인공 추간판 디스크 전치환술 (Total Disc Replacement, TDR)의 목적은 요통의 원인이 되는 퇴행성 조직을 제거하여 분절의 운동성을 회복하고 인접 분절의 퇴행성 발병률을 감소시키는 것이다. 이러한 인공 추간판 디스크 전치환술에 대한 생체 역학적 연구에는 실험 연구 (In-vitro Test)와 유한요소 연구 (Finite Element Analysis, FEA)가 있다. 실험 연구는 사체 및 동물 시편을 이용한 직접적인 측정을 통하여 정확한 결과를 도출할 수 있는 반면 생체 역학적 실험 과정이 복잡하고 시편 확보의 어려움이 있다. 그리고 유한요소 연구는 많은 유한요소 모델의 준비가 쉬우며 변수 조절이 자유로워 다양한 결과를 도출할 수 있는 반면 많은 가정을 바탕으로 구현된 유한요소 모델의 지나친 간략화가 결과에 영향을 미칠 수 있다. 이처럼 실험 연구와 유한요소 연구는 상호 보완적인 기술로써 생체 역학적 연구 분야에서 많은 연구자들에 의해 이용되어지고 있다. 따라서 본 연구에서는 사체 시편을 이용한 실험 연구와 유한요소 연구를 병행하여 인공 추간판 디스크 전치환술의 시술 효과를 생체 역학적 측면에서 분석하고자 하였다. 실험 연구는 (1) 단분절 유한요소 모델의 검증을 위하여 사체 시편 (L3-4)을 이용한 압축 실험을 실시하였고 (2) 사체 시편을 이용한 실험 연구를 수행하기 위하여 MTS system과 결합된 하중 인가 시스템 (a non-constraining pure moment loading system)을 구축하여 동물 시편을 이용하여 시스템의 성능 확인을 하였고 (3) 실험 연구의 운동 분석을 위한 3차원 운동 분석 시스템의 정확도를 검증하고자 하였다. 또한 유한요소 연구는 사체 시편의 CT data를 바탕으로 단분절 유한요소 모델 (L4-5), 퇴행성 디스크 모델, 그리고 인공 추간판 디스크 (ProdiscⓇ-L)를 삽입한 시술 모델을 구현하여 (4) 척추체 전․후방 요소의 하중 분담 특성과 (5) 회전 중심의 변화를 분석하였으며, 다분절 유한요소 모델(L3-S1)을 구현하여 (6) 인공 추간판 디스크 전치환술의 시술 및 인접 분절의 척추 분절 간 회전 운동량을 평가하였다. 사체 시편을 이용한 실험 연구를 수행하기 위하여 pure moment loads가 인가 가능한 형태의 하중 인가 시스템을 구축하였다. 하중 인가 시스템은 고정되어 있지 않는 pulley에 하나의 wire를 평행하도록 연결시켜 같은 크기의 하중이 서로 반대 방향으로 작용하도록 하여 pure moment loads를 인가하는 형태를 갖추었다. 구축된 하중 인가 시스템에 대한 성능 확인을 위하여 돼지 요추 시편을 대상으로 총 5회의 연속 cycle (load/unload steps)로 굴곡/신전 운동에 대한 최대 10Nm의 pure moment를 인가하여 하중-변위의 결과 그래프를 획득하여 neutral zone을 측정하였다. 아울러 3차원 운동 분석 시스템의 정확도 검증은 motion analysis unit을 이용하여 각 축에 대한 이동 및 회전각의 운동성을 분석하여 system error를 분석하였다. 유한요소 연구는 구현한 유한요소 모델에 대하여150N의 압축 하중과 10Nm의 굴곡/신전 pure moment를 인가하여 척추체 전․후방 요소의 하중 분담 특성과 회전 중심의 변화를 분석하였고, 척추 분절간 회전 운동량은 hybrid protocol을 적용하여 시술 및 인접 분절, 그리고 정상 모델과 같은 운동을 하기 위해 요구되어지는 moment를 구하여 각각 비교․분석하였다. 하중 인가 시스템을 이용한 실험의 결과 그래프는 전형적인 하중-변위의 결과 그래프를 보여주었다. 이에 neutral zone의 정의에 따라 3번째 cycle 이후에 그 변위의 편가가 줄어들어 일정하게 유지됨을 알 수 있었다. 따라서 1, 2번째 cycle의 하중은 실험 연구를 위한 pre-condition으로 간주되어졌다. 3차원 운동 분석 시스템의 축 변위에 대한 최대 오차는 Y축에 대하여 0.22mm로, 축 회전에 대한 최대 오차는 Z축 (Lateral Bending)에 대하여 0.95?로 나타났다. 구현한 단분절의 유한요소 모델과 실제 사체 시편을 이용한 압축 실험의 stiffness 값은 각각 1566N/mm, 1679N/mm을 나타내어 본 연구에서 사용된 유한요소 모델의 타당성을 입증하였다. 이에 단분절 유한요소 모델을 이용한 하중 분담 특성은 굴곡 운동 시에는 시술 여부에 관계없이 척추체 전․후방 요소에서 6:4의 일정한 비율을 보였으나 신전 운동 시에는 시술 모델이 정상 모델에 비하여 후관절의 응력이 2.5배 증가하였다. 회전 중심의 변화는 퇴행성 디스크 모델은 정상 모델과 유사하게 L4-5 디스크의 중앙에 위치하였으며, 시술 모델은 정상 모델에 비하여 굴곡 운동 시에는 전방 아래로, 신전 운동 시에는 후방 아래로 이동하였다. 다분절 유한요소 모델을 이용한 각 분절의 회전 운동량 분석에서 퇴행성 모델은 시술 분절의 운동 증가 (8~25%)를 보였으며 시술 모델은 시술 분절의 운동 증가 (33~63%)를 보였으나 인접 분절에서는 운동 감소 (4~25%)를 보였다. 또한 시술 모델은 정상 모델에 비하여 1.8~2.5Nm의 적은 모멘트로 같은 회전 운동을 가능하게 하였다. 사체 시편을 이용한 실험 연구를 수행하기 위하여 구축된 하중 인가 시스템은 비 구속적인 형태의 pure moment loads가 인가 가능하며 이를 이용하여 생리학적 하중에 따른 생체역학적 실험 연구가 가능할 것으로 사료된다. 또한 유한요소 연구를 통하여 인공 추간판 디스크 전치환술의 영향을 예측해 본 결과, 비정상적인 회전 중심의 변화는 척추체 구성 요소의 응력 변화를 야기할 것으로 생각되며 이는 시술 분절의 응력 집중 결과로부터 확인할 수 있었다. 그리고 시술에 따른 분절의 운동성 회복은 보였으나 과도한 척추 분절의 운동량 증가나 비정상적인 운동 형태는 후관절 및 인접 분절의 응력 변화를 야기할 것으로 생각된다. 따라서 인공 추간판 디스크 전치환술은 후관절의 퇴행성 진행 및 인접 분절의 합병증 등이 발생할 우려가 있을 것으로 사료된다. 향후 연구에는 실험 연구를 위하여 구축된 하중 인가 시스템 및 유한요소 연구 결과를 바탕으로 사체 시편을 이용하여 인공 추간판 디스크에 대한 종합적인 분석을 수행하고자 한다.

The goal of total disc replacement (TDR) is to replace the symptomatic degenerated disc and restore the function of the spinal segment. There are two methods: in-vitro test and finite element (FE) analysis in the field of biomechanics. In-vitro test is possible to get the precision results that are measured directly using cadaver and animal specimen. But it has complexity of test in regard of a various variable and difficulty of preparing many specimen. FE analysis are easy to prepare many FE models and possibility of parametric study. On the other hand, it may able to be affected by oversimplification of FE model based on various assumption. In-vitro test and FE analysis are complementary technique, which is utilized by many researchers in biomechanics. In this study, we evaluated the biomechanical effect of TDR using in-vitro test and FE study. In order to make an experiment with cadaver specimen, we were constructed a non-constraining pure moment loading system with MTS system. A loading system was tested to a maximum of 10Nm in flexion and extension motion for a total of five cycles using a porcine lumbar spine. The neutral zone (NZ) was calculated from the resulting load-displacement curve. And the accuracy of 3-D motion analysis system was validated with motion analysis unit. System error was determined under each axis-displacement and axis-rotation. For the FE analysis of biomechanical efficacies of TDR, a FE model of the intact lumbar spine was created from computer tomography (CT) images of the vertebra(male, age 55, no pathologies). It is then altered to construct a FE model with severely degenerative disc (Grade IV) before surgery. Further, implantation of ProdiscⓇ-L was simulated into L4-5 disc space through anterior approach with removal of the nucleus, ALL, and anterior part of the annulus. All of the models were subject to 150N compressive pre-load and 10Nm pure moment at the superior endplate, while the inferior endplate was fully constrained. The FE models (L4-5, functional spinal unit) were used to predict the load-sharing characteristics and the changes in the center of rotation under flexion and extension motion. To study the intersegmental motion of the lumbar spine, hybrid portocol was used to flexion, extension, lateral bending, and axial rotation motion the FE models (L3-S1, multi-level motion segments) of lumbar spine. A loading system allowed the specimens to move without constraint. Physiological equal and opposite forces were applied to the superior vertebra using a loading system that created pure moment loads under flexion, extension, right/left lateral bending, and right/left axial rotation modes. The resulting curve of test using a loading system showed the typical resulting load-displacement curve. Based on NZ definition, deviation of displacement of each cycle was consistent values from third to five cycles. And first-second cycles were considered pre-condition for in-vitro test according to the previous study. 3-D motion analysis system showed that the maximum value of system error was 0.22mm in Y-axis displacement, 0.95? in Z-axis rotation. For the FE analysis, a constructed FE model was validated to compare the actual compression test using cadaver specimen (L3-4). The stiffness of actual compression test and prediction with FE model was 1566N/mm, 1679N/mm, respectively. The FE model prediction was in very close agreement with the experiment data. From our FE results, the load-sharing characteristic between the anterior (vertebral body and intervertebral disc) and posterior (facet joint) columns remained relatively unchanged in all models under flexion (about 60% an d40%, respectively). However, during extension, 34% of the total load was shared by the facet joint in the intact FE model. On the other hand, in the degenerative disc and the implanted FE model, the load supported by the facets increaed to 35%, 86%, respectively. The center of rotation in the degenerative disc case remained unaffected near the mid-section of the L4-5 disc space in flexion and extension. On the other hand, after TDR implantation, the location of the center of rotation shifted to near the inferior endplate of the ProdiscⓇ-L in flexion and extension. In kinematics analysis, we found that the implanted FE model was required to less moment (1.8~2.5Nm) than intact FE model to experience same range of motion with intact FE model. At the implanted level (L4-5), the degenerative disc FE model allow more intersegmental rotation than the intact FE model by 8~25%. The range of motion of the implanted FE model increased by 33~63% at the implanted level and decreased by 4~25% at the adjacent level. In the study, a non-constraining pure moment loading system was constructed to in-vitro experiment test using cadaver specimen and a computational approach was used to evaluate the load-sharing characteristics, the changes in the center of rotation, and intersegmental motion of TDR implantation. A constructed loading system may be tested for the biomechanical study under physiological conditions based on flexibility test protocol. From the FE study, downward shifting of center of rotation following TDR means increase in moment arms in flexion and extension, which would subsequently alter load distribution between the spinal elements. As the results, we found that the motion segment became more mobile and imposed more stresses on the facet. It remains to be seen whether this change of the load-sharing, center of rotation, and kinematics may lead to further facet degeneration at the implanted level and additional disc degeneration at the adjacent levels. In the further study, we are needs to address a comprehensive analysis of TDR implantation based on the in-vitro test using a loading system and FE study under various condition.

As the results, we found that the motion segment became more mobile and imposed more stresses on the facet. It remains to be seen whether this change of the load-sharing, center of rotation, and kinematics may lead to further facet degeneration at the implanted level and additional disc degeneration at the adjacent levels. In the further study, we are needs to address a comprehensive analysis of TDR implantation based on the in-vitro test using a loading system and FE study under various condition.
