Currently, according to health insurance system in Korea, nursing management fee consists of 25% of total hospitalization management fee, meaning that nursing activities provided by nurses are unde- restimated. Thereupon, many researchers in Korea have been proposing to establish an independent optimal nursing reimbursement system("NRS") since 1980s, by presenting optimal nursing cost based on cost analysis. However, in spite of such efforts for the NRS, they failed to realize the anticipated result so far. This is mainly due to the discordance with government policies or the burden of increasing healthcare expenditures comparing to the current national health insurance system called "Fee for service" Besides, in preceding studies, Korean researchers had confused the concept of nursing cost with the nursing fee and had failed to distinguish them. These may bring about the accuracy and reliability issues and, at the same time, will make it difficult to generalize the preceding studies. This article analyzed the trends of domestic researches regarding the nursing fee and tried to find out a proper way to set up an independent optimal NRS by groping for measures to include the new healthcare technology in the health insurance. According to literature review, there are total 62 Korean articles dealing with the nursing-fee from 1980 till 2012. The 63% out of preceding studies analyzed cost of nursing activities incurred by a specific field, implying that they are largely based on cost analysis. Because almost none of articles had implemented their survey on overall nursing activities, it is hard to generalize the findings of preceding studies and apply to clinical nursing practice and it is far deviated from the current health insurance system. As a way to implement the NRS in the national health insurance, this article suggests two alternative measures as follows: 1)to newly establish the NRS by seperating the nursing practices from existing medical practices, 2)to actualize the nursing management fee. The first measure could minimize the resistance because there will be no additional burden from the insurer and consumers pers- pectives. It is needed to follow the methodology suggested by HIRAS, and it is also necessary to reach a consensus with experts in other healthcare industries and policy makers. And it should be backed up by repeated studies for the effectiveness and safety of nursing practices as well as cost effectiveness. When it is difficult to achieve the first measure, it is needed to maintain current nursing management fee system but to actualize the fee up to the level of nursing cost can be covered. For the purpose of this, it is required to benchmark the previously mentioned Japan's nursing fee system equipped with diverse inclusion system. And we should apply the inclusion system firstly to the nursing practices for children, nighttime operation, or emergency department which is the same approach of other medical practices so as to secure the fairness of the system.
