Introduction: Pain is the sensual or emotional experience to the actual or potential impact. In general, it is reported that pain is accompanied in the dental treatment as well as orthodontic treatment. In the orthodontic treatment pain is maximum after 1-2 days and then almost disappeared at next visit. Most of patients are motivated well so they stand the pain. Therefore, it is hard the dentists know how much pain the patients bear. The degree of pain is varied by individual. It is reported that more than 90% have pain during the orthodontic treatment and 8% want to stop the treatment due to pain. Klepac et al. reported that 77% suffered from the general dental practice. The patients undergoing the orthodontic treatment have more and frequent pain comparing to the patients received the general practice. In Oliver & Knapman, anxiety about pain aroused by the orthodontic treatment made the people give up the treatment and it meant the orthodontists and assistants should be concerned about the pain. Pain by the orthodontic force was divided into immediate pain and delayed pain by Burstone and immediate pain was produced by pressure of PDL and delayed pain was by hyperalgia of PDL. The pressure and extension of PDL is occurred by the orthodontic force in all of the cases. Therefore, the orthodontists should recognize the pain of the patients undergoing the orthodontic treatment. This study is to compare the severity of pain between the patients felt and the orthodontists and dental hygienists recognized in the specific orthodontic practice, and to know how much the operators sympathize with the severity of pain. Method: The subjects were the patients received full mouth orthodontic treatment in the department of orthodontics, G university hospital, Incheon. The dentists were trained with residentship of Orthodontics and have treated only orthodontic treatment. The dental hygienists worked in the orthodontic clinic. 601 patients ( 117 extraction, 76 stripping, 122 orthodontic mini-implant installation, 166 bracket removal, 120 wire insertion), 84 orthodontists and 121 dental hygienists were answered questionnaires. The degree of pain was evaluated by 100 ㎜ horizontal VAS method. The severity of pain was checked immediately after stripping and bracket removal, and recorded 24 hours after premolar extraction for the orthodontic treatment, installation of orthodontic mini-implant and insertion of early leveling wire and then questionnaires were withdrawn at next visit. Results: There is no significant difference in the pain of extraction among the patients, orthodontists and hygienists. In stripping the orthodontists (p<0.05) and hygienists (p<0.01) thought pain is more greater than that the patients felt actually and it showed statistically significant difference. The orthodontists tended to expect less pain in orthodontic mini-implant installation but it was not statistically different. However, the hygienists expected more pain than that the patients (p<0.01) felt and the orthodontists (p<0.001) expected and it was significantly different. In bracket removal, both the orthodontists and the hygienists expected more severe pain than that the patients felt and it was significantly different (p<0.001). In wire insertion for leveling the orthodontists and the hygienists expected more severe pain and it was significantly different (p<0.001). It was evaluated that the influence on pain by age and sex of the patient in specific orthodontic practice. Pain was significantly different by age in orthodontic mini-implant installation (p<0.05) but the others were not influenced by age and sex. It was evaluated that the influence on the severity that the orthodontist expected by sex and career of the orthodontist. Pain was significantly different by sex in stripping (p<0.05) but the others were not influenced by sex and career. It was evaluated that the influence on the severity that the dental hygienist expected by career of the hygienist. It was significantly different by career in extraction (p<0.05), orthodontic mini-implant installation (p<0.01), bracket removal (p<0.05) and wire insertion (p<0.01). Conclusion: This study shows the orthodontists and the dental hygienists should be concerned about the pain of the patients undergoing the orthodontic treatment and make efforts to reduce the pain.
