Lower back pain is a frequently occurrence among Emergency Medical Technicians(EMT) personnel. As one of the most common types of first aid given by EMTs, CPR has an important role in the pre-hospital chain of survival in emergency medical service. But, CPR is a very repetitively strenuous movement, so it may be a risk factor for low back pain. This study was conducted with a biomechanics analysis of EMTs’ trunk during CPR at abdominal drawing-in. The subjects were 15 EMTs, which performed hands-only CPR during 3 minutes after a 50minute resting period, then performed CPR at abdominal drawing-in during 3 minutes. Using a motion analysis and force plate, changes in trunk angle and ground reaction force were compared. Using an electromyogram measurement system and SkillReport changes in trunk muscle activation(triceps, latissimus dorsi, erector spinae, multifidus, gluteus maximus, pectoralis major, rectus abdominis, internal oblique-transverse abdominis, external oblique and rectus femoris) and depth and accuracy of CPR were compared. One-Way ANOVA and paired T test was employed to see the difference between-period and pre-post differences according to CPR type. When CPR was performed at abdominal drawing-in, trunk angle within sagittal plane was significantly decreased in event 1 and event 3, compression phase and decompression phase(p<.001). Trunk angle within transverse plane was significantly decreased in event 1(p<.001), event 2(p<.05) and event 3(p<.001). GRF(Fy) showed a significant difference in event 1 and event 3(p<.001). And GRF(Fx) was significantly increased in event 3(p<.001). When CPR at abdominal drawing-in performed, muscle activation showed a significant increase in triceps(p<.001), pectoralis major(p<.05), gluteus maximus(p<.05), rectus abdominis(p<.001), internal oblique-transverse abdominis(p<.001), external oblique(p<.001) and rectus femoris(p<.01) during compression phase and decompression phase. Also, muscle activation of latissimus dorsi, erector spinae and multifidus was increased but there were no statistic. However, as time passes, trunk muscles such as erector spinae, multifidus, internal oblique-transverse and external oblique was activated continuously. Accuracy and depth of CPR at abdominal drawing-in showed to be significantly higher than hands-only CPR. In conclusion, CPR at abdominal drawing-in showed a trunk stabilization by continuous muscle's activation. Also, muscles activation of arm and shoulder was increased thus there will be effectiveness to CPR that is related higher compression force delivery. Trunk angle was decreased by trunk muscle activation within sagittal plane as major direction of CPR, if EMT perform a CPR delivery for a long time, abdominal drawing-in maneuver will be effectiveness to reduce muscle fatigue, loss of endurance and trunk disturbance or instability. Also, GRF(Fy) showed an increase to anterior, this is mean center of body is loading to manikin consequently will be delivery great force. This study was performed by healthy EMTs, because it did not examine long time exercise effect. In the future, if efficient physiological method that allow trunk stabilization is possible to arrange and EMTs gain experience, when EMTs perform a procedure such as CPR there will be protection from lower back pain and other musculoskeletal disorders. Further studies are needed.
