Based on this understanding, the current study aimed to establish a foundational basis for implementing differentiated and specified nursing practices at specific stages for the better performance of nursing intervention in patients who have undergone colorectal cancer surgery. This study addressed the question of how the survival experience of patients with colorectal cancer changed according to the stage of their journey to recovery. To do this, it described and analyzed the survival experiences of patients who had undergone colorectal cancer surgery using an ethnographic method so as to understand the unique life of these patients. The study participants comprised a total of 13 subjects, of whom seven were male and six were female. All were diagnosed as having colorectal cancer and were in recovery from surgical resection or under follow-up observation after completion of treatment. Data was collected through in-depth interviews and observations from the out-patient unit or the in-patient ward of C university hospital from October 2010 to November 2011. The study results were analyzed according to the line by line reading method as well as by Agar (1980)'s three steps of event, theme and adaptation. The survival journey of patients with colorectal cancer surgery includes an initial confrontation of the diagnosis, followed by strong medical compliance, which then levels off, until they finally begin a new life as cancer survivors. First, we analyzed the patients’ initial reaction to their diagnosis, as a key part of the survival experience. The survival experiences identified from the confrontation to the diagnosis of colorectal cancer, as “evadable cancer”, “Resenting unfair and uneasy life”, “Fear” and “Hiding behind the Almighty”. Secondly, the patients went through a fierce process of adaptation while undergoing post-operative, active adjuvant chemotherapy. Major experiences presenting during this stage were the sense that it was a “painful but tolerable treatment process”, and the desire to maintain health “thorough diet control”, “regular exercise”, “intensive concern and cooperation of family”, and “positive interaction with fellow cancer patients”. They also encountered negative experiences, such as “getting hurt by the inconsiderate behaviors of friends” and the “temptation of inappropriate treatment-induced conflict”. Thirdly, survival experiences and behaviors that took place after this compliance had leveled off were “introspection”, “self-comforting”, “maintaining a healthy lifestyle” and “depending on religious faith”. Finally, the survival experiences that occurred during the long term survival stage were “thankfulness”, “a sense of relief”, an awareness that they had “unexpected extra time in life”, wanting to lead a more philanthropic life, and feeling happy about insignificant matters. There was also a feeling of being born again, which led to a sense of discreet mind. Based on these findings taken collectively, the following conclusion could be derived. After surgical resection for colorectal cancer, patients recalled that they were diagnosed at a point when they had been dismissive about their abnormal physical signs but had begun to develop a presentiment about the possibility of cancer. As they confronted the reality of colon cancer, either they sank into confusion from the emotional shock, or they accepted and tried to understand the existence of cancer through information acquisition, and depended on the positive words or attitudes of medical staff and social as well as spiritual support. They went through a journey involving a strong medical compliance, in which they established aggressive survival strategies during the aggressive process of post-operative adjuvant chemotherapy. After the completion of aggressive treatment, their compliance leveled off and they converted to defensive survival strategies that were designed to reinforce their physical health. And then, finally, they began to live a new life as cancer survivors, in which they experienced positive changes. As aforementioned, this study may have significance for researchers or practitioners who are preparing nursing intervention strategies that involve planning differentiated and specified nursing practices according to specific cancer stages. When performing these nursing interventions for survivors of colorectal cancer, an in-depth understanding of patient survival experiences may accompany improvements in long term survival processes.
