Phantom(CT Dose phantom kit for adult head and body, Model 76-414-4150)과 100mm 전리함(CT Ion chamber), X-선 검출기(VictoreenR Model 4000M+)를 사용하였다.

Purpose : In the SPECT/CT scan, we evaluates the difference of the SPECT image of the attenuation correction where it adapts CT image former and after and confirms the change to the quality of the patient dose change according to CT dose change and image and reduces the patient dose tries to be sought. Materials and methods : The Flangeless Esser PET phantom was used in order to evaluate the quality of the image by using Precedence 16 SPECT/CT of Philips corp. the Phantom for CT dosimetry(CT dose phantom kit for adult head and body, Model 76-414-4150)and 100mm ionization chamber(CT ion chamber) and X-ray detector(VictoreenR Model 4000M+) was used for the patient dosimetry. In addition, the patient dose in the condition where it is actually similar to the test condition by using the ImPACT Dosimetry Calculator using the Monte Carlo method was evaluated. The SPECT image of the Flangeless Esser PET phantom and CT image was obtained in the experimental method, the SPECT imaging was corrected by attenuation map. The quality of the image was evaluated compared with the SPECT image which it set up ROI in the Hot spot of corrected image and not corrected image, then it measured SNR and FWHM, the image. The estimation of quality of the SPECT image set up ROI in the cylinder(25mm, 16mm, 12mm, 8mm) of the Phantom image which diversifies and obtains each CT condition and BKG and measured the SNR value through the coefficient comparison average counts. And it measured FWHM of the smallest cylinder(8mm) and compared the quality of the image. By using CT dose phantom for the patient dosimetry, the condition (kVp, mAs) of CT was diversified and each CTDI was measured. And by using the ImPACT Dosimetry Calculator, the CTDlw, CTDlv, DLP, and Effective dose was measured. Results : ① The result of comparing the SPECT image according to the attenuation correction application and non-application and SNR value about 4 cylinders were measured that there were 25mm-5.01 times, 16mm-4.77 times, 12mm-4.43 times, 8mm-4.38 times, and difference of BKG-5.13, and FWHM of 8mm cylinder did not have the difference in the phantom image which diversified the dose of CT and which it obtains irrelative of the change of the kVp and mAs in each condition nearly.(Table 10.) When evaluating SNR in the image and FWHM value we used 120 kVp and which diversified the mAs and which it obtains, it expressed the a little more excellent image. But it shows no statistically difference. ② The results of measuring the patient dose by using CT Dose phantom, there was 54 times differences were generated with CTDI in the minimum dose (90 kVp, 25 mAs) use and the maximum doses (140 kVp, 250 mAs) were used. The difference of CTDI of the conditions that it uses mainly in the general CT scan (120 kVp, 200 mAs) and conditions that it uses mainly in SPECT / CT scan (120 kVp, 50 mAs) was 4.2 times. ③ The results of using ImPACT Dosimetry Calculator experiments showed the 35 times difference of effective dose in the conditions which express maximum and minimum dose like CT dose phantom experiments. And the effective dose difference in the condition that it uses mainly in the general CT and SPECT/CT was investigated as 4.1 times. Conclusion : In SPECT/CT scan, the change of CT dose do not affect on the quality of SPECT / CT image which with the object of the attenuation correction and localization. And the dose increase of CT confirmed to improve the medical exposure about the radiation. In this way, presently the increase of use of SPECT / CT scan increases the medical exposure inevitably and seeing this situation with viewpoint as the patient dose reduction, we need to determine the dose of CT narrowly. We need to set the purpose of SPECT/CT scan clearly, when scanning for general attenuation correction or localization that we change the setting that can be lower CT dose and minimize the medical exposure about the patient.
