About hospitals under 300 beds account for 87% of the nation’s hospital industry and they provide the key service to the health and welfare of our nation. There are three main factors causing the dilemma which, in turn, hindering the operation of the small & medium hospital industry. These factors are as follow : 1. External factor - There are external factors which are into three classes such as the sides of medical demand, medical supply and medical system. In the beginning, the sides of medical demand are increasing old age, changing aspect of disease from acute disease to chronic disease, improving of national thinking level, increasing of the national income and decreasing of patients. Secondly, the sides of medical supply are imbalance of medical manpower supply & demand and deficiency of medical manpower especially speaking of nurse manpower, that is serious. Due to increasing medical institutions, competition between medical institutions and others has been increasing with large sized the general hospital. The third, the sides of medical system are ineffectual in some ways which means medical treatment processes system and accepting of international medical service trade. 2. Government regulations - There are also government regulations which are into three classes such as regulations of hospital management, medical law, and tax law. The regulations of hospital management are control of charge for medical treatment, compulsory appointment of medical institution applied with medical insurance. Moreover, regulations of medical law are prohibited in making profits of non profit making medical corporation, control of extravagant medical advertisement. On the other hands, regulation of tax law is inequality imposition of non profit making medical corporation to compared with social welfare corporation and school corporation. 3. Internal factor - Internal factors are weakness of financial ability, difficulty of security on nurse manpower, increasing of medical personnel expenses, lack of capable administrator and lack of leadership by chief executive officer. Poor operational management is running now, because poorly designed organizational structure, not enough chief executive manager and lack of qualified hospital staff(s). In order to overcome the above factors, following actions are needed to activate small & medium hospital industry : reduce the government regulations to raise the charge for medical treatment and introduce the profit-making medical corporation, and emphasize the internal abilities of the hospitals for generating profits, increasing efforts to attract hospital staffs, high quality customer service, increasing market, and specialization of medical treatments.
