MELAS환자 (MT53 가계)는 FASTKD2 유전자 내의 복합 이형접합 돌연변이를 확인하였다. 시토크롬c 산화효소 결핍증과 연관된 미토콘드리아성 뇌근병증 (encephalomyopathy) 환자에게서 FASTKD2 유전자의 동형접합 돌연변이가 보고된 적이 있으며 그 이후로 더 이상 보고된 적은 없다. FASTKD2 유전자의 c.613C>T (Arg205Ter), c.764T>C (Leu217Pro)를 제외하고 다른 변이들은 dbSNP, 1000g, ESP, ExAC 데이터베이스를 이용하여 다형성인 것으로 확인하였다. The purpose of this study is to identify pathogenic mutations in mitochondrial disease patients. Although the genetic diagnosis of mitochondrial disease has been conventionally dependent on the direct sequencing of major causative genes, it has low isolation efficiency in spite of laborious procedure. This study performed next-generation sequencing-based gene panel sequencing including mtDNA and nuclear 73 genes which mutations have been reported to the underlying causes of Mitochondrial diseases. Gene panel sequencing were performed in 40 Korean patients with mitochondrial disease. Three causative mtDNA mutations were identified from the gene panel data. Three causative mtDNA mutations were identified in the examined families. A m.11778G>A mutation in MT-ND4 gene was detected in a LHON patient (family ID: MT59), m.9176T>C (Leu217Pro) mutation in MT-ATP6 gene was detected in a Leigh syndrome patient (family ID: MT130), and a m.8344A>G mutation in MT-TK gene was detected in a MERRF patient (family ID: MT132). m.8344A>G and m.9176T>C mutations were found as the homoplasmy in the affected probands except for m.11778G>A mutation. Compound heterozygous mutation is identified in FASTKD2 gene which is located in nuclear gene about MELAS patient (family ID: MT53). A homozygous nonsense mutation in FASTKD2 was reported in the mitochondrial encephalomyopathy patients associated with cytochrome c oxidase deficiency, thereafter no more case has been reported. Thorough consideration of all variants and comparison with the dbSNP, 1000g, ESP, ExAC database revealed that most of the variants were polymorphic mutation, with the exception of c.613C>T (Arg205Ter) and c.764T>C (Leu217Pro) in FASTKD2. The missense mutation site (Leu217Pro) was highly conserved from mammals to fish. Also, GERP scores were high in the most mutation sites and In silico predictions of the effect of the mutation revealed that it is highly likely to be deleterious (SIFT, PolyPhen2), and that the mutant protein has decreased stability (MUpro) Application of NGS may reduce testing cost and time with high fidelity. In particular, heteroplasmic rates in mtDNA could be directly determined by counting altered reads from total read depth at the corresponding mutation site. Therefore, this study suggests that gene panel sequencing by NGS will be a powerful tool for the molecular diagnosis of genetically and clinically heterogeneous mitochondrial diseases.
