Background : The aim of this study was to compare and analyze retrospectively the factors which effect for postoperative pulmonary complications in patients who underwent esophagectomy due to esophageal cancer. Material and Method : A total of 116 patients were enrolled in the study from January 1994 to June 2008, and patients with previous neoadjuvant chemotherapy or radiotherapy were excluded. Of the total 116 patients, 51 patients(44%) developed complications within 30 days of their operation and of them, the most common complication was pulmonary complication, and occured in 27 patients(23%). Patients were divided into two groups: In Group A, 27 patients with postoperative pulmonary complications, in Group B, 89 patients without pulmonary complications. Preoperative evaluations included pulmonary function test, underlying diseases, and operative and postoperative evaluations included histological type, cancer location, pathologic stage, operative duration, the thoracic root of the gastric conduit and anastomotic site and vocal cord palsy. Result : Of the total 116 patients, 51 patients(44%) developed complications, and of them, the most common complication was pulmonary complication and occured in 27 patients(23%). There was no significant difference between two groups for preoperative pulmonary function test. In Group A, there was significantly high in patients with diabetes mellitus(p<0.05), and preoperative cirrhosis, hepatitis, and other liver diseases were related to the incidence rates of postoperative pulmonary complications(p=0.005). But according to preoperative hypertension, there was no significant difference. No statistically significant difference was found in the incidence rates of postoperative pulmonary complications associated with histologic type of cancer, cancer location, postoperative pathologic stage and operative duration. There was an association between the thoracic root of the gastric conduit and anastomotic site and postoperative pulmonary complications. Consequently cervical anastomosis through the retrosternal route had more common complications than thoracic anastomosis through the postmediastinal route on incidence rate. There was no significant difference between two groups for postoperative vocal cord palsy. A statistically significant difference was found in the incidence rates of postoperative pulmonary complications in patients aged 65 years or older (p=0.03). 10 postoperative pulmonary complications occurred in 20 patients with preoperative lung problem; there was significantly high incidence rate in preoperative lung problem compared with normal group(p=0.007). Conclusion : Clinical factor for the pulmonary complications after esophagectomy of esophageal cancer was significantly associated with diabetes mellitus, liver disease, cervical anastomosis through the retrosternal route, age (65 years or older) and FEV1(<80%).
