The therapeutic values of horticultural occupation were accepted in the various fields of socio-horticulture comprising horticultural therapy (HT). To enhance the values, approaching with the medical professionalism and developing the related horticultural therapy programs was required. For the interdisciplinary research between occupational therapy (OT) and horticultural science, this study was focused on establishing of the theory of horticultural occupation therapy (HOT) introducing horticultural activities as the means of OT and the practical application of it. From 2007 to 2010, a healing garden was prepared on the roof of Seoul Metropolitan E hospital, where HOT was performed. Relationship between the environments and the effects of HOT was analyzed as follows:
1. Occupancy Evaluation of Hospital Roof Garden as Healing Garden
Among 6 roofs of Seoul Metropolitan E hospital in 2007, A and B were constructed as healing garden and C, D, E and F as normal roof garden. All the gardens were evaluated by the use of Healing Garden Evaluation Scale (HGES) comprising 24 evaluation items. To evaluate these gardens, HGES was developed through modified Post-Occupancy Evaluation (POE). Garden A was evaluated as a adequate healing garden possessing the evaluation components such as safe access, various sensory factors and social functions, but garden B wasn't because of failing in providing safe success. In the social occupancy evaluation of garden A, the awareness of hospital staffs and the emotion of visitors were changed positively. In the healing occupancy evaluation, 59 patients taking advantage of roof healing garden showed decrease in negative emotion and increase in positive emotion (p<0.001). Especially, the effect of decrease in pain and increase in refreshed mind was the most prominent.
2. Effect of HOT utilizing Healing Garden on the Emotional Rehabilitation of Stroke patients
Thirty patients suffering from stroke were grouped into two groups, OT group as the control and HOT group as the treatment. The treatment group was provided with HOT utilizing the healing garden on the hospital roof in addition to OT but the control group was provided with only OT. They were provided with their respective trial one time per week for three months and then their scores in evaluation scales were inspected. In treatment group, geriatric depression scale (GDS) and sense of powerless (PS) were decreased, and stroke specific quality of life (SS-QOL) was improved. In control group, however, only SS-QOL was improved. In addion, from the closely positive relation between SS-QOL and GDS, HOT was considered helpful to the emotional rehabilitation of stroke patients.
3. Effect of HOT utilizing Healing Garden on the Emotional Rehabilitation of acute and chronic mental disorder patients
Twenty patients diagnosed as chronic mental disorder at Seoul Metropolitan E hospital and 22 as acute mental disorder at Incheon H hospital, were divided into 2 groups for HOT and Group occupational therapy (GOT). For three months, they were provided with their respective treatment, performed one time a week. Among the acute mental disorder patients, the treatment group showed a significant decrease in beck depression inventory (BDI), beck anxiety inventory (BAI) and emotional stress scale (ES), whereas the control group didn't (p<0.001). On the other hand, such significant changes were not seen among the chronic GOT group , Futhermore, the acute HOT group showed the significant effect in physical stress scale (PSE).
4. Effect of HOT utilizing Healing Garden on the Pre-Vocational Rehabilitation of the Chronic Mental Disorder Patients
From February in 2009 to November in 2010, the treatment group of 17 patients was provided with HOT utilizing a healing garden on the hospital roof and the control group of 12 patients was provided with art occupation therapy (AOT). Even through there was no difference in PSE and ES between two groups, the grooved pegboard test (GPT) was enhanced significantly with the treatment of HOT (p<0.001), suggesting that HOT is a mean for the pre-vocational rehabilitation of the chronic mental disorder patients. Also, horticultural performance (HP) and horticultural satisfaction (HS) were improved by HOT.
From the above result, constructing hospital roof as healing garden showed a significant effectiveness in improving therapeutic environments and enhancing therapeutic functions of HOT for the stroke and the mental disorder patients. Progressively, the indoor-and-outdoor HOT program introducing healing garden for the mental rehabilitation was proposed.
