The purpose of this study was to evaluate the diabetic knowledge and self - management of diabetes mellitus and to evaluate the effect of nutritional counseling on patients with type 2 diabetes who visited a general hospital in Daejeon. This study was conducted on 70 type 2 diabetic patients who were admitted to the S general hospital in Daejeon city or who visited the outpatient clinic to participate in the study. Nutritional counseling was conducted three times in total. The first and third nutritional counseling sessions were face-to-face counseling sessions and the second nutritional counseling sessions were conducted by telephone counseling. In the first consultation, we were educated about how to calculate the daily calorie requirement of diabetic patients, the appropriate intake by food group, the importance and basic principle of diet therapy, and the foods that should be restricted. In the second counseling session, the contents of the first counseling session were reconfirmed and explained to the patients. In the third consultation, patients were taught about food substitution, food substitution, hypoglycemia and coping methods, and desirable menu for eating out. The questionnaires used in this study consisted of the general characteristics of the subjects, evaluation of knowledge about diabetes (knowledge of meals, complications), self - management activities of diabetes (diet, exercise therapy, pharmacotherapy, blood glucose check). This was used to assess diabetes knowledge level and self - management activity before and after counseling. There was a significant difference according to gender in the knowledge related to diabetes, as a result of analyzing the diabetic knowledge, self-care activities, and glycated hemoglobin according to the characteristics of the subjects. Self-management activities showed significant differences according to gender, economic level, occupation, age, education, duration of illness, and treatment methods. Those with high school or higher education showed lower HbA1c than those with lower than middle school education. In addition, the HbA1c of the woman was well controlled compared to the men. Diet related knowledge (t = -7.969, p = 0.000) and diet management (t = -8.731, p = 0.000) were significantly different before and after nutritional counseling. However, diabetes self - care (t = -1.842, p = 0.068) did not show any significant difference before and after nutritional counseling. In the biochemical test after diabetes education, the fasting blood sugar level was 191.0 mg/dl, before, 165.0 mg/dl after education, in HbA1c, 7.80% before and 7.49% after education, there was a difference in average value after education compared to before, but no statistically significant difference. As a result of comparing the percentage of correct answers and wrong ones before and after education according to the per individual or question of the knowledge evaluation, we found significant improvement in most of the dietary knowledge questions(p <0.01). This study showed that diabetes education could help to improve general knowledge about the disease, but we couldn’t find any significant differences in self-care activities and blood glucose control. Diabetes is a long-term care disease for a patients’lifetime, and the most crucial part of the treatment depends on how the patients care it for themselves. Therefore the patients should have sufficient knowledge about diabetes and they need to improve dietary management and self-care activity for the disease, and more effective education has to be provided for them. To do this, it is desirable to have the patients continue to receive diabetes education, not just one-time instruction.
