ABSTRACT
This purpose of this study was to evaluate the effect of nutritional counseling for diabetic patients on their nutritional knowledge and attitude towards nutritional therapy in diabetes. We collected various information from 43 patients, who visited the Ulsan University Hospital between June 2013 and march 2014. Collected information includes demographic, lifestyle, dietary and clinical information, nutritional knowledge, and adherence levels to the recommendations. Patients’ characteristics were examined according to sex, dietary counseling, disease duration, and the repeated counseling, using chi-square test, paired t-test, independent t-test and ANOVA. The results of this study are summarized as follows. 1. The subjects’ mean age was about 43, and 69.8% of the subjects were 40 years old or older. 2. When obesity was classified according to BMI, only 27.9% of the subjects were underweight or normal, and 41.9% were obese. 3. After the nutritional counseling, the use of dietary therapy, exercise therapy, and drug treatment increased for treating diabetes. 4. After the nutritional counseling, self‐monitoring of blood glucose, exercise frequency, and exercise time increased significantly (p<0.05). 5. After the nutritional counseling, drinking frequency and alcohol intake per time decreased significantly (p<0.05), and smoking quantity also decreased significantly (p=0.01). 6. After the nutritional counseling, the total score of nutrition knowledge increased significantly (p=0.001), and especially the scores of knowledge about ‘food elevating blood glucose level,’ ‘fruit exchange,’ and ‘my daily prescribed calorie’ increased markedly (p<0.05), and the questions showing the highest correct‐answer rate were those about ‘basic principles of dietary therapy and the rate showed significant difference (p=0.02). 7. After the nutritional counseling, actual intake of calorie and protein compared to individual prescribed calorie decreased significantly and it was lower than the recommended intake (p<0.05). The intake of vitamin C and folic acid increased significantly (p<0.05). The subjects’ CPF ratio was generally equal to the recommended ratio both before and after the nutritional counseling. 8. In the comparison of practice of dietary life between men and women, men practiced ‘reducing simple sugar’ best while women practiced ‘sufficient intake of vegetable’ best. 9. In the comparison of obstacles to the performance of dietary therapy, the most difficult obstacle was ‘Don’t want to give inconvenience to the family or others’ and the second most one was ‘Cannot eat as I want in fear of the elevation of blood glucose level. 10. In the survey of nutrition intake according to gender, men took more iron than women both before and after the nutritional counseling and the difference was significant (p<0.05). After the nutritional counseling, women took significantly less calcium, vitamin C, vitamin B6, potassium, etc. than men (p<0.05). 11. As to disease duration, those whose disease duration was shorter than a year were more frequent in men than in women, and the mean disease duration was significantly shorter in men than in women (p=0.007). 12. As to the practice of dietary life according to disease duration, dietary life was practiced best by those whose disease duration was shorter than a year, and especially this group was better than the other two groups in ‘adequate amount of food intake,’ ‘regularity of diet,’ ‘observation of the nutritionist’s instructions’ (p<0.05). 13. Those with longer disease duration felt more difficult in the performance of dietary therapy, and there was no significant difference among the groups. 14. There was no change in knowledge resulting from additional nutritional counseling. In addition, the practice of dietary life was lowered, higher difficulty was felt in the performance of dietary therapy, and no significant difference was observed among the group. Summing up these results, diabetic patients who had received nutritional counseling showed improved dietary habits and a consequent decrease in BMI, and the score of nutrition knowledge increased significantly. Those with short disease duration showed a higher score of nutrition knowledge, and because of their good practice of dietary life they felt less difficult in the performance of dietary therapy. These results suggest that active nutritional counseling for patients in the early stage of diabetes give many positive helps to their management and health promotion. Those with long disease duration showed higher difficulty in the performance of dietary therapy and lower practice of dietary life, which may lead to lower quality of life. In nutritional counseling, therefore, it is considered most crucial to emphasize the importance of continuous management through ample encouragement and motivation.
