B 모드 초음파 스코어와 탄성 초음파 스코어, 재분류 스코어의 각각의 진단 능력을 receiver operating characteristic 곡선분석법으로 평가 하였다.
결 과:
B 모드 초음파 스코어 (2.6 대 1.7), 탄성 초음파 (1.7 대 0.8), 재분류 스코어 (3.3 대 1.6)의 평균치는 통계적으로 유의하게 악성 병변군에서 높았다 (p < 0.001). B-mode 초음파 스코어의 AUC값은 0.795 (95% CI, 0.707-0.866)이었으며 탄성 초음파 스코어의 AUC값은 0.852 (95% CI, 0.771-0.913), 재분류 스코어의 AUC값은 0.916 (95% CI, 0.847-0.960)이었다. 재분류 스코어의 AUC값은 B 모드 스코어의 값보다 통계적으로 유의하게 높았다 (p = 0.035). 1과 2사이의 컷오프 값을 기준으로 하여 B 모드 초음파 스코어와 재분류 스코어간의 민감도는 두 스코어에서 모두 100%로 차이를 보이지 않았으나 특이도는 B 모드 초음파 스코어에서 26.5%, 재분류 스코어에서 42.9%로 증가 하였다.
결 론:

Among them, 100 patients (mean age, 45.9 years; age range, 15-73 years) of 109 circumscribed breast masses (98 benign and 11 malignant; mean size, 11mm) were included in this study. Two radiologists retrospectively reviewed B-mode US and USE images in consensus without clinico-histologic information. Based on the American College of Radiology (ACR) Breast Imaging Reporting and Data System (BI-RADS), we determined the categories of B-mode US images and then assigned B-mode US scores from 1 to 5 corresponding to BI-RADs category 2 or 3, 4a, 4b, 4c and 5. USE scores were assessed by using a 3-point scale (negative, 0; equivocal, 1; positive, 2). We reclassified B-mode US scores considering USE scores. When USE score of the lesion was 0 or 2, we downgraded or upgraded B-mode US score, respectively. Reclassified B-mode US score was defined as ‘reclassification score’. The mean scores of benign and malignant lesions were compared with Student’s t-test. The diagnostic performances of B-mode US, USE and reclassification scorings were compared using receiver operating characteristic (ROC) curve analysis.
Results:
Mean B-mode US score (2.6 vs. 1.7), USE score (1.7 vs. 0.8) and reclassification score (3.3 vs. 1.6) were significantly higher in malignancy than in benign lesions (p < 0.001). The AUC were 0.795 (95% CI, 0.707–0.866) for B-mode US scoring, 0.852 (95% CI, 0.771–0.913) for USE scoring, and 0.916 (95% CI, 0.847–0.960) for reclassification scoring. The AUC of reclassification scoring was significantly higher than that of B-mode US scoring (p = 0.035). At the cutoff value between 1 and 2, the sensitivity and specificity of B-mode scoring were 100% and 26.5%, respectively, and those of reclassification scoring were 100% and 42.9%, respectively. After reclassification, the specificity was increased from 26.5% to 42.9% without sensitivity loss (100%)
