We collected 64 mitochondrial families with mitochondrial disorders of Korean origin, which were consisted of MELAS (25 cases), MERRF (5 cases), LHON (2 cases), Leigh (3 cases), KSS/CPEO (22 cases) and unclassified (7 cases). The purpose of this study was to identify causative mutations from the entire mtDNA and nuclear genes (MFN2 and GDAP1) which are related with the mitochondrial functions. For the screen of mtDNA mutation, we used a two-step screening strategy. First, eleven common point mutations (A3243G, T3291C, G3460A, T3271C, A8344G, T8356C, G8363A, T8993G/C, T10158C, G11778A and T14484C) and large deletion were examined. Then, we also continued the sequence analysis of the whole mtDNA in samples with no causative mutation from the first screen. We identified 18 point mutations (9 known and 9 novel mutations) and 4 large deletions (1 known and 3 novel deletions) from 30 families. Nine known point mutations were found from 20 families. The A3243G mutation in tRNAeuLeu was predominantly observed from 9 MELAS families, and followed by A8344G mutation in tRNALys from 4 MERRF families. Others were A856G (12S rRNA) in a Leigh family, T3271C (tRNALeu), T9957C (Phe251Leu, CO3) and T10191C (Ser45Pro, ND3) in each MELAS family, G5979A (CO1) in a CPEO patient, G8363A (tRNALys) in an uncertain type family, and G11778A (Arg340His, ND4) in a LHON family. From the whole mtDNA analysis, we could able to identify total of 9 unreported mutations in the MITOMAP database (http://mitomap.org) from 8 families. These mutations were not found in about 200 control samples. Four mutations (A2294G, A3145G, T5553G and C15891T) were found in the rRNA and tRNA genes, and other six mutations (G7119A, T9187C, C9717T, T11204C, C13438T and A13849C) were identified in the amino acid-coding regions. Of them, the A13849C (Asn505His) in ND5 was found in a MT48 MELAS patient who also harbors T9957C (Phe251Leu) in CO3. The T11204C (Phe149Leu) in ND4 was found in a MT64 LHON patient who also harbors G11778A in the same gene. From the large deletion screen, the common deletion (4977bp) was found from three patients (KSS and CPEO). One CPEO patient showed multiple deletions (5112bp and 9024bp). From the screen of the MFN2 and GDAP1 nuclear genes in patients with no pathogenic mtDNA mutation, we identified a novel C1376T mutation of the MFN2 gene, however, we didn’t identify any pathogenic mutation in the GDAP1 gene. MFN2 encodes a mitochondrial membrane protein, mitochondrial GTPase mitofusion, that participates in mitochondrial fusion and contributes to the maintenance. Mutations in the MFN2 gene have recently been reported to cause peripheral neuropathy, such as CMT2A, HMSN V, HMSN VI. It has been reported that patients with MFN2 mutations have a high incidence of CNS involvements. The C1376T (P456L) mutation was found in a family with clinical features of both atypical MELAS and CNS impairment. This mutation was not detected in the 200 controls. From the pedigree analysis, the mutation was transmitted from father to daughter. The affected members with the MFN2 mutation (P456L) had a right cerebellar infarction and lactic acidosis, but not had a peripheral neuropathy. Thus, MFN2 mutations have affinity to involve the nerve system at various levels, such as CNS and PNS system. Our findings emphasize that the whole analysis of mtDNA and related nuclear genes are worthwhile in the molecular diagnostic evaluation of patients with clinically probable mitochondrial disorders.
