This study compared prospectively the diagnostic yield of sputum induction with bronchoscopy. Methods: The patients with suspected active PTB, who could not expectorate sputum or who had AFB smear-negative sputum were included between February 2010 and July 2011. They underwent a sputum induction test and bronchoscopy. The concordance of the two tests and sensitivity of the AFB smear, mycobacterial culture and TB-PCR were evaluated. Results: Of 122 subjects who completed both tests, 68 were diagnosed with active PTB. The results of the AFB smear in induced sputum and bronchial washing fluid were concordant in 90.9% (111/122 cases, κ=0.735). For the mycobacterial cultures, the results were concordant in 86.9% (106/122 cases, κ=0.736). There were no significant differences between the sensitivities of the AFB smear and mycobacterial culture between induced sputum and bronchial washing fluid. Conclusion: In this study, sputum induction had diagnostic yield and sensitivity similar to bronchoscopy for diagnosing active PTB. For patients from whom it is difficult to collect sputum, or who have AFB smear-negative sputum, the sputum induction test is an alternative approach for the diagnosis of active PTB.
