MSCs는 다른 세포와는 달리 자기 재생 (self- renewal), MSCs 특이적 세포 표면 항원의 발현, alkaline phosphatase (AP)의 활성, 높은 초기 전사인자 (Oct3/4, Sox2, Nanog)의 발현, 중배엽성 조직으로의 분화 및 생체 내에서 항염증, 면역조절 등의 특징을 가지고 있다. 이러한 MSCs는 다양한 조직에 분포하고 있으며, 이러한 다양한 조직 유래 MSCs는 기원 별로 각기 다른 특성 및 치료 효과가 보고되고 있으므로, 임상적용 시 성공적인 치료 효과를 위해서는 MSCs의 선택에 있어서 in vitro 및 in vivo 검증을 통한 잠재성이 있는 우수한 MSCs에 대한 연구가 필수적이다. 그러므로 본 연구는 관절 유래 MSCs의 비교 특성화를 통한 우수한 MSCs를 발굴하고, 선택된 MSCs를 류마티스 관절염 동물 모델에 적용하여 치료효과를 규명하고자 하였다. 본 연구는 세 part로 나누어 실시하였고, Part I 에서는 돼지 MSCs의 정확한 유전자 분석을 위한 안정한 reference 유전자 발굴, Part II 에서 다양한 관절 유래 MSCs의 비교 특성화 및 발굴된 reference 유전자를 통한 유전자 검사를 통하여 가장 적절한 MSCs를 선정, 그리고 최종적으로 Part III에서는 선택된 MSCs를 류마티스 관절염 모델에 직접 적용을 통한 그 치료적 효과를 규명하였다. Part I에서는, reference 유전자도 실험 환경에 따라 변화할 수 있으므로, 돼지 MSCs의 정확한 탐구를 위하여 미분화/분화 상태에서도 안정한 reference 유전자의 탐구를 수행하였다. 세 마리의 돼지에서 골수, 지방 및 피부 유래 중배엽성 줄기세포를 분리하였고, 이를 지방, 뼈, 연골로 분화를 시행하였다. 12개의 잘 알려진 reference 유전자 (18S, H2A, HPRT1, GAPDH, TBP, RPL4, PPIA, B2M, ACTB, YWHAZ, SDHA 및 HMBS)를 선택하여 각 MSCs 및 분화 상태의 threshold cycle (Ct 값)를 qRT-PCR을 이용하여 측정하였다. 18S와 B2M을 제외한 나머지 10개의 유전자의 Ct 값에서 유의적인 차이가 확인됨에 따라, 비록 reference 유전자라 하더라도 분화에 따라서 그 발현양에 차이가 있음을 시사하였다. 안정한 reference 유전자 탐구 프로그램인 geNorm을 사용하여 미분화 상태, 지방 분화, 뼈 분화, 연골 분화, 모든 분화 상태의 다섯 가지 군으로 나누어 발현의 안정성을 분석해 본 결과, HMBS, YWHAZ 및 TBP는 미분화 상태와 지방세포로 분화 시에 가장 안정한 발현을 나타내었고, HMBS, SDHA 및 TBP는 뼈 분화와 모든 분화 상태에서 가장 안정한 reference 유전자임을 확인 하였다. 또한 HMBS, SDHA 및 YWHAZ는 연골 분화 시에 가장 안정한 발현을 나타내었다. geNorm에 의하여 분석된 각 군 별로 가장 안정한 세가지 reference 유전자 (NF3)와 추천된 7-10개의 reference 유전자 (NFopt) 사이의 피어슨 상관관계를 분석해 본 결과, 다섯 가지 군 모두에서 0.9 이상의 높은 상관관계를 보였다. 특히 HMBS가 다섯 가지 군의 NF3에 모두 속하여 있었으므로 이를 추후 실험의 reference 유전자로 결정하였다. Part II에서는 임상적용을 위해서, 관절 조직 유래 MSCs를 골수 유래 MSCs와 특징 비교 및 HMBS 대비 상대적 유전자 발현 비교 통하여 가장 효율적인 MSCs 후보를 선별하였다. 동일 개체의 돼지에서 골수 유래 (BM-MSCs), 활막 유래 (SM-MSCs), 활액 유래 (SF-MSCs) 줄기세포를 분리하였다. 세 MSCs 모두 MSC 특이적 세포 표면 항원에 있어서 양성을 나타내었고, 특히 SF-MSCs는 가장 낮은 MHC class II 발현을 보였다. SF-과 SM-MSCs는 BM-MSCs 보다 유의적으로 높은 Oct3/4, Sox2 발현 및 높은 증식력을 나타내었다. 모든 MSCs는 특이 분화 유도 조건하에서 지방, 뼈, 신경, 연골로 분화되었고, 각기 특이적 세포화학적 염색 및 면역 세포화학적 염색을 통하여 분화가 유도된 것을 검증하였다. qRT-PCR을 통한 HMBS 대비 조직 특이 유전자 발현 분석을 하였고, BM-MSCs가 지방 분화 및 신경 분화에 있어서 유도 후에 다른 MSCs에 비해 유의적으로 높은 관련 유전자의 발현 증가가 관찰되었다. 세 MSCs에서 뼈 분화 관련 유전자의 발현은 모두 분화 전에 비하여 분화 후에 유의적인 유전자의 증가가 관찰되었지만 군 간 분화 능력의 차이는 보이지 않았다. 하지만 연골 분화에 있어서는 SF-MSCs가 분화 기간에 따라서 관련 조직 특이 유전자의 증가 양상이 다른 두 MSCs에 비해서 안정적으로 나타났을 뿐 아니라, 조직학적 소견 및 aggrecan 발현 양상에 있어서도 안정적으로 분화가 유도됨이 나타났다. 따라서 SF-MSCs가 세포 확보 및 분리의 용이성, 높은 증식력 및 초기 전사 인자의 발현, 낮은 MHC class II 발현 및 안정적인 연골 분화 능력이 다른 MSCs 보다 우수하게 나타났고, 따라서 추후 실험인 관절 질환 동물 모델 적용에 있어서 잠재력 있는 후보 MSCs로 선정하였다. Part III에서는 최근 많이 보고되고 있는 MSCs의 면역 조절 능력에 초점을 맞추어, 관절 유래 MSCs를 류마티스 관절염 동물 모델에 적용해 보았을 때 그 면역 조절에 의한 치료적 효과에 대해 연구하였다. 이전 결과에서 선정된 SF-MSCs 및 BM-MSCs를 실험군으로 설정하고 PBS을 대조군으로 하여 콜라겐 유도 마우스 (CIA 마우스)에 적용하여 그 임상적 평가 및 조직학적 평가를 수행하였다. 관절 염증, 뼈 파괴, 연골 파괴 같은 류마티스 양 소견이 관찰된 CIA 마우스에 5일간 매일 SF-MSCs 또는 BM-MSCs를 3×106의 세포를 복강 내 주입하고, 대조군으로서 PBS를 주입하였다. MSCs 주입 후 CIA 마우스의 관절에서 임상 평가, 조직학적 평가, cytokine 분석을 수행하였다. 임상 평가 및 후지 발목 관절 굵기 측정에 있어서 MSCs 주입군이 대조군 보다 유의적으로 증상이 완화된 소견을 나타내었다. 또한 조직학적 평가에서는, H&E 염색 및 Safranin O 염색을 통하여 MSCs 주입군이 대조군보다 관절 조직의 뼈 파괴가 진행이 되지 않으면서 연골이 잘 유지되어 있는 것이 관찰되었다. 추가적으로 TRAP 염색 시에도 MSCs 주입군이 대조군보다 낮은 뼈 파괴 세포의 활성을 나타내고 있었다. 특히 그 정도를 수치화 하였을 때는 SF-MSCs 주입군이 BM-MSCs 주입군보다 유의적으로 더 개선된 치료효과가 나타났다. Cytokine 분석에서는 염증 인자인 TNF-α 및 IL-6의 양에 있어서는 군 간의 유의적 차이가 없었으나, 항염증 인자인 IL-10은 SF-MSCs 주입군에서 유의적으로 증가된 것으로 나타났다. 결론적으로 류마티스 관절염 같은 자가 면역 질환에, SF-MSCs는 면역 조절 기능을 통하여 치료적 효과를 나타내었다. 따라서 본 실험에서는 SF-MSCs의 확보 및 구축의 용이성, 높은 증식력 및 다능성, 낮은 면역 거부 반응, 면역 및 염증 조절 능력 등의 장점을 규명함으로써 자가 면역 질환 중의 하나인 류마티스 관절염에 적용될 수 있는 대체 치료제로서의 가능성을 확인하였다.

Rheumatoid arthritis (RA) is an autoimmune disease caused by systemic chronic inflammation that primarily attacks synovial joints, leading to articular cartilage degeneration, bone destruction and functional disability. Incidence of RA imparts a massive burden on health services worldwide and efforts to discover new medicines, such as TNF-α inhibitors and B-cell depleting therapies. However, these may have some side-effects and limited efficiency. Hence, it is important to investigate for development of new and more effective therapy for RA. On the basis of similarly physiological and genetic characteristics to human, research of porcine stem cells play an important role in experimental understandings in vitro and preclinical application to humans. Mesenchymal stem cells (MSCs) are characterized by unique properties, such as unlimited self-renewal, expression of MSCs specific cell surface antigens, alkaline phosphatase (AP) activity, high expression of early transcriptional factors, differentiation into mesenchymal lineages, anti-inflammatory and immunomodulatory capacity for clinical application. MSCs have also been established from various sources, however, those MSCs from different tissues were revealed different characteristics including therapeutic potential. For successful remedial effect of MSCs in clinical application, it is needed to comparatively investigate to various MSCs and select promising cell sources verified in vitro and in vivo experiments. The present study was therefore performed to characterize MSCs isolated from synovial sources, along with in vivo application into RA mice model, and comprised of three parts. Detailed researches were conducted to assess their stability of candidate reference genes in porcine MSCs for evaluating gene expression in further study (Part I), comparative characterization and gene expression against selected reference gene of MSCs from synovial tissues for selection of promising cell sources for clinical application (Part II), and finally comparative investigation of therapeutic potential in selected MSCs from synovial tissues in Rheumatoid arthritis (RA) animal model (Part III). In part I, because expression of reference genes varies depending on different conditions, finding out the most stable reference gene in porcine MSCs across before and after differentiation is prerequisite for reliable results in MSCs by qRT-PCR. MSCs derived from bone marrow (BMSCs), adipose (AMSCs) and skin (SMSCs) were isolated and in vitro differentiated into mesenchymal lineages such as adipocytes, osteocytes and chondrocytes. Twelve commonly used reference genes (18S, H2A, HPRT1, GAPDH, TBP, RPL4, PPIA, B2M, ACTB, YWHAZ, SDHA and HMBS) were investigated their threshold cycle (Ct values) by qRT-PCR. Without 18S and B2M, significant differences in average of Ct values were observed in ten genes in both before and after differentiation, consequently, these findings indicated the possibility that the expression patterns of each gene could be changed during differentiation process. Following analyzed stability of selected reference genes in MSCs before and after differentiation by geNorm software, the present study showed that HMBS, YWHAZ and TBP are revealed high stability in undifferentiated MSCs and differentiated MSCs into adipocytes, while HMBS, SDHA and TBP investigated high stability in differentiated MSCs into osteocytes and MSCs in before and after differentiation into whole lineages, and thus HMBS, SDHA and YWHAZ showed high stability in differentiated MSCs into chondrocytes. Moreover, Pearson’s correlation determined high correlation (r > 0.9) between the three most stable reference genes and optimal number of reference genes in each MSCs status. Consequently, HMBS, TBP, YWHAZ and SDHA were evaluated as optimal reference genes for precise normalization at target gene expression research in various status of porcine MSCs. Comprehensively, HMBS in the study is selected as a reference gene for normalization in further study because of stable expression across undifferentiated and differentiated MSCs. In part II, to evaluate the promising cell sources from synovial sources for clinical application, comparative characterization was investigated about distinctive features among bone marrow (BM-MSCs), synovium (SM-MSCs) and synovial fluid MSCs (SF-MSCs) from a single male pig. All three kinds of MSCs were positive for MSCs specific cell surface antigens, and SF-MSCs had less MHC class II expression. SF- or SM-MSC were showed high expression of Oct3/4 or Sox2, respectively, and investigated significantly increased proliferative potential than BM-MSCs. All MSCs were successfully differentiated into adipocytes, osteocytes, neurons and chondrocytes confirmed by cytochemical or immunocytochemical staining, and lineage specific gene expression analysis by qRT-PCR. Comprehensively, BM-MSCs presented high differentiation potential in adipogenesis and neurogenesis. All three kinds of MSCs expressed up-regulated osteogenic specific genes, however, SF-MSCs were evaluated the most stable gene expression and morphological patterns in chondrogenesis. Consequently, SF-MSCs were proposed the suitable source for clinical application to synovial disease animal model, due to easy acquisition and isolation, a good proliferative ability, high expression of early transcriptional factors, less expression of MHC class II and stable chondrogenic differentiation potential than BM-MSCs. In part III, based on recent studies that MSCs have immunomodulatory properties and immunosuppressive effects by direct cell to cell contact or by secreting soluble factors, MSCs derived from synovial tissues were evaluated the therapeutic potential of immunomodulation in RA animal model. Bone marrow derived MSCs (BM-MSCs) and synovial fluid MSCs (SF-MSCs) were investigated therapeutic potential by clinical estimation and histological analysis from collagen induced arthritis (CIA) mice. After confirming RA pathology in CIA mouse by detecting inflammation, bone erosion and cartilage destruction, the mice were injected 3×106 SF-MSCs or BM-MSCs as experimental groups, and PBS as a control into the abdominal cavity for the five day daily. After 20 days from MSCs injection, CIA mice were evaluated clinical score, histological analysis and cytokine analysis. Clinical score revealed that MSCs injected CIA mice recorded low values of arthritic score and hind paw thickness than PBS control. Histological analysis by H&E staining and Safranin O confirmed the maintenance of completed joint structure and undestroyed cartilages in MSCs injected CIA mice than the control, and especially, SF-MSCs injected CIA mice were determined better histological score than BM-MSCs injected mice. TRAP staining showed that MSCs injected CIA mice have less active osteoclast than control. Results from cytokine analysis, the levels of TNF-α and IL-6 revealed no significant differences in all groups but IL-10 an anti-inflammatory cytokine was significantly increased in SF-MSC injected CIA mouse. In conclusion, SF-MSCs can be a promising cell source in alternative therapy for immunomodulation of autoimmune diseases such as RA. SF-MSCs have multiple advantages such as easy collection, cell isolation, high population, pluripotent ability, less immune rejection, and adjustable capacity to immune modulation and inflammation.
Rheumatoid arthritis (RA) is an autoimmune disease caused by systemic chronic inflammation that primarily attacks synovial joints, leading to articular cartilage degeneration, bone destruction and functional disability. Incidence of RA imparts a massive burden on health services worldwide and efforts to discover new medicines, such as TNF-α inhibitors and B-cell depleting therapies. However, these may have some side-effects and limited efficiency. Hence, it is important to investigate for development of new and more effective therapy for RA. On the basis of similarly physiological and genetic characteristics to human, research of porcine stem cells play an important role in experimental understandings in vitro and preclinical application to humans. Mesenchymal stem cells (MSCs) are characterized by unique properties, such as unlimited self-renewal, expression of MSCs specific cell surface antigens, alkaline phosphatase (AP) activity, high expression of early transcriptional factors, differentiation into mesenchymal lineages, anti-inflammatory and immunomodulatory capacity for clinical application. MSCs have also been established from various sources, however, those MSCs from different tissues were revealed different characteristics including therapeutic potential. For successful remedial effect of MSCs in clinical application, it is needed to comparatively investigate to various MSCs and select promising cell sources verified in vitro and in vivo experiments. The present study was therefore performed to characterize MSCs isolated from synovial sources, along with in vivo application into RA mice model, and comprised of three parts. Detailed researches were conducted to assess their stability of candidate reference genes in porcine MSCs for evaluating gene expression in further study (Part I), comparative characterization and gene expression against selected reference gene of MSCs from synovial tissues for selection of promising cell sources for clinical application (Part II), and finally comparative investigation of therapeutic potential in selected MSCs from synovial tissues in Rheumatoid arthritis (RA) animal model (Part III). In part I, because expression of reference genes varies depending on different conditions, finding out the most stable reference gene in porcine MSCs across before and after differentiation is prerequisite for reliable results in MSCs by qRT-PCR. MSCs derived from bone marrow (BMSCs), adipose (AMSCs) and skin (SMSCs) were isolated and in vitro differentiated into mesenchymal lineages such as adipocytes, osteocytes and chondrocytes. Twelve commonly used reference genes (18S, H2A, HPRT1, GAPDH, TBP, RPL4, PPIA, B2M, ACTB, YWHAZ, SDHA and HMBS) were investigated their threshold cycle (Ct values) by qRT-PCR. Without 18S and B2M, significant differences in average of Ct values were observed in ten genes in both before and after differentiation, consequently, these findings indicated the possibility that the expression patterns of each gene could be changed during differentiation process. Following analyzed stability of selected reference genes in MSCs before and after differentiation by geNorm software, the present study showed that HMBS, YWHAZ and TBP are revealed high stability in undifferentiated MSCs and differentiated MSCs into adipocytes, while HMBS, SDHA and TBP investigated high stability in differentiated MSCs into osteocytes and MSCs in before and after differentiation into whole lineages, and thus HMBS, SDHA and YWHAZ showed high stability in differentiated MSCs into chondrocytes. Moreover, Pearson’s correlation determined high correlation (r > 0.9) between the three most stable reference genes and optimal number of reference genes in each MSCs status. Consequently, HMBS, TBP, YWHAZ and SDHA were evaluated as optimal reference genes for precise normalization at target gene expression research in various status of porcine MSCs. Comprehensively, HMBS in the study is selected as a reference gene for normalization in further study because of stable expression across undifferentiated and differentiated MSCs. In part II, to evaluate the promising cell sources from synovial sources for clinical application, comparative characterization was investigated about distinctive features among bone marrow (BM-MSCs), synovium (SM-MSCs) and synovial fluid MSCs (SF-MSCs) from a single male pig. All three kinds of MSCs were positive for MSCs specific cell surface antigens, and SF-MSCs had less MHC class II expression. SF- or SM-MSC were showed high expression of Oct3/4 or Sox2, respectively, and investigated significantly increased proliferative potential than BM-MSCs. All MSCs were successfully differentiated into adipocytes, osteocytes, neurons and chondrocytes confirmed by cytochemical or immunocytochemical staining, and lineage specific gene expression analysis by qRT-PCR. Comprehensively, BM-MSCs presented high differentiation potential in adipogenesis and neurogenesis. All three kinds of MSCs expressed up-regulated osteogenic specific genes, however, SF-MSCs were evaluated the most stable gene expression and morphological patterns in chondrogenesis. Consequently, SF-MSCs were proposed the suitable source for clinical application to synovial disease animal model, due to easy acquisition and isolation, a good proliferative ability, high expression of early transcriptional factors, less expression of MHC class II and stable chondrogenic differentiation potential than BM-MSCs. In part III, based on recent studies that MSCs have immunomodulatory properties and immunosuppressive effects by direct cell to cell contact or by secreting soluble factors, MSCs derived from synovial tissues were evaluated the therapeutic potential of immunomodulation in RA animal model.
