This research was conducted to identify the biomechanical change in the lower limbs of person with total knee arthroplasty of the knee according to their gaits of different gait conditions. The subjects were 10 elderly person with TKR, 10 elderly patients diagnosed with degenerative osteoarthritis of the knee, and 10 healthy elderly persons without knee degenerative osteoarthritis. The first gaiting factor was that they had to walk with weight loads bearing 0%, 10%, 20% of their body weight placed evenly on the front and at the back of their trunks. The second gaiting factor was obstacle 0%, 15%, 30% walking with weight loads. Kinematic and kinetic variable was measured by Vicon system(Oxford Metrics, Oxford, England) and force platform to compare gait analysis. The results were as follow: Changes in spatiotemporal and kinetic variables on the weight-bearing gait were as follows. There was no significant difference in cadence, step length, stride length, step width and walking speed in elderly with TKR. Comparing with the elderly with the TKR and the elderly with osteoarthritis, there was a significant increase in step length and stride length. Comparing with the elderly with the TKR and the normal elderly, there was no significant difference in dorsiflxion of ankle and adduction of hip at the initial contact. Comparing with the elderly with the TKR and the elderly with osteoarthritis, there was a significant decrease in lateral rotation of hip and comparing with the elderly with the TKR and the normal elderly, there was a significant decrease in adduction of hip at the mid stance. Comparing with the elderly with the TKR and the elderly with osteoarthritis, there was a significant decrease in flexion, increase in internal rotation of hip and decrease in internal rotation of ankle at the preswing. Comparing with the elderly with the TKR and the elderly with osteoarthritis, there was a significant increase in adduction of knee, decrease in plantar flexion and increase in internal rotation of ankle and comparing with the elderly with the TKR and the normal elderly, there was a significant decrease in adduction at the midswing. Comparing with the elderly with the TKR and the elderly with osteoarthritis, there was a significant increase ground reaction force of lateral and posterior at first peak. Changes in spatiotemporal and kinetic variables on the weight-bearing gait with obstacle were as follows. Comparing with the elderly with the TKR and the elderly with osteoarthritis, there was a significant decrease in step length and stride length. Comparing with the elderly with the TKR and the elderly with osteoarthritis, there was a significant increase in lateral ground reaction force. Comparing with the elderly with the TKR and the normal elderly, there was a significant decrease in TC. The findings of this study indicate that after a long time after surgery, there was no significant difference in level walk between the elderly with TKR and the normal. In many areas, there was more significant differences between normal elderly and elderly with osteoarthritis, than between the elderly with osteoarthritis and the elderly with TKR. During the weight-bearing walking, the lower joint angles were different and during the obstacle walking elderly with TKR showed a strategy for center of gravity. Thus, in the case of long-term treatment for post-operative patients, there was no changing pattern of gait. But on the weight-bearing gait with obstacle, elderly with TKR felt difficult in maintaining balance because of the differences in temporal and spatial variables. In case of weight-bearing gait with obstacle, Elderly with TKR by the osteoarthritis has instability gait than normal elderly due to imbalance, it is not enough to complete the treatment. Thus, I suggest for elderly patients with TKR, as well as walking on level ground and under different conditions, developing other treatment methods to improve the walking ability.
