육을 받았던 두 그룹에서 행동과 염도 및 복부지방률의 유의한 변화가 있었으며, 교육기간의 경과에 따른 변화 시 교육을 받지 않았던 대조군을 포함한 모든 그룹에서 지식과 태도 및 이완기 혈압, 운동 ,BMI가 교육직후에만 유의하게 변화 되었다. 이는 고혈압의 치료 중 중요한 부분인 생활습관의 변화가 단시간적인 교육을 통해서는 어렵다는 것과 단순한 측정만으로도 대조군의 변화를 유도할 수 있었다는 측면에서 통합적인 교육의 필요성이 요구된다. 이에 습관의 변화를 유도할 수 있는 실제적이고 구체적인 교육 프로그램이 요구되며, 지속적인 관리를 할 수 있는 체계적인 프로그램이 통합된다면 고혈압 환자의 건강관리를 비롯한 건강증진을 가능하게 하는 중재 전략으로 적용될 수 있을 것이다.

Objective: This study was intended to determine the effects of hypertension education programs on knowledge, attitude and practice in hypertension patients and to suggest an effective intervention in the treatment of hypertension. Methods: A total of 96 hypertension patients who were treated in a district clinic center in C city participated in the study. Of 96 subjects, 50 received the hypertension education program (the education group), the remaining 46 subjects served as the control group. The education group was further divided into the intensive education group (25) who underwent a secondary exercise program that lasted for three months starting 3 months after education and the exercise education group (25). All subjects'' blood pressure, blood sugar levels, levels of salinity, BMI, percent body fat and WHR were measured using bioelectrical impedance analysis and indicator before, just after education and 6 months after education. A structured question was also served to all subjects at the same times to identify changes in knowledge, attitude and practice. Chi-square, ANOVA and Repeated measure ANOVA were performed for statistical ananlysis, using SPSS 11.0. Results: A statistically significant difference was seen between age and occupation. The mean age of the control group was lower than that of the education groups (p=0.004). A larger number of subjects had a job in control group (p=0.058). The self-awareness of hypertension symptoms was higher in the intensive education group and the difference was statistically significant (p=0.007) Repeated measure ANOVA test revealed significant differences in practice, levels of salinity and waist-hip ratio between the three groups. No significant differences were observed in knowledge, attitude, diastolic blood pressure, exercise and BMI between the three groups. But significant differences were found in these factors as time passed, when three measures taken before and immediately after education and 6 months after education were compared. The number of subjects who took action in an effort to lower their blood pressure increased in the intensive education group and the education group immediately after education, when compared with baseline measures (p=0.02 for both). Levels of salinity significantly reduced in the education group immediately after education, when compared with baseline measure (p=0.02). There was no difference in waist-hip ratio between the groups just after education and 6 month later of education from baseline measure. But a significant difference was seen when pre-education measure and measure taken 6 months later of education was compared. Knowledge(p<0.001), attitude(p=0.05) and exercise (p=0.003) in the education groups significantly improved immediately after education, when compared with baseline measures. The diastolic blood pressure (p=0.02) and BMI (p=0.014) significantly decreased immediately after education, when compared with baseline measure. There was a significant change in systolic blood pressure as time passed. But a significant change was observed, when two measures taken before and immediately after education were compared. (p=0.021) Conclusion: There were significant differences in practice, levels of salinity and waist-hip ratios in the two education groups immediately after education. Among changes associated with education period, significant differences were observed in knowledge, attitude, diastolic blood pressure, exercise and BMI between the three groups, including the control group, only immediately after education. The results of the study suggested that a short-term education program was not effective to help subjects adjust their lifestyle, which is considered an important factor in the treatment for hypertension. At the same time, this study found that simple measures could elicit changes in the control group, signaling the need for integrated education. A practical education program is therefore needed to help hypertension patients change their lifestyle and diet and help them stick to adjusted lifestyle. When the education program satisfies these two requirements, it can serve as an effective intervention in the treatment of hypertension.
