Rehabilitation is essential for injured athletes to fully recover and return back on the field; however, rehabilitation can be a not only physically but also psychologically painful process for them. Sports injuries exert direct impact on athlete's psychological well-being, which in turn, directly influences on physical health, performance outcome, and risk of further physical injuries. There has been growing interest in the psychological aspects of the rehabilitation process for injured athletes; however, there are little empirical data available at the present time. The purpose of this study was to investigate which psychological variables explain the progress of rehabilitation and the discrepancy between physical recovery and psychological recovery. The participants of this study were seventy-nine athletes who were diagnosed by doctors or personal trainers as severely injured and are not able to participate in the game for more than four weeks. They were assessed individually three times: initial stage, intermediate stage and advanced stage of rehabilitation, which were identified by their personal trainers. At each assessment, psychological characteristics variables, stress-coping variables and sport motivation variables were examined by self-report questionnaires. Improvement in the physical functioning, the psychological recovery, and the participation rate in rehabilitation process were also assessed, which were then used as dependent variables. The data were analyzed by applying multilevel modeling, which was an appropriate method in analyzing repeatedly measured data for the analysis of change. To run the analyses, the software package HLM for Windows, Version 6.0 was used. First, multilevel modeling analyses demonstrated that psychological variables were differentially related to initial psychological recovery and psychological improvement trend. The psychological recovery proceeded in the positive direction throughout the rehabilitation process; however, this positive recovery process was faster in athletes who showed lower level of psychological recovery in the beginning than athletes with higher level of psychological recovery in the initial stage. Second, discrepancies between the physical and the psychological recovery was explained by distraction coping and mental toughness. In other words, those who showed a higher level of distract coping and low mental toughness showed less discrepancies at the beginning, but later showed bigger discrepancies. Third, the low psychological recovery group differed from the high psychological recovery group on the negative emotions and sport motivation. The low group experienced a higher level of negative emotion before the return to the field, and lower level of internal and external motivation on their interested sports than the high group. Overall, implications of this study are as follows. First, in order to promote positive rehabilitation outcomes, psychological factors such as coping skills, emotional states, and motivation levels must be considered. Second, physical recovery and psychological recovery following sport injury are not synonymous. In order to prevent another injury and further loss of confidence in performance, injured athletes should be fully consulted on their physical and psychological recovery and readiness to return to competition during rehabilitation period. Finally directions for future research and practical implications were discussed.
