우리나라는 세계에서 가장 빠른 속도로 인구고령화가 진행되고 있는 나라중 하나이다. 노인인구의 급속한 증가는 장기요양보호를 받아야 하는 노인의 빠른 증가를 가져왔고, 핵가족화로 변한 가족구조는 효사상과 부양의식의 약화를 가져와 가족의 힘만으로는 노인을 케어 할 수 없는 심각한 사회문제가 되었다. 2006년에 조사한 통계청 「장래인구추계」에 따르면, 65세 이상의 노인인구는 1960년대 이후 증가하기 시작하여, 2000년에는 노인인구 비율이 7.2%로 고령화 사회로 진입하였고, 2007년 7월에는 노인인구가 총 인구의 9.9%에 달하여, 2018년에는 고령사회, 2026년에는 초고령 사회로 진입하게 될 것으로 예상하고 있다. 뿐만 아니라 노인들 중에는 한 가지 이상의 노인성 만성질환을 앓고 있는 사람들이 전체 노인인구의 90%를 넘었고, 타인의 도움을 필요로 하는 와상노인과 치매노인, 중증 장애노인, 독거노인 등이 크게 증가하여 노인성 질환의 유병율과 치료비용도 늘어났다. ‘2010년 고령자의 건강보험 노인의료비 통계’에 의하면 2009년 12월 현재 건강보험의 65세 이상 노인의료비는 총 12조 391억 원으로 전체 의료비 39조4천 300억 원의 30.5%를 차지하였으며 계속 증가하는 추세에 있다. 이러한 현상은 가족에게 과중한 경제적 부담을 주게 되었고 국민건강보험의 재정에도 악영향을 끼치고 있다. 많은 선진국들이 이미 이러한 문제에 직면하여, 우리보다 먼저 문제해결을 위해 다양한 노인요양보호프로그램을 도입하였다. 특히, 독일과 일본은 사회보험방식으로 노인문제를 해결하여 우리나라의 노인장기요양보험제도의 모델이 되었다. 그러므로 독일과 일본의 요양보험제도와 장기요양인력에 관한 연구는 우리나라의 장기요양보험제도의 활성화에 많은 도움을 주리라 생각한다. 이 논문에서는 독일, 일본의 노인요양보험제도와 요양인력의 활성화를 위한 노력을 비교․분석하여 공통점과 차이점을 도출하여 보기 위하여 Gilbert와 Specht의 정책분석틀을 적용하여, 제도의 도입배경, 적용대상, 급여의 내용, 전달체계, 재원조달, 그리고 요양보호사제도의 분석을 하였다. 도입배경의 공통점은, 인구의 고령화, 가족에 의한 요양보호의 한계, 국가재정 및 건강보험재정의 악화 등이고, 차이점은 독일과 일본의 도입 시기는 ‘고령사회’이고 한국의 도입 시기는 ‘고령화사회’라는 점이다. 적용대상의 공통점은, 독일과 일본은 부담의무와 수급권이 일치하고 있고, 차이점은, 한국은 보험료의 부담의무와 수급권이 불일치하고 있다는 점이다. 또 독일은 모든 국민에게 수급권을 부여하고 있지만, 일본과 한국은 수급권을 특정 연령계층으로 한정하고 있다는 점이 다르다. 뿐만 아니라 우리나라는 전 국민이 보험료를 내는 대신 수급자는 65세 이상의 노인과 노인성 질환을 가진 사람으로 제한하고 있다. 급여의 내용으로 독일은 현물급여, 현금급여, 혼합급여를 모두 인정하고 있으나, 일본은 현물급여를 원칙으로 하고 있다. 한국은 현물급여가 원칙이지만 예외적으로 현금급여를 인정하고 있다. 전달체계는, 독일의 관리운영주체는 장기요양금고이고, 일본의 관리운영주체는 시․정․촌이며, 한국은 국민건강보험공단이다. 독일의 재원조달방법은 국비의 보조가 없는 것이 특징이다. 그들은 보험료만 가지고 제도를 운영하고 있고, 일본은 보험료와 국비가 각각 50%로 구성되어 있으며, 한국은 보험료 80%, 국고부담20%로 구성되어 있다. 독일은 서비스제공의 전문 인력을 연방차원의 통합된 ‘노인수발직업교육법’을 만들어 뽑고 있다. 그들은 ‘노인 수발사’라 불리우며, 교육과정이 3년이고, 매년 이론 및 실기를 2000시간 이상 이수한 후 자격시험을 통해 뽑는다. 이렇게 전문화된 교육과정을 거친 후, 법적보호를 받으면서 시설과 재가서비스 분야의 중요한 수발인력으로 자리 잡게 하고 있다. 일본의 ‘개호복지사’ 도 고교졸업 후 지정된 양성시설에서 2년 이상이 전문교육을 받거나, 또는 복지계 대학 졸업 후 1년 이상의 전문교육을 받거나, 고교졸업 후 1년 이상의 전문교육 또는 실무경력 3년에 준하는 시설에서 교육을 받은 후에야 자격시험을 통과하여 자격이 주어진다. 그들의 양성교육기간은 총 1500여 시간이며 노인요양업무의 전문화를 실천하고 있다. 그러나 한국은 80시간의 이론과 80시간의 실기와 80시간의 실습 후에 자격시험을 거쳐 자격이 주어진다. 이것은 독일과 일본과 비교할 때 상대적으로 적은 시간과 노력으로 주어지는 것이다. 그러므로 장기적으로는 전문인의 양성이 되지 않아 서비스의 질이 낮아질 우려가 있다. 또, 우리는 다른 나라처럼 요양보호사의 학력제한이 없다는 것과 요양보호사의 5년 경력으로 장기요양기관을 설립할 수 있다는 점은 기회를 준다는 측면은 있으나 요양보호사의 자질의 저하를 가져와 총체적인 장기요양서비스의 질 저하를 가져올 수 있다는 문제점이라 할 수 있다. 노인장기요양보험제도의 최종적인 목적은 요양보호의 사회화일 것이다. 노인장기요양보험제도를 필요로 한 사회적 배경과 노인복지 향상을 위한 근본 취지에 따라 본 제도가 국가 책임 아래 있음을 분명히 주지할 필요가 있다. 물론, 현재 발생되고 있는 여러 요인들과 상황들을 복합적으로 검토하고 이에 대해 접근하여 발전시키려는 노력이 지속적으로 이루어져야 할 것이다. 우리나라 노인 장기요양보험제도의 보다 발전적인 방향과 요양보호사제도의 활성화를 위한 시사점은 첫째, 전 국민이 보험료를 내는 우리도 독일처럼 전 국민에게 수급권이 보장되어야 한다는 것이다. 물론 독일은 우리보다 9.3배 이상의 보험료를 내고 있어 재정부분에서는 국민간의 합의가 있어야 할 것이다. 둘째, 다양한 장기요양 욕구에 부응한 급여체계를 확보하되 독일처럼 본인이 현금급여와 현물급여 중에서 선택하게 하여야 한다. 셋째, 가족요양을 위한 가족의 노동력이 정당한 노동력으로 인정받아야 할 것이다. 이미 핵가족화 되어있는 가족구조 속에서 가족요양을 인정하지 않으면 가족구조의 해체로 이어질 수 있기 때문이다. 넷째, 다양한 재정안정화 방안 도입을 위해 독일처럼 민간보험을 강제로 가입하게 하는 방안도 고려해 보아야 한다. 다섯째, 등급 판정의 절차와 판정도구의 간소화로 경증환자들의 수급권 확대가 이루어져야 한다. 현실적으로는 맞벌이를 해야 하는데도, 국가에서는 어떤 보상도 없이 가족이라는 이름으로 돌보게 한다면, 또 다른 사회문제가 될 수 있기 때문이다. 여섯 째, 요양보호서비스의 질의 향상을 위하여, 요양보호사의 전문화를 위한 교육시간과 실습시간을 늘리고, 요양인력의 자질을 위해 학력제한을 하여야 한다. 정부가 요양인력의 확보에만 신경 쓴 나머지, 특별한 자격기준 없이 양성교육과 시험을 통해 누구나 요양보호사가 될 수 있는 길을 열어놓음으로서 낮아진 서비스의 질과 요양보호사의 자질문제는 심각하게 고민하여 해결하여야 할 것이다.

Moreover, as the family structure has been getting smaller, it caused to weaken the filial and supporting duty. As a result, it became a serious issue that senior citizens could not be cared only by their own families. According to "The Estimated Future Population" investigated by National Statistical Office, people aged 65 and over has started to increase since 1960s and Korea came to change into an aging society where the ratio of aged population hit 7.2%. In July 2007, the population of aged reached to 9.9% of the total population and now Korea is expected to be categorized as an society in 2018 and super-aged society in 2026. Besides, 90 percent of aging population suffer from at least one senile chronic disease and there increased the aged with dementia, severe handicaps, and Aged living alone who needs the help of others, and which increased prevalence rate and medical expense in Korea. Such views are putting a quite economic strain on the family and adversely affecting the finance of National Health Insurance. Confronting these problem, a number of advanced countries are adopting the various program for elderly care and nursing to solve the problem in advance of us. In particular, Germany and Japan became models of Korean National Long-term Care Insurance, by resolving problem of the aged in a way of social insurance. Therefore, it is thought that study on Long-term Care Insurance and workforce of Long-term Care in German and Japan would help Korea to invigorate Long-term Care Insurance of the aged. In this paper provide a comparative analysis in the efforts of Japan and Germany to vitalize the Long-term Care Insurance for aged by applying the 'Analytic Framework by N. Gilbert and H. Specht' and analysis for the background of introduction, the object to apply, benefit contents, delivery systems, financing and Nursing Care System. First, the common point for introduction background of them is Aging of Population, the limitation of nursing care by family and deterioration of national finance and health insurance. In case of the object to apply, Germany and Japan match up with obligation and pensionable right but Korea don't match up. And Germany authorize to all the citizen but Korea and Japan limits its right to particular ages. Third, regarding the benefit contents, Germany acknowledge three of In-Kind benefit, Cash benefit and mixed benefit but Japan only make the in-kind benefit as principal. Korea is based on in-kind benefit but exclusively acknowledge cash benefit. Regarding delivery system, the subject of operating management of Germany is Finance Long-term Care and that of Japan is City, Town and Village and that of Korea is National Health Insurance Corporation. In Germany, it is characteristics that they have no national support for financing. They manage the system only with insurance cost and Japan is composed of 50% for insurance cost and 50% for government expenditure. Korean financing is composed with about 80% with insurance cost and 20% with state liability. Germany made the federally integrated policy regarding training for senile care and hire the professionals for the service. They are called as "Aged-Care Worker" and the educational curriculum is for three years. They can complete the curriculum after take both theory and practice class for 2,000 hours. To be Care Worker in Japan, people have to be get specialized training in nominated facility for two years after graduating high school, or trained for a years after graduating the university related with social welfare and pass the licensing test. but, in case of Korea, people could be qualified when pass the licensing exam after curriculum for each 80 hours training and practice. This is relatively less time and effort comparing with Germany or Japan. That's why, it is feared that the quality of service could go downhill due to lack of the professional. Korea also has the aspect that they have no restrictions for academic background and people who has five years of experience can establish the facility of Long-term care, but it could lead to a decline in qualification so that it could damage the quality for general long-term care service. The final aim for National Long-term care Insurance is the socialization of Care system. People need to know that this system is under the responsibility of state according to the social background needed Long-term care insurance and main thrust for elderly welfare. Of course, we should continue to review the several factors and conditions which is currently occurred in combination, try to access and develop. The implication for more development in Long-term Insurance System is like below First, people should be guaranteed for right of benefit as they pay the insurance in Korea. Of course there need national consensus in the financial aspect as the German pay the insurance 9.3 times much than Korean. Second, We need to secure the salary system which meets the need of various Long-term care but let the employment choose among the in-kind benefit and cash benefit. Third, the workforce of family for family care should be acknowledged legally. Unless the family care is acknowledged, it could lead to the dissolve of family structure.
