Patients with stroke have a problem with balance and posture control, and their standing and walking are disturbed by unsymmetric posture, abnormal physical balance, defect in weight shifting, loss of delicate functions and so on. These problems hinder everyday life activities, and secondarily reduce the stability of the body and increase the danger of falling. The hip joints of the lower extremities and ankle joints are important in balance control for physical stability. Comparing the patients who had experienced falling with those who had not, there was a significant difference in hip extensor muscle and ankle dorsiflexor muscle, not in other muscles. Therefore, this study investigated the effects of ankle strength exercise on the balance ability of stroke patients. The subjects were the patients with stroke(mean 54±11.4) who had been treated at Bobath Hospital in Bundang-gu, Seongnam-si as inpatients or outpatients, and at least six months had passed since the attack. Ankle strength exercise was performed from December, 2004 to February, 2005. The Timed Up and Go Test(Up & Go Test) and Messen Trairuieren Dokumentieren Systems for balance were measured and compared before and after the treatment. The results of this study were as follows: 1) After the patients with stroke had performed the ankle strength exercise, the TUG was decreased from 38.70 seconds before the exercise to 33.32 seconds after it, which had a significant meaning(p<.01). The details about this were as follows: First, investigating the balance ability according to gender, the male's TUG was decreased from 38.72 seconds before the treatment to 33.58 seconds after it, and the female's TUG was reduced from 38.65 to 32.59. Putting the men and women together, the TUG was changed from 38.50 to 33.32 and it was statistically significant(p<.01). Second, investigating the balance ability according to age, the TUG of the patients under 49 was changed from 35.57 seconds before the treatment to 30.34 after it. In the case of the patients between 50~59, the TUG was reduced from 37.71 to 31.76. The TUG of those over 60 was decreased from 42.78 to 38.17. Considering all the ages, the TUG was reduced from 38.70 before the excercise to 33.32 after it, and it had a significant meaning(p<.01). Third, investigating the balance ability according to the side of hemiplegia, the persons with right hemiplegia showed that the TUG was dropped from 44.89 before the exercise to 38.73 after it. The TUG of the left hemiplegic patients showed the change from 34.57 to 29.73. Putting the both right and left hemiplegia together, there was a meaningful change that 38.70 before the treatment reduced to 33.32 after it(p<.01). 2) The results of Messen Trairuieren Dokumentieren Systems after the ankle strength exercise showed the change from 128.60 before the treatment to 90.71 after it, and it was statistically significant(p<.01). The details concerned with this were as follows: First, investigating the balance ability according to gender, the male showed the reduction from 149.06 to 113.59 and the female represented the change from 67.24 to 22.06. In the case of the both genders, there was a significant reduction from 128.60 to 90.71(p<.01). Second, investigating the balance ability according to age, the subjects under 49 had the reduction from 150.70 to 120.10, and the persons between 50~59 showed the change from 128.56 to 85.68. Finally, the patients over 60 represented the decrease from 110.25 to 78.10. Considering all the ages, there was a significance that 128.60 before the exercise reduced to 90.71 after it. Third, investigating the balance ability according to the side of hemiplegia, the persons with right hemiplegia represented the reduction from 149.15 to 109.63. In the case of the left hemiplegia, there was a change from 114.91 to 78.10. Considering the both right and left hemiplegia, there was meaningful change from 128.60 to 90.71(p<.01). From these results of this study, the ankle strength exercise for three months has an effect on improvement of the balance and walking ability; therefore, the ankle strength exercise is considered to be important when the stroke patients are treated.
