The aim of this review was to assess the effects of acupuncture on stroke. Twenty-eight databases and twelve major Korean traditional medicine journals were searched without language restrictions from their inception until September, 2008. We included randomized clinical trials (RCTs) testing the effects of acupuncture with or without electrical stimulation as a sole treatment for stroke and sham acupuncture. Methodological quality was assessed using the modified Jadad score and PEDro score. We searched 598 potentially relevant studies, of which ten RCTs met our inclusion criteria. Nine RCTs assessed acupuncture for functional recovery, and one RCT tested acupuncture for insomnia as a secondary symptom following stroke. Eight high quality RCTs failed to show favorable effects of acupuncture on stroke rehabilitation, while one RCT showed favorable effects of acupuncture on stroke. The other RCT showed significant effects of acupuncture on sleep control. The results of meta-analysis failed to show any beneficial effect of acupuncture on global neurological defect [n=152, standard mean differences (SMDs) -0.01; 95% confidence intervals (CIs) -0.33, 0.30; P=0.93; heterogeneity: Tau²=0.00, Chi²=0.02, P=0.88, I²=0%]. Three RCTs testing acupuncture failed to show favorable effects on activities of daily living (ADL) after the intervention period [n=244, SMDs 0.07; 95% CIs -0.18, 0.32; P=0.58; heterogeneity: Tau²=0.00, Chi²=1.30, P=0.52, I²=0%] or during the follow-up [n=244, SMDs 0.10; 95% CIs -0.15, 0.35; P=0.43; heterogeneity: Tau²=0.00, Chi²=0.18, P=0.91, I²=0%]. Inconclusion, sham controlled RCTs failed to show specific effects on the functional recovery of stroke patients, but this finding was inconclusive due to several limitations of the RCTs included.
