The utilization of medical institutions has increased a lot by the implementation of National Health Service and patients who get operation also increased as numerous diseases are now treated by operation thanks to the advance in modern medical science. According to the National Statistics portal, total 39,420 tonsillectomy and chronic adenoid-disease operations had been done during 2007, which are the 8th most operations by disease. Among them, the tonsillectomy operations on children/juveniles between 1 to 19 years-old are 72.9%, which are 957 persons per 100,000 persons. Meanwhile, most of the operation patients think the operation itself as a very serious matter. The unfamiliar environment and pain give inconvenience and worry to the patients and increases the pain after the operation. It is reported that 64% of operation patients suffer serious pain. Especially, children before the operation suffer more worry than adults because of worry on bodily harm, separation from parents, short of knowledge, uncertainty on future events and the loss of controllability. This worry of children gives negative impact on the vital sign, pain and complication after the operation. It also delays the recovery after the operation. With such background, this research was done with purpose to establish an independent nursing intervention by finding out the impact of planned pre-operation information provision to the children, who will get tonsillectomy and their mothers on worry, pain, knowledge and satisfaction on nursing. This research is a quasi-experiment research of non-equivalent control group pretest-posttest design. Data were collected from July 2010 to December 2010. Research subjects were 11 experimental group children and 15 control group children, who had been admitted to the clinic for tonsillectomy, and their mothers in the Otorhinolaryngology (ENT) Department of an university hospital in D City. The research method was as following. Both experimental group and control group were visited one day before the planned operation. The experimental group children and mothers were given information using the education material prepared by this researcher. The control group children and mothers were given information based on the basic material. Post-survey was made two days after the operation. The collected data were analyzed by SPSS program, PASW 18.0 version. Homogeneity test of the experimental group and control-group were examined by X²-test and Mann-Whitney U test. The differences in worry, pain, knowledge and satisfaction on nursing were analyzed by Mann-Whitney U test and Wilcoxon Signed Ranks test. 1. The experimental group who received the information before the operation and the control group who did not receive the information before the operation did not have significant difference in worry after the operation. 2. The experimental group who received the information before the operation and the control group who did not receive the information before the operation did not have significant difference in postoperative pain. 3. The experimental group who received the information before the operation had more knowledge than the control group who did not receive the information before the operation (U=27.500, p=.004). 4. The experimental group who received the information before the operation and the control group who did not receive the information before the operation did not have significant difference in satisfaction on nursing regarding the education aspect. 5. The experimental group who received the information before the operation had significant difference in the sub-items of satisfaction on nursing compared to the control group who did not receive the information before the operation. In conclusion, information provision before the operation did not make statistically significant differences in pain of children after the operation, pain of children as perceived by the mothers, worry of the mothers and satisfaction on nursing; however, it reduced the extent of worry and pain. Satisfaction on nursing also increased in a limited extent. It was also found that information provision is an effective nursing intervention because the experimental group showed higher knowledge level than the control group.
