최근 전 세계적으로 고령화 진입속도가 급격히 가속화 되면서 노인성 질환의 발병률이 급격하게 증가하고 있다. 골다공증 (Osteoprosis)은 대표적인 노인성, 여성 질환으로 주로 폐경 이후 여성들이 골다공증으로 인해 많은 고통을 겪고 있다. 골 (Bone)은 파골세포 (Osteoclast)에 의한 골 흡수와 조골세포 (Osteoblast)에 의한 골 형성을 통한 Bone remodeling 과정을 통해 항상성을 유지하게 된다. 하지만 여성의 경우 폐경 이후 체내 에스트로겐 분비가 저하되면서 파골세포의 활성이 급격히 증가하게 되어 골 흡수와 골 형성간의 균형이 붕괴되어 약한 충격에도 쉽게 골절이 일어날 수 있는 골다공증으로 진행되게 된다. 폐경 이후 급격한 골밀도 감소를 개선하데 있어 파골세포에 의한 골 흡수의 조절은 매우 중요하며, 현재 많은 골다공증 환자에게 처방되고 있는 대표적인 곱 흡수 억제제인 bisphosphonate 제재의 경우 과도한 파골세포 활성 억제로 인하여 턱뼈가 괴사하는 osteonecrosis of the jaw와 유방암, 자궁암 등 여러 가지 심각한 부작용을 나타내고 있는 것으로 보고되었다. 본 연구에서 사철쑥 (Artemisia capillaris Thunberg) 추출물과 사철쑥의 주요 활성물질인 scoparone을 이용하여 In vitro와 In vivo 상에서 골다공증 예방 효과 및 그 작용기전에 대한 연구를 수행하였다. In vitro 상에서 사철쑥 추출물과 scoparone을 조골세포인 MC3T3-E1 subclone 4에 처리하였을 때 조골세포 분화 marker중 하나인 ALP의 활성을 농도 의존적으로 증가 시켰으며, 세포의 무기질화 정도를 나타내는 Alizarin Red S staining결과에서도 칼슘 축적이 증가하여 석회화 (Mineralization)를 증가시킴을 확인하였다. RANKL로 파골세포 분화를 유도한 RAW 264.7세포에 사철쑥 및 scoaprone을 처리하여 tartrate-resistant acid phosphatase (TRAP) 활성과 염색 결과 농도 의존적으로 TRAP 활성과 TRAP양성의 다핵세포인 파골세포의 수를 감소시켰다. 난소절제를 통해 폐경을 유도한 SD rat 모델을 이용한 12주간의 In vivo 동물실험 결과 난소절제로 유도된 골밀도 (BMD) 및 골함량 (BMC)의 감소를 사철쑥 추출물 투여군 (ACE, 125 or 250 mg/kg/day)에서 유의적으로 감소시켰으며, 양성대조군으로 이용한 17β-estradiol (E2, 50 μg/kg/day)와 비슷한 정도의 골밀도 감소 억제활성을 나타내었다. 혈액 내 골 형성 지표물질인 procollagen type I c-terminal peptide (PICP), osteoprotegerin (OPG), bone alkaline phosphatase (BALP)의 농도를 유의적으로 증가시켰으며, 골 흡수 지표물질인 receptor activator of NF-κB ligand (RANKL), TRAP, cross-linked C-terminal telopeptide of type I collagen (CTx)의 농도가 유의적으로 경감된 것을 확인할 수 있었다. 생체 내에서 발생하는 reactive oxygen species (ROS)는 정상적인 상태에서 체내 항산화 효소에 의해 소거되지만, 과도하게 생성된 ROS는 산화적스트레스를 유발하여 관련 질병을 유도하게 된다. 또한 최근 보고에 따르면 낮은 농도의 ROS는 세포내 2차 신호전달자로 세포내 신호전달 체계를 조절하는 것으로 나타났으며, 항산화 물질을 이용하여 산화스트레스를 감소시키는 것은 ROS에 의한 세포손상 및 세포내 신호전달을 조절하여 각 종 질병을 예방할 수 있는 식이전략 중 하나이다. 파골세포 분화 과정 중 발생하는 ROS는 파골세포의 분화 신호전달을 조절하는 것으로 여러 연구자들에 의해 밝혀졌으며, 사철쑥의 활성물질인 scoparone이 어떤 경로를 통해 파골세포 분화를 억제하는지 확인하였다. HepG2 세포에서 AAPH 그리고 H2O2로 유도된 산화스트레스를 scoparone이 농도 의존적으로 감소시켜 세포내 항산화 활성을 가지는 것을 확인하였다. 파골세포 분화 과정 중 발생하는 ROS를 분석한 결과 총 5일 간의 분화과정 중 2일 차에서 가장 크게 증가하는 것을 확인하였다. 2일간의 파골세포 분화 과정에 scoparone을 처리한 결과 TRAF6/s-Src-/PI3K 신호전달을 조절하여 NADPHoxidase 1 (Nox1)의 활성을 감소시켰으며, 이를 통해 분화과정 중 발생하는 superoxide anion을 포함하는 ROS를 감소시키는 것을 확인하였다. 또한, superoxide dismutase 1 (SOD1) 그리고 catalase (CAT)과 같은 phase II antioxidant enzyme의 발현을 증가시키고, 파골세포 분화 과정 중 mitochondria membrane electron transport chain system을 통해 발생하는 ROS의 발생 또한 조절하는 것으로 확인되었다. 이상의 연구결과는 사철쑥 추출물은 사철쑥의 주요 활성물질인 scoparone이 파골세포 분화과정 중 발생하는 ROS를 3가지 기전 (Mechanism 1: NADPH oxidase1 complex 억제, Mechanism 2: Phase II antioxidant enzyme 발현 증가, Mechanism 3: Mitochondria membrane electron transport chain system 억제)을 통해 조절하여 파골세포에 의한 골 흡수를 억제하며, 이를 통해 동물 모델에서 폐경 이후 발생하는 골밀도의 감소를 억제하는 것을 확인할 수 있었다. 사철쑥 추출물은 골 흡수를 억제함으로 골 흡수와 관련된 골격질환들을 예방할 수 있는 건강기능식품소재로서 효과적으로 사용될 수 있을 것으로 사료된다

Oxidative stress is a crucial pathogenic factor in the development of osteoporosis. The present study was conducted to investigate the effect of Artemisia capillaris extract (ACE) and its main compound scoparone on osteoclast differentiation and function in vitro, and bone mass, biomarkers in serum of bone formation and resorption in ovariectomized SD rats as a postmenopausal bone loss model. In addition, anti-osteoporotic effect mechanism of scopaorne was examined in RANKL-induced osteoclastic RAW 264.7 cells. The antioxidant ACE were investigated using a in vitro and cellular antioxidant capacity assay system. In this study, ACE was dose-dependently increased peroxyl-radical scavenging and reducing capacity, and decreased to the intracellular oxidative stress caused by AAPH or Cu2+ in HepG2 cells. Also, ACE showed that stimulate osteoblast differentiation including ALP and mineralization activity and suppress osteoclast differentiation. This results indicate that the inhibitory effect of ACE on osteoclast differentiation may be due to the its antioxidant potential. To examine the effect of ACE on in vivo bone loss, OVA SD rats were administered with ACE with two different doses (125 mg/kg or 250 mg/kg). Administration of ACE caused a significant increase in BMD and BMC in right femurs, compared with sham group. In serum bone formation related markers analysis, as compared with sham group, the OVA group showed decreased PICP, BALP, and OPG levels. Compared with OVA group, OVA SD rats administered with ACE 250 mg/kg had a significantly lower markers level with almost similar return to sham group. In bone resorption markers such as RANKL, TRAP, and CTx analysis, ACE administrated groups showed decrease in serum levels compared with OVA group. The present study demostrated that ACE prevented the decrease of bone mass induced OVA, especially in femurs by suppression of bone turnover. The suppressive effect of scoparone on the ROS level during osteoclast differentiation of RANKL-induced RAW 264.7 cells was investigated after confirming its cellular antioxidant capacity in HepG2 cells, and it was determined how the ROS level can be controlled in view of its production and scavenging by scoparone. The intracellular ROS level can be maintained as a result ROS generation and removal. In this study, it was confirmed that scoparone suppresses osteoclast differentiation of RANKL-treated RAW 264.7 cells through attenuating the ROS level. Whether scoaprone can inhibit the translation and activation of NOX1 and the disruption of the mitochondrial electron transport chain system and whether scoparone can induce the expression of phase II antioxidant enzymes, such as SOD1 and catalase, were investigated. Although, scopaorne may prevent and improve various bone diseases such as osteoporosis resulting from imbalance between bone formation and bone resorption through depressing osteoclast differentiation form progenitor cells. Therefore, ACE could be used for the development of functional foods to prevent menopausal osteoporosis in women.
