The purpose of this study was to analyze the effect and examine the feasibility of an intervention of ankle strategy exercise on balance of patients with hemiplegia. Thirty patients with hemiplegia participated in this study and divided in two exercise and control groups. Ten patients from the control group(Group C) received conventional physical therapy (neurodevelopmental treatment) for 30 minutes. Patients performed the session 5 times a week for a total of 6 weeks. Ten patients from the ankle strategy group(Group A) received the same conventional physical therapy plus ankle strategy exercises for 20 minutes in one session. Ten patients from balance exercise group(Group B) received the same conventional physical therapy plus balance exercises for 20 minutes in one session. Two active groups were performed in the session 3 times a week, for a total of 6weeks. Assessment of balance was performed by the author, using moving distance of center of pressure(COP) in the anteroposterior(AP) and mediolateral(ML) direction, Berg Balance Scale (BBS), Timed “Up and Go” test (TUG) and Functional Reach Test(FRT). Data were analyzed using an IBM SPSS Statistics 18(2010), assuming an alpha level of .05. According to the results, all groups showed improvements in balance parameters. In especial, the group A was statistically significant differences in almost evaluation items except FRT and patients of Group A showed more improvements in moving distance of COP, BBS and TUG parameters than Group B and Group C. We concluded that ankle strategy exercises had more influence on balance than balance exercises and neurodevelopmental treatment. Therefore, ankle strategy exercises can be effective in improving the balance in patients with hemiplegia.
