The purposes of this study were to develop Korean versions of the transtheoretical model-based measures and to test transtheoretical model for explaining Korean adults’ smoking cessation. Self-reported questionnaire were used to collect data in May 1 to 20, 2014 from 450 ex-smokers and smokers in S city and J province. Data from 410 out of the 450 questionnaires were analyzed using SPSS 12.0 program and Amos 21.0 program. The several measures in English for assessing constructs in the transtheoretical model were translated into Korean via the translation and reverse-translation process, and the Korean versions were validated through expert content validation, exploratory factor analysis and confirmatory factor analysis. Finally, the 23-item Smoking Process of Change scale (five-factor hierarchial model), 15-item Smoking Decisional Balance scale (four-factor correlational model) and 15-item Smoking Temptation scale (three-factor hierarchial model) were validated. The transtheoretical model was not satisfactory in explaining Korean adults’ smoking cessation process which was assessed with the Korean versions of the original measures. Therefore a modified model was suggested. Data analysis revealed that the modified model consisted of two types of 6 variables: exogenous variables of experiential processes and behavioral processes, and endogenous variables of pro, cons, temptation, and stage of change. The results of the model fitness analysis were satisfactory (χ2=270.370, p<.000, df=55, χ2/df=4.913, GFI=.905, CFI=.916). The predictor variables of the modified model explained 49.0% of variance in smoking change of stage. The results of the hypotheses tests were as follows; (1) behavioral processes of change had significant effects on temptation and pros, and (2) three variables of behavioral processes, temptation, and pros had significant effects on stage of change and the behavioral processes showed the biggest effect. In Korean adults’ smoking cessation, it was found that the more the behavioral processes and the less the temptation and pros, the higher the stage of change. The results of this study showed that the modified transtheoretical model and its related measures in Korean are adequate to use while developing smoking cessation programs for Korean adults and evaluating its outcomes. Specifically it would be preferable for any smoking cessation intervention to be successful to include strategies enhancing behavioral processes and reducing temptation and pros of smoking.
