본 연구의 주요 결과는 다음과 같다. 1) 수술실 간호사가 지난 1년간 수술실에서 경험한 안전사고의 빈도는 의료장비 오작동(61건), 거즈•바늘 등 계수의 불일치(53건), 검체 분실•검체 바뀜 등의 사고(34건), 낙상•화상•압박 등의 환자상해(15건), 수술 후 X-ray촬영으로 체내 이물질이 발견되어 재수술(11건), 오염된 기구의 사용(9건) 순으로 많은 것으로 분석되었다. 2) 수술실 간호사의 안전문화에 대한 인식은 평균 3.16으로 분석되었으며, 하부영역별로는 환자안전 분위기가 3.57로 가장 높았고, 다음으로 조직문화 3.38, 근무환경 3.12, 직무만족 3.00, 병원행정 관리 2.90, 스트레스 2.63 순으로 나타났다. 수술실 간호사의 안전관리 활동 수행도는 평균 4.23이었으며, 하부영역별로는 계수가 4.49로 가장 높았고, 다음으로 손상예방 4.42, 수술 전 확인 4.23, 검체관리 4.20, 의료장비 및 부속품 관리 4.10, 감염관리 3.69의 순으로 나타났다. 3) 수술실 간호사의 일반적 특성에 따른 안전문화에 대한 인식정도는 직위(F=3.499, P=.032), 총 근무경력(F=3.382, P=.020), 수술실 근무경력(F=4.166, P=.007), 주당 평균 근로시간(F=3.416, P=.035)에서 유의한 차이를 보였다. 또한 수술실 간호사의 일반적 특성에 따른 안전관리 활동정도의 차이는 연령(F=5.739, P=.004), 직위(F=7.094, P=.001), 총 근무경력(F=3.840, P=.011), 수술실 근무경력(F=6.193, P=.001)에서 유의한 차이를 보였다. 4) 수술실 간호사의 안전문화 인식과 안전관리 활동의 변수 간 상관관계는 유의한 정(正)의 상관관계(r=0.289, p<0.001)인 것으로 분석되었다. 5) 수술실 간호사의 안전간호 활동에 영향을 미치는 관련 특성에 대한 회귀분석에서는 근무시간, 수술실 안전관련 교육, 병원 안전관련 교육에서 유의성이 없는 것으로 나타났으며, 안전문화 인식의 하위 항목 중 환자안전 분위기와 직무만족이 안전간호활동에 유의한 영향을 미치는 것으로 나타났다.

This study analyzes safety culture awareness and safety management activity performance degree of operating room nurses to propose fundamental data for operating room safety culture improvement program and quality improvement of operating room nurses. Research data was conducted on 169 operating room nurses working at a university hospital located in Seoul by using survey, and statistical analyzing methods such as frequency, ratio, difference analysis, and correlation analysis were used. Followings are the results of this study. 1) Frequency of negligence accident experiences in operating room during last year were analyzed as medical equipment malfunction (61 cases), discordance of numbers in gauze•needle (53 cases), accidents of specimen loss•specimen change (34 cases), patient injury of fall•burn• pressure ulcer (15 cases), reoperation due to foreign substance discovery in body after surgery with x-ray (11 cases), and use of contaminated instruments (9 cases). 2) Awareness on safety culture of operating room nurses were analyzed as average 3.16, patient safety climate was the highest with 3.57 in subcategories, next was teamwork climate 3.38, working condition 3.12, job satisfaction 3.00, perceptions of management 2.90, and recognition of stress effects 2.63. Safety management activity performance degree of operating room nurses was 4.23, the highest coefficient was 4.49 in subcategories, next was injury prevention 4.42, confirmation before surgery 4.23, specimen management 4.20, medical equipment and parts 4.10, and infection management 3.69. 3) Awareness degree on safety culture according to general characteristics of operating room nurses showed significant difference in position(F=3.499, P=.032), employment history(F=3.382, P=.020), operating room experience(F=4.166, P=.007), and weekly average working hours(F=3.416, P=.035). In addition, difference of safety management activity degree according to general characteristics of operating room nurses showed age(F=5.739, P=.004), occupation position(F=7.094, P=.001), total work experience(F=3.840, P=.011), and operating room employment experience(F=6.193, P=.001). 4) Correlation between safety culture awareness of operating room nurses and variables of safety management activities was analyzed that 6 sub-composition factors forming safety culture had positive correlation with safety management activity variables. 5) Regression analysis on related factors affecting safe nursing activities of operating room nurses showed no significance in business hours, operating room safety related education, hospital safety related education. Among sub-categories of safety culture awareness, patient safety atmosphere and occupation satisfaction had significant impact on safe nursing activity.
According to the analysis of this study, patient safety atmosphere had biggest impact on safety awareness of operating room nurses. The higher the safety culture awareness, the higher performance degree of safety management activity is. Thus, solution of manpower shortage which impedes positive safety culture awareness is urgent, and efforts to instill patient safety atmosphere is required. Even though coefficient verification and damage prevention related performance degree is high for safety management activity, infection management was relatively low. Hence, continuous education and evaluation is needed for improvements. Specific measures to achieve practical effects on safety related education of operating room nurses should be discussed at operating room and hospital level. Furthermore, institutional support which is necessary for safety management practice such as periodic evaluation system and feedback system should be arranged.
