The present study was conducted for the elderly at sanitariums who were suffering from dementia with the aim to understand the impact of horticultural therapy on their sentiments and minor muscles. In terms of a research plan, using the before-and-after experiment design, among the four types of Solomon group designs, this study was conducted on 10 people as a control group and 10 as an experimental group, who participated in a total of 12 sessions of horticultural therapy, once every week from April to June of 2006. The horticultural healing program was conducted for 3 months, each with a 60-minute-session from 2 to 3 pm every Saturday afternoon, including the time for preparation and cleaning up afterwards. The topics of the program utilized tools safe for dementia patients and non poisonous plants. Since the elderly had poor perception and muscular reaction, the chosen programs were simple and had a high percentage of success rates. Moreover, in order to give them pride and self-confidence in horticultural activities, the jobs were repetitive in nature. As evaluation tool the control and the experimental groups were measured against GDS, self-respect scale, and emotional balance scale, while their minor muscle systems were also examined. In terms of a change in their depression, the control group showed an average shift from 18.7 to 24.0, which was statistically very significant. This indicated that while the experimental group underwent therapy, the control group became gloomier. On the other hand, the depression point of the experimental group, comparing before and after horticultural therapy, went down from an average of 22.2 to 14.4. This was statistically very significant as an indication of a decrease in depression. In terms of a change in the feeling of self-respect, the control group went down from 25.2 to 24.8, a statistically insignificant change which indicated no change in self-respect of the control group during horticultural therapy. The change in self-respect in the experimental group before and after horticultural therapy showed an increase from an average of 24.9 to 25.8, which was statistically significant. In terms of a change in emotional balance, the control group showed the change from an average of 14.7 to 13.3, which was statistically insignificant, indicating no change of emotional balance during horticultural therapy. For the experimental group there was an increase from an average of 14.2 to 15.9 in emotional balance when comparing before and after the experiment, which was statistically significant. In terms of minor muscles, grabbing power, side-holding, three-finger-tip holding were examined. In the case of grabbing power, the control group showed an average decrease of power in their left hands, which was yet statistically insignificant. Looking at their right hands, which showed an average decrease of power, showed a very significant change during the first half of therapy, and a significant change in the latter half. For the experimental group, their left hands showed an average increase of grabbing power, which was statistically insignificant. Their right hands showed an average increase of power between before and after, with a statistically significant change in their grabbing. In terms of side-holding power, the experimental group's left hands decreased in their average power, with statistically significant change between before and after experiment. There was a change on average in their right hands as well, with both the first and the latter halves having statistically significant changes, indicating a decrease of their function during the period of horticultural therapy. The left hands of the experimental group on average showed in increase in their side-holding power, with a significant change before and after statistically. Their side-holding power of the right hand increased on average, with statistically significant change in the latter part of therapy and statistically very significant change in the first half, and before and after experiment This indicated an increase in their side-holding power. In terms of three-finger-tip holding power, the left hands of the control group showed an average decrease of power, with statistically significant change in the latter part, and between before and after experiment. In their right hands too, three-finger-tip power decreased on average, with statistically significant change in the first half of experiment, indicating that their function had decreased during the period of horticultural therapy. For the experimental group, the three-finger-tip power in their left hands had increased on average, with statistically significant change occurring in the first and latter halves of the experiment and statistically very significant change especially in the latter half of the experiment. Their right hands also increased in power on average, with statistically significant change in the tree-finger-tip power during the latter half of the experiment and statistically very significant change between before and after experiment, indicating an overall increase in the three-finger-tip power. In summary, this study showed that horticultural therapy could impact patients' emotions through changes in depression, self-respect, and emotional balance for the control group and the experimental group and that it could also influence on their minor muscles through changes of power in grabbing, side-holding, and three-finger-tip holding. All of these results indicated that horticultural therapy could be employed as a method of socio-environmental therapy for patients at long-term sanitariums who suffered from dementia. It was difficult to generalize such results since the present experiment was conducted in a short period of time with a limited number of participants as control group and experimental group. However, considering that the elderly with dementia tend to stay at sanitariums for quite a long period of time, studies for developing a long-term program for performing horticultural therapy should be conducted more in the future.
