Recovery-based philosophies have recently come under much attention in both mental health and addiction services and research. For social welfare services and practice based on the recovery perspective, evidence-based research is in need and a reliable scale development related to recovery is necessary.
The purpose of this study is to conceptualize ‘recovery’ and to develop a reliable and valid scale to measure recovery from alcoholism. Both quantitative and qualitative methods were used for the scale development. In order to extract the concept of recovery and its components, theories were reviewed and interviews and memoirs were analyzed. Items for each component were developed based on researcher’s clinical experience as well as on text materials used for alcohol recovery programs. After examining face validity by related experts and a pre-test, 64 items were identified. For the quantitative analysis, questionnaires were distributed to 409 adults who have been recovering for more than 3 months from 3 alcohol hospitals, 21 alcohol counseling centers, and 5 rehabilitation centers. For the analysis, 368 responses were used. Exploratory factor analysis and confirmatory factor analysis were conducted using SPSS 19.0 and Mplus 3.0.
Study results showed that from the exploratory factor analysis, 4 factors of ‘acceptance of alcohol problems’, ‘aspiration and challenge’, ‘managing a new life’, and ‘satisfaction toward life’ were extracted. From each factor, 6 sub-factors were extracted, resulting in a total of 24 factors. Confirmatory factor analysis indicated a valid model fit for the alcoholism recovery scale. To test the reliability of the scale, internal reliability and standard error of measurement were analyzed. Cronbach’s alpha for the whole model was .92, and .81 (acceptance of alcohol problems), .84 (aspiration and challenge), .81 (managing a new life), and .89 (satisfaction toward life) for the four factors. The test of reliability through standard error of measurement also indicated an acceptable reliability for the scale. Next, convergent validity was assessed for the scale. In order to analyze the correlation between two scales measuring an identical concept, the Client Assessment Summary (CAS) was utilized, and correlation with the quality of life was analyzed as a theoretically related variable. The correlation between alcoholism recovery scale and CAS was .75 (p<.01) and the correlation with the quality of life was .64 (p<.01), confirming the validity of the scale.
The study findings have several implications. A development of the first alcoholism recovery scale based on the recovery paradigm will lead to more expansive discussion in recovery in the mental health in Korea. Second, the scale development was based on the actual recovery experiences of addicts as well as on quantitative analyses. Therefore, it will be useful and reliable in measuring recovery from alcoholism. Third, the scale components were extracted multi-dimensionally, which allows for exploration and evaluation of a person’s overall aspect of the recovery. Fourth, the scale can be utilized to examine recovery program outcomes based the evidence based practice model, which may enhance addiction service quality enhancement. Finally, the scale development is expected to encourage future studies examining different variables related to recovery, which will serve as an evidence for program development and recovery promoting service environment.
