Background and objectives: Although the pulmonary vasculature is imposed by a similar degree of pressure or volume overloading, the development or severity of secondary pulmonary hypertension (PH) shows the diverse inter-individual’s variability. We hypothesize that the lung tissue gene expression pattern of patients with secondary PH has a characteristic profile when compared with the gene expression pattern of structurally normal lungs. Methods: Using oligonucleotide microarray technology, we characterized the expression pattern in the lung tissue obtained 13 patients with secondary PH, which is due to congenital shunt disease (volume overloading, n=6) or valvular heart disease (pressure overloading, n=7), and form 5 patients with histological normal lungs. Results: There are a number of differentially expressed genes between secondary PH vs. normal subjects and volume overloading vs. pressure overloading type PH. All secondary PH lung tissue samples showed a decreased expression of genes encoding transcriptional factor (BHLHB2, EGR3, JUNB, KLF4, KLF6, and MAFF), cytoskeleton protein (VIL2 and XLKD1), cell differentiation and viability (MCL1, SNF1LK, and TNFAIP3). Volume overloading type PH shows an increased expression of genes encoding proliferation of pulmonary capillary endothelial cell (ESM1), cell proliferation (IGFBP2 and BMP6), collagen synthesis (COL4A2 and SERPINH1), and cytoskeleton (TMSL8). Importantly, we could distinguish by pattern comparison between pressure overloading vs. volume overloading PH, because of up-regulation in the expression of proliferation of pulmonary capillary endothelial cell (ESM1), cell proliferation (EGR2, PLK2, and TNC), and collagen synthesis (COL4A1) and down-regulation in the expression of inflammation (IL1RL1, IL7R, CCL5, CCL19, CXCR 6, and XCL1;XCL2) and immune response (IGHM and TRA@;TRAC). Conclusions: We conclude that the microarray gene expression technique is ad new and useful molecular tool that provides novel information pertinent to a better characterization and understanding of the pathobiology of the distinct clinical phenotypes of secondary PH due to various heart diseases. These findings may have important implications toward diagnosis, screening, and therapeutic strategy in the future of this disease.
