최근 생활수준의 향상, 서구식 식생활패턴으로의 변화 등으로 인해 당뇨병 유병률이 꾸준히 증가하는 추세에 있으며, 연령이 증가함에 따라 그 유병율도 급격히 증가하고 있다. 이에 따라 당뇨병에 대한 관리 및 예방의 필요성이 부각되고 있으며, 당뇨병 영양교육이 당뇨병 식사요법에 대한 지식, 식생활 태도 및 식행동 등을 개선시킬 수 있다는 연구결과도 일부 보고되고 있다. 특히 저혈당지수는 식사 후 당질의 흡수 속도를 나타내기 위해 제안된 개념으로 저혈당지수 식품의 섭취는 혈당조절에 매우 효과적이라는 연구보고도 있다. 이에 본 연구에서는 50세 이상 여성 노인 당뇨병 환자 46명을 대상으로 일반적특성, 생활습관 및 당뇨병 관련인자, 식습관 등을 분석한 후, 저혈당지수에 대한 영양교육을 4개월간(1회/월) 총 3회를 실시하여 자가 당뇨 조절행위 및 당뇨병 관련 지식의 변화 정도와 혈당개선 효과를 분석함으로써, 당뇨병 환자를 대상으로 한 저혈당 지수에 대한 영양교육의 효과와 그 중요성을 알아보았다. 연구 대상자는 대부분 과체중(23.91%), 비만(50.00%) 경향을 보이고, 당뇨병 관련인자로 질병 여부를 보면 고혈압을 동반한 경우가 91.30%였으며, 규칙적인 운동을 하는 경우는 19.57%로 낮게 나타났다. 음주와 흡연은 각각 2.17%로 거의 하지 않는 것으로 나타났다. 외식은 대상자의 54.35%가 거의 하지 않았고, 간식빈도는 하루 1일 2회(41.30%), 1일 1회(50.00%)로 하루 1-2회는 거의 하는 것으로 나타나 대체적으로 간식을 즐기는 편이였다. 간식 종류는 과일류(82.61%)에서 토마토, 수박, 참외를 주로 섭취하여 가장 많은 비율을 차지했고, 다음으로 음료류(63.04%)로 믹스커피를 하루 1-2잔을 섭취하는 것으로 나타났다. 그 외 감자와 옥수수 등(60.87%)을 섭취하는 것으로 나타나 계절적인 요인으로 볼 수 있으며, 식후 2시간 혈당을 영양교육 전과 후로 비교한 결과 208.35 mg/dl에서 174.09 mg/dl로 유의적으로 감소를 보였다(p=0.0040). 자가 당뇨 조절행위를 알아보고자 영양교육 전과 후로 설문조사를 한 결과 식사요법 관련 문항으로 식이요법을 어렵게 생각하는 비율이 낮아져서 유의적 차이를 보였으며(p<.0001), 처방된 약 복용여부는 교육 전과 후에 비슷한 결과를 보였으며, 운동여부는 변함없는 결과가 나타났다. 혈당조절에서는 교육 전에 식이요법이(50.00%), 가장 어려운 점으로 나타났고, 다음으로 운동요법(39.13%) 또한 어렵다고 하였으나 교육 후에는 식이요법보다(65.22%) 운동요법이(69.52%), 가장 높게 유의적으로 나타나 교육을 받은 후 식이요법을 긍정적으로 받아들이는 것으로 보여지는 결과이다. 당뇨병에 대한 지식을 객관적으로 평가하여 영양교육의 효과를 확인하기 위해 시행한 설문조사에서 저혈당지수식품 영양교육전과 교육 후에 당뇨병에 대한 지식변화를 총 7개 문항으로 조사한 결과 모든 문항에서 교육 후에 2.26 ± 1.60에서 5.43 ± 1.11로 유의하게(p<.0001) 높게 조사되어 영양교육에 효과가 크게 나타나는 결과이다. 3회에 걸쳐 실시한 저혈당지수에 관한 영양교육은 저혈당지수 식품의 유용성에 대한 인식과 실천도 등이 유의하게 개선되었고 식후 2시간 혈당이 유의하게 개선되므로, 당뇨환자의 식사관리에 유용한 교육임을 알 수 있었다.

For this reason, to manage and prevent of diabetes mellitus has been evident. Some studies reported diabetics nutritional education improve the patients’ dietary life, eating behavior and knowledge of diet therapy. Low Glycemic Index is the idea to refer to carbohydrate absorption speed. Some researches reported to intake Low Glycemic Index is highly efficient in control blood glucose. This study had analyzed 46 people who has had type2 diabetes mellitus and over 50 years old and the subject’s general characteristics, characteristics related diabetes, lifestyle and dietary habits. The subjects had received a low glycemic index nutritional eduction for 4-month(1time per month). The change of diabetes self-management, diabetes knowledge and improvement of blood glucose were shown an effect of the low glycemic index nutritional eduction to diabetics patients and importance. Most of the subjects had an ‘overweight(23.91%)’ or ‘obesity(50.00%)’. One of the biggest characteristics related diabetes is ‘hypertension(91.30%)’. In addition ‘regular exercise(19.57%)’, ‘smoking(2.17%)’ and ‘drinking(2.17%)’ had small proportion of the large numbers. The half of subjects(54.35%) chose ‘Not at all’ in ‘Eating out frequency’. Almost all of subjects(1time per day : 50.00%, 2times per day : 41.30%) had snack 1 or 2times per day. The most popular types of snack are ‘fruit(82.61%)’, ‘beverage’(63.04%), ‘potato and corn(60.87%)’. The snack’s type was strongly influenced by seasonal effects. Change in 2-hour blood glucose before and after low glycemic index nutrition education were significantly and practically decreased(p=0.0040). Change in the scores of the diabetes self-management before and after low glycemic index nutrition education were significantly and practically improved in the part of ‘diet therapy’(p<.0001), but the part of prescribed time taking medicine and regular exercise were at a standstill. The most difficult thing control blood glucose at pre-education had were ‘diet therapy(50.00%)’ and ‘exercise therapy(39.13%)’, but at post-education were ‘diet therapy(65.22%)’ and ‘exercise therapy(69.52%)’. Therefore, after the education the subjects positively accepted ‘diet therapy’. ‘Administration of medication of insulin’ had not so far been much alteration between pre-education and post-education. Besides ‘Frequent hospital visits and blood tests’ was stationary for a period of the education. We did a survey with questionnaires for verifying objectively efficacy of the low glycemic index nutritional eduction with 7 questions. Therethrough all of the question’s result got improved significantly and practically(p<.0001) after the education. The low glycemic index nutritional education improve the knowledge about utility of low glycemic index food and practice. Consequently, the education is helpful for diabetes mellitus patients’ meal management.
